Journal of the American College of Dentists

Dentistry’s evolution as a profession reflects both scientific progress and enduring tensions between commercial and service-oriented models of care. Despite long-standing commitments to ethical education, dental training continues to privilege technical mastery over professional identity formation, leaving some future practitioners underprepared for the ethical and relational complexities of clinical practice. This paper situates these challenges within the historical separation of dentistry and medicine and argues that integrating the health humanities into dental education can strengthen professional identity formation and clinical ethics competency. Using a case study, we demonstrate how biases, professional obligations, and patient trust intersect in practice. We introduce …

Background Cutaneous larva migrans is a serpiginous helminthic infection common among certain occupations dealing with contaminated soil especially in warm climate. Most common predilections include the feet, legs, and buttocks with only a fraction of cases found on the trunk and upper extremities.

Case Presentation A rare scapular cutaneous larva migrans manifestation on a thirty-six years old sand porter with high occupational exposure was described. Physical examination revealed progressive pruritic and warm rash in a serpiginous pattern on the left shoulder.

Background Toll road workers are exposed to traffic noise and lead, both of which may increase the risk of sensorineural hearing loss (SNHL). This study aimed to examine the association between noise and lead exposure and the occurrence of SNHL among toll road workers.

Methods A cross-sectional study was conducted using secondary data derived from medical check-up records of 1,445 toll road workers in 2022. Background Fatigue is a recognized contributor to impaired performance and increased occupational risk in safety-critical industries. Although various assessment tools are available, their real-world implementation and role in operational decision-making remain unclear.

Methods This scoping review mapped fatigue assessment methods in workplace settings and evaluated their application in operational contexts. A systematic search of PubMed and Google Scholar identified 693 records, of which 21 studies met the inclusion criteria for real-world occupational settings.

A Narrative Review Of The Central Neural Control Of Swallowing And Its Implications For Dysphagia Rehabilitation, Kuo-Chang Wei, Tyng-Guey Wang, Ming-Yen Hsiao

Swallowing is controlled by a distributed neural network spanning the brainstem central pattern generator, subcortical modulatory circuits, and cortical regions responsible for volitional planning and sensorimotor integration. Evidence from lesion studies, neuroimaging, and neurophysiology demonstrates that dysphagia arises from disruptions at different nodes of this network, producing lesion-specific impairment profiles rather than a uniform swallowing deficit. This network-based perspective has shifted rehabilitation from predominantly compensatory management toward mechanism-driven interventions aimed at restoring physiological function. Purpose: Despite an important need for health care services targeting pediatric dysphagia, there exist a number of barriers to doing so, including workforce competence. The current study surveyed practicing Speech-Language Pathologists to determine (1) the importance of pediatric dysphagia in their caseload, (2) the adequacy of existing educational resources, and (3) perceived barriers to providing quality care in Taiwan.

Methods: An anonymous survey was completed by Speech-Language Pathologists working in Taiwan. Responses were analyzed with respect to current work setting, experience, preparation, and perceived barriers.

Selecting an appropriate non-instrumental dysphagia screening tool is a complex process. This scoping review aimed to identify dysphagia screening tools with established reliability and validity, and to develop administrator-based flowcharts to guide tool selection across different clinical populations. A scoping review was conducted using studies identified in CINAHL, Embase, PsycINFO, PubMed, and Web of Science. Our search was guided by five keywords: “dysphagia,” “deglutition,” “screening,” “test,” and “measure.” Original studies about the development and validation of non-instrumental dysphagia screening tools were selected. Objective: To investigate the impact of serum vitamin D levels on marginal bone loss (MBL) in patients restored with unsplinted-mandibular-implant overdentures using LOCATOR attachments. Materials and Methods: Thirty participants rehabilitated with three-implant-mandibular overdentures were included. Based on serum vitamin D at the time of prosthetic loading, participants were categorized into two equal groups: a test group with deficient vitamin D (< 20 ng/mL) and a control group with sufficient levels (≥30 ng/mL). Patients with borderline levels (21–29 ng/mL) were excluded. Peri-implant vertical (VBL) and horizontal bone loss (HBL) were measured at baseline (T0), 6 months (T6), and 12 months (T12). Cephalometric Norms And Sexual Dimorphism In The Tanzanian Population: A Comparative Study With Caucasian And West African Norms, Ali K. Hamad, Ferdinand M. Machibya, Matilda M. Mlangwa, David N. Ngassapa

Objective: This study aimed to establish cephalometric norms for the Tanzanian population, assess sexual dimorphism in cephalometric measurements, and compare these findings with established Caucasian and West African norms to enhance orthodontic diagnosis and treatment planning. Materials and Methods: A crosssectional study was conducted on 309 subjects (142 males, 167 females) at Muhimbili University Dental Clinic. Cephalometric radiographs were digitally traced using Quick Ceph Studio. Reliability was assessed using the inter-class correlation coefficient. The Kolmogorov-Smirnov test checked normality, while one-sample t-tests and Student’s ttests (p < 0.05) were used for comparisons. Data were analyzed with RStudio. Results: Tanzanians showed significantly smaller craniofacial measurements than Caucasians. Males had larger skeletal dimensions than females. Compared to West Africans, Tanzanians had generally higher values, with a few exceptions. Conclusion: Tanzanians display distinct craniofacial characteristics, differing from both Caucasians and West Africans. These findings support the need for population-specific cephalometric norms to guide accurate orthodontic diagnosis and treatment.

