Histopathologic Diagnosis And Classification Of Prostate Adenocarcinoma: Biologic Significance, Jill M. Peters, John D. Crissman

Early diagnosis and accurate, biologically meaningful classification of prostate neoplasia remain important goals. The relation of evolving clinicopathologic concepts of histologic appearances to potential tumor progression is a major advance in classification of prostatic neoplasia. The criteria for recognizing incidental or "occult" stage A-l adenocarcinomas remain problematic in diagnosis, and focal neoplasms with little or no propensity to progression must be differentiated from cancers with a high likelihood of aggressive behavior. Current histologic grading systems in classifying prostate adenocarcinoma accurately identify two cancer subsets: 1) focal well differentiated tumors which rarely progress, and 2) diffuse poorly differentiated tumors which invariably …

Urothelial neoplasia is a unique cancer in that it consists of a spectrum of tumors with different biologic behaviors. The most common urothelial neoplasm is the low grade superficial papillary carcinoma or papilloma which may recur numerous times but does not result in significant morbidity or mortality. A variant of the superficial papillary carcinoma, which represents approximately 10% of the tumors, is the noninvasive papillary neoplasm which progresses to a less differentiated invasive transitional cell carcinoma (TCC). Effect Of Cyclosporine On The Rate Of Renal Function Recovery After Renal Transplantation, Francis Dumler

To assess the effect of cyclosporine therapy on the rate of renal function recovery after renal transplantation, patients with no clinical evidence of rejection and who were treated with either cyclosporine or azathioprine in addition to steroid therapy were studied (n = 74). Of the patients with immediate renal function (n = 57), those receiving organs from living, related donors had a faster recovery rate of glomerular filtration than patients with cadaveric grafts (azathioprine, 15 ± 2 versus 7 ± 1 mL/min/day, P = 0.0001; cyclosporine, 14 ± 3 versus 6 ± 1 mL/min/day, P = 0.001). We have identified six cases of quinidine-induced immune thrombocytopenia based on clinical evidence and in association with elevated amounts of platelet surface IgG. The degree of thrombocytopenia did not correlate with severity of clinical symptoms, nor did it predict the amount of IgG on the platelet surface. Three of the patients recovered promptly after drug discontinuation alone whereas the other patients received additional corticosteroid therapy. The clinical presentation, mode of diagnosis, and therapeutic considerations in the treatment of drug-induced thrombocytopenia are discussed.

Surgical Management Of Spinal Epidural Disease: An Update, Jack P. Rock, Dale V. Hoekstra, Henry H. Schmidek

Management of spinal cord compression from metastatic malignant disease remains unsatisfactory. Results of surgical decompression are at best less than those of radiation therapy alone. However, new surgical approaches now focus on removing the anterior-situated tumor tissue which produces neural compression in about 85% of the cases. The results of these procedures that allow removal of the ventrally compressing tumor show significant improvement in the management of patients with spinal epidural disease. We review the surgical strategy of these new approaches and the attendant results.

Three patients with fever and pneumonitis were found to have pulmonary valve vegetations by echocardiography. Pulsed and color flow Doppler studies demonstrated mild to moderate pulmonary valve insufficiency in these patients. All patients responded to the appropriate antibiotics without requiring valve surgery. Echocardiography is of value in diagnosing pulmonary valve endocarditis and should be considered in every patient with recurrent pneumonitis.

We report the case of a young man with an acute infarction of the left putamen and caudate nucleus, whose symptoms appeared six hours after intranasal use of approximately 0.5 g of cocaine hydrochloride. It seems probable that in this patient cocaine consumption played a role in the development of stroke.

ในระหว่างที่ทำการศึกษาประสิทธิภาพของวัคซีนกาฬโรคเป็ด ได้เกิดการระบาด ของโรคกาฬโรคเป็ดขึ้น โดยการเกิดโรคเกิดหลังการทดลอง ในวันที่ 6 หลังการฉีดวัคซีน ครั้งที่ 2 ซึ่งเป็นวันที่เป็ดเริ่มตาย จึงได้แบ่งเป็ดที่ไม่ได้รับวัคซีนมาก่อน และเลี้ยงรวมกันกับ เป็ดที่เป็นโรคออกเป็น 2 กลุ่ม กลุ่มแรกมีเป็ด 267 ตัว นำมาฉีดด้วยวัคซีนกาฬโรคเป็ดชนิด เชื้อเป็น ที่เหลือจำนวน 64 ตัว ไม่ได้รับการฉีดวัคซีน เป็ดทุกตัวเลี้ยงรวมกันในเจ้า ปรากฏ ว่าเป็นกลุ่มที่ฉีดวัคซีนจะตาย 54% ส่วนกลุ่มที่ไม่ได้รับวัคซีนเลยตาย 97% จากข้อมูลทางระบาดวิทยาชี้ให้เห็นว่า สาเหตุของการระบาดเกิดจากวัคซีนเชื้อตายที่ฆ่าเชื้อไม่หมด ผลการ ศึกษาสรุปได้ว่าการใช้วัคซีนเชื้อเป็นหลังการระบาดของโรคตั้งแต่เนิ่น ๆ จะป้องกันโรคได้บางส่วนเท่านั้น

สุกรพันธุ์ผสม อายุ 45 วัน จำนวน 3 ราย รายแรกแสดงอาการป่วย ซึม เบื่อ อาหาร อุจจาระเหลวสีเหลืองเป็นน้ำ แล้วตาย อัตราป่วยและตาย 13.04% (3/23) ผลการชันสูตรซากพบกระเพาะอาหารเกิดการทะลุและมีพยาธิไส้เดือนจำนวน 2 ตัวร่วมอยู่ สุกรรายที่ 2 แสดงอาการผอม เบื่ออาหาร ซีด มีอาการดีซ่านและตาย อัตราป่วยและตาย 3.33% (1/30) ผลการตรวจซากพบพยาธิไส้เดือนอุดตันอยู่ภายในท่อน้ำดี สุกรรายที่สามป่วยโดยถ่ายอุจจาระมีเลือดปนออกมาแล้วตาย อัตราป่วย 100% (20/20) และอัตราตาย 30% (6/20) ตรวจซากพบพยาธิไส้เดือนจำนวนมากอุดตันภายในลำไส้เล็ก

กระต่ายจำนวน 64 ตัว เพศผู้และเมียอย่างละเท่ากัน อายุระหว่าง 4 เดือน ถึง 2 ปี จากตลาดและผู้เลี้ยงในเขตกรุงเทพมหานคร นำมาเจาะเลือดจากหัวใจโดยไม่ได้ ทำการสลบสัตว์ เพื่อศึกษาหาค่าโลหิตวิทยาได้ค่าเฉลี่ยต่าง ๆ ดังนี้ ค่าฮีมาโตคริต 36.13 ± 4.45% ค่าฮีโมโกลบิน 11.62 ± 1.50 กรัม จำนวนเม็ดเลือดแดง 5.89 ± 0.79 X 106 เม็ดต่อเลือดหนึ่ง ลบ.มม. จำนวนเม็ดเลือดขาว 10.36 ± 3.17 X 103 เม็ดต่อเลือดหนึ่ง ลบ.มม. จำนวนเม็ดเลือดขาวชนิดนิวโทรฟิล 54.06 ± 9.47% ลิมโฟไซต์ 45.31 ± 9.82% โมโนไซต์ 0.25 ± 0.47% อีโอซิโนฟิล 0.41 ± 0.68% และเบโซฟิล 0.09 ± 0.29% โดยที่ค่าเหล่านี้ไม่มีความแตกต่างในระหว่างเพศอย่างมีนัยสำคัญทางสถิติ (P > 0.05)

ผลการตรวจซากสุกรป่วยที่มีอาการอาเจียนและท้องร่วงรายหนึ่งพบว่ารอยโรคที่ เห็นด้วยตาเปล่า รอยโรคที่เห็นจากกล้องจุลทรรศน์ และผลทางโลหิตวิทยา สนับสนุนการเป็น โรคกระเพาะอาหารอักเสบที่เกิดจากเชื้อราที่มีลักษณะของโรค Mucormycosis พบแผลหลุม ขนาดใหญ่ในกระเพาะอาหารส่วน fundus และเมื่อตรวจดูด้วยกล้องจุลทรรศน์พบว่าภายใน แผลหลุมดังกล่าวมีเชื้อราที่มีลักษณะ Coarse, branching, non-septate hyphae มากมายซึ่งเชื้อราเหล่านี้ได้เข้าทำลายผนังเส้นเลือดในบริเวณแผลและทำให้เกิด thrombosis ด้วย ผลการตรวจทางโลหิตวิทยาพบสภาพ leukocytosis