Purpose: To evaluate, both clinically and radiographically, the use of extended platelet-rich fibrin versus a pericardium membrane in mandibular ridge splitting accompanied by immediate implant placement.

Materials and Methods: A total of sixteen implants were placed in eight patients with horizontally atrophic posterior mandibular ridges measuring 3–6 mm in width were randomly assigned to two groups with equal numbers. Integrated behavioral health (IBH) is recognized as the standard for holistic healthcare, addressing the intersection of mental and physical health. As IBH gains prominence, graduate healthcare programs must equip students with strategies to meet its demands. However, limited research exists on training methods that address diverse professional identities and the unique needs of graduate students. Our novel IBH training incorporates interprofessional education (IPE) through modules and simulations grounded in Bandura’s (2001) social cognitive theory. Background. Osteogenesis Imperfecta (OI) is a rare disorder caused by variations in collagen. Clinical manifestations include multiple fractures, short stature, scoliosis, blue sclera, hearing loss, and opalescent teeth. The objective of this cross-sectional study was to evaluate associations between systemic and oral factors with tooth loss in a retrospective study of periodontal maintenance patients.

Background. We evaluated perioperative intrathecal morphine (ITM) compared with placebo or standard postoperative pain strategies in adults undergoing elective posterior lumbar fusion.

Methods. A systematic search of EMBASE, MEDLINE, the Cochrane Library, and Google Scholar (December 12, 2023) identified studies involving ITM in posterior lumbar fusion. Eligible patients were ≥18 years. Two reviewers extracted outcomes and assessed non-randomized studies with the Newcastle- Ottawa scale. Odds ratios (OR) and mean differences (MD) with 95% confidence intervals (CI) were calculated using fixed- and random-effects models.

Background. Pyogenic granulomas (PGs) are benign vascular lesions commonly encountered in primary care settings. They are often linked to trauma, hormonal changes, infections, or medications. While traditional treatments such as excision, curettage, and silver nitrate application are the treatment of choice, recurrence is possible, and they may result in discomfort, anxiety, or scarring. This poem explores the complex intersection of memory, identity and the challenges imposed by Lewy body dementia, using the metaphor of a lost expedition traversing an isolated icy landscape. A profound sense of solitude reflects the disconnection that accompanies the dementia and tremors that follow, while also highlighting the danger of memory loss with mythological sirens representing its perilous allure (with some inspiration from The Odyssey). Within the United States (US), colorectal cancer (CRC) remains the fourth most common malignancy and is responsible for over 50 000 deaths annually, second only to lung cancer in cancer mortality. Despite increased public awareness and increasing screening rates, the US remains far below the 80% screening mark set by the National Colorectal Cancer Roundtable at 59%. The addition of non-invasive blood-based tests for CRC screening could potentially increase screening rates and lower CRC deaths.

Pessaries are commonly used to treat pelvic organ prolapse. While these devices generally pose negligible risk, serious morbidity can be associated with neglected use.

Case Presentation

A 91-year-old woman began to experience acute pain related to an impacted pessary. An Exam in the operating room under anesthesia revealed that the pessary was deeply embedded within the vaginal canal and was noted to have a hard metal wire under a rubber exterior. Staphylococcal scalded skin syndrome (SSSS) is a dermatologic emergency that most commonly affects young children, requiring prompt recognition and treatment to avoid severe symptoms and fatal sequelae. It is characterized by a desquamation of the skin caused by exotoxins produced by the Staphylococcus aureus species and may present differently in melanated skin.

The thin, vascularized skin of the eyelids makes them prone to irritation and discoloration, potentially revealing underlying issues, such as subcutaneous deposits, and an abundance of pigments like carotenoids and lipofuscins. First described in 2008 by Assouly et al, orange palpebral spots (OPS) present as painless orange-yellow lesions on the upper eyelids, primarily affecting White women with Fitzpatrick skin types I to III. Lichen planus pigmentosus inversus (LPPI) is a rare variant of lichen planus pigmentosus (LPP), characterized by sharply defined violaceous-brown to gray macules, papules, patches, or plaques primarily affecting the intertriginous and flexural areas. Unlike LPP, which predominantly affects Fitzpatrick skin types III to IV in sun-exposed areas, LPPI spares these regions and has been predominantly reported in White and Asian patients.

Hughes-Stovin Syndrome (HSS) is a rare vasculitis characterized by pulmonary artery aneurysms (PAAs) and peripheral venous thrombosis. Anticoagulation, the standard treatment for venous thrombosis, is often contraindicated in HSS due to the risk of fatal pulmonary hemorrhage, especially since mortality rates of patients with a history of hemoptysis are reported at 50%-100% from PAA ruptures, yet treatment guidelines are limited due to the condition’s rarity with fewer than 90 reported cases globally.