ศึกษาการเจริญเติบโตของกระต่ายขุนหลังหย่านมเมื่ออายุ 6 สัปดาห์ (10) ถึงส่งตลาดเมื่ออายุ 14 สัปดาห์ (W8) เป็นเวลา 8 สัปดาห์ ในกระต่ายจำนวน 110 ตัว เกิดจากแม่พันธุ์ 4 กลุ่มพันธุ์คือ นิวซีแลนด์ไวท์ (NZW) พื้นเมือง (N) ลูกผสมระหว่างนิวซีแลนด์ไวท์กับกระต่ายพื้นเมืองทั้ง 2 แบบ (NXNZW และ NZWXN) ผสมกับพ่อพันธุ์นิวซีแลนด์ไวท์ พบว่ากลุ่มพันธุ์มีผลต่อน้ำหนักเริ่มต้น และส่งตลาด, น้ำหนักเพิ่มและอัตราการเจริญเติบโตหลังหย่านมของกระต่าย และประสิทธิภาพการเปลี่ยนอาหารเป็นเนื้อ (P < .05) แต่ไม่มีผลต่อ ปริมาณอาหารขึ้นที่กระต่ายกิน ส่วนเพศไม่มีผลต่อน้ำหนักและลักษณะการเจริญเติบโตหลังหย่านม และไม่พบปฏิกิริยาร่วมระหว่างกลุ่มพันธุ์กับเพศของกระต่าย กระต่ายที่มีน้ำหนักหย่านม มากจะโตกว่ากระต่ายที่มีน้ำหนักหย่านมน้อยกว่าเมื่อส่งตลาดอายุเท่ากัน (P < .01) กระต่าย 100% นิวซีแลนด์ไวท์มีน้ำหนักตัวเพิ่ม และอัตราการเจริญเติบโตต่อวันต่ำกว่ากระต่ายลูกผสม 50 และ 75% นิวซีแลนด์ไวท์ (P < .05) และมีประสิทธิภาพในการเปลี่ยนอาหารเป็น เนื้อต่ำกว่ากระต่าย 75% นิวซีแลนด์ไวท์แบบ NZWXNXNZW) (P < .05) เมื่อเปรียบเทียบ ระหว่างกระต่าย 100 กับ 75% นิวซีแลนด์ไวท์ ปรากฏว่า กระต่ายขุน 75% นิวซีแลนด์ไวท์ มีน้ำหนักส่งตลาดมากกว่า กินอาหารน้อยกว่า แต่มีการเพิ่มน้ำหนักตัว และอัตราการเจริญ เติบโตสูงกว่าและประสิทธิภาพการเปลี่ยนอาหารเป็นเนื้อดีกว่ากระต่าย 100% นิวซีแลนด์ไวท์ (P < .05) ส่วนกระต่าย 50 และ 75% นิวซีแลนด์ไวท์ไม่แตกต่างกันในลักษณะการเจริญเติบโต และประสิทธิภาพการเปลี่ยนอาหารเป็นเนื้อ และเมื่อเปรียบเทียบระหว่าง กระต่าย 75% นิวซีแลนด์ไวท์ 2 กลุ่มคือ NZW X (NXNZW) และ NZW X (NZWXN) ก็ได้ผลเช่นเดียวกัน ยกเว้นน้ำหนักหย่านม ดังนั้นการผลิตกระต่ายขุนจะใช้ระดับของนิวซีแลนด์ไวท์ 50 หรือ 75% ก็ได้แล้วแต่ความเหมาะสมของระบบการผลิต ถ้าใช้แม่พันธุ์ลูกผสม การผลิตกระต่ายขุน แบบ NZW X (NXNZW) มีแนวโน้มว่าจะมีประสิทธิภาพการเปลี่ยนอาหารสูงสุด
