Aim: Overactive bladder (OAB) and detrusor overactivity (DO) may interfere because one may cause the other. Not all patients with OAB have DO, neither do all patients with DO have OAB. Aim: Familial Mediterranean fever is an autosomal recessive disease affecting primarily populations surrounding the Mediterranean basin. The aim of this study was to find the distribution of MEFV gene mutations in a group of Egyptian patients with FMF and to evaluate any predictive genotype-phenotype correlation in this group of patients. Materials and Methods: The study included 112 patients (59 males and 53 females). A Simple Surgical Technique For Repair Of Iridodialysis, Şengül Özdek, Mehmet Cüneyt Özmen

A simple surgical technique is described for repair of iridodialysis and a traumatic iridodialysis case is presented. In this technique, a 10-0 polypropylene suture is used for repair. The knot is left in the anterior chamber to avoid suture related complications such as erosion, patient discomfort, and infection. This technique can be a safe and easy way to repair large iridodialysis.

Linear and whorled nevoid hypermelanosis (LWNH) is a reticulate pigmentary disorder with a sporadic occurrence, generally representing a genetic mosaicism. In this case, we describe a two-month-old girl with trisomy 13, who presented with various systemic anomalies such as congenital ventricular septal defect, microcephaly, auricular deformities, flatness of nasal root, overlapping fingers, umbilical hernia, and LWNH. G-banding chromosomal analyses were performed on cultured peripheral blood lymphocytes of the patient and the parents. The karyotype of the patient was 47, XX,+13[100], with no mosaicism. Background: Pilomatricoma is a rare, benign adnexal tumor differentiating toward elements of the hair matrix and shaft. It typically presents as a solitary, deep, dermal nodule. In addition, bullous changes can be rarely seen. Methods: A 16-year-old girl with firm, mobile, multilobule and non-tender mass that has a soft, hemorrhagic blister-like formation on the left infraclavicular region is described. Results: The histopathologic findings were consistent with pilomatricoma. In the dermis above the tumor, there were numerous dilated lymphatic and blood vessels. A 64-year-old man underwent a left radical nephrectomy 25 years ago. The pathological examination revealed renal cell carcinoma (RCC) and no lymph node metastasis. He suffered a recurrence in the renal fossa 25 years after the operation. To our knowledge, this is the first report of late (about 25 years) recurrence of RCC without distant metastases after radical nephrectomy. The isolated fossa recurrence of RCC without any distant metastasis is very uncommon. We suggest that the patients with RCC who underwent radical nephrectomy should undergo lifelong follow up.

Aim: The aim of this study was to assess the size and shape characteristics of prostatic adenocarcinoma cell nuclei using a computer-assisted analysis system, and to compare the results with the Gleason score. Materials and Methods: Morphometric nuclear parameters, such as roundness factor, form ellipse, area, length, and perimeter, were evaluated based on specimen slides of 130 prostatic adenocarcinoma cases (77% needle biopsies and 23% prostatectomy specimens) using a computerized image analysis system. Cerebral arteriovenous fistulas (AVFs) are rare in childhood and are generally associated with arteriovenous malformations (AVMs). They may lead to an increase in intracerebral pressure, seizure, hemorrhage, or cardiac decompression. They may be treated with endovascular embolization. Disseminated Superficial Actinic Porokeratosis: A Case Report, Asli Eralp, Yeşi̇m Kaymak

Disseminated superficial actinic porokeratosis (DSAP) is a type of porokeratosis observed in parts of the skin exposed to intense sunlight and usually appears in the third or fourth decade of life. Since it is a rare disorder, herein we present our experience of a 57-year-old man with DSAP. Dermatological examination of the patient revealed pink/red multiple papular lesions, ranging in diameter from 0.5 to 1 cm, with an atrophic center and hyperkeratotic contours limited to sun-exposed skin parts especially in the extremities and rarely in trunk. Cryotherapy was effective in healing the lesions.

Inpatients experience frequent psychiatric disorders. Long-term stays in hospitals increase this risk of having a psychiatric disorder and cause negative effects on their ongoing treatments. Several studies emphasized that physicians should be careful for psychiatric disorders affecting the treatment process of inpatients as well as the period of stay in the hospital. Aromatase Inhibitors In Infertile Patients: Effects On Seminal Parameters, Serum And Seminal Plasma Testosterone Levels, And Estradiol Levels During Short-Term Follow-Up, M. Murad Başar, Devri̇m Tuğlu

Aim: To evaluate the effects of an aromatase inhibitor (anastrozole) on seminal parameters, and on serum and seminal plasma testosterone/estradiol ratios in infertile patients, as well as to clarify its place among empirical infertility treatment modalities. Patients and Methods: The study included 32 patients with spermatozoa numbering > 5 million/mL in ejaculate and a serum testosterone/estradiol ratio < 0.14. Anastrozole, an aromatase-inhibiting agent, was given (1 mg b.i.d.) and patients were re-evaluated 2 months later based on semen analysis, and measurements of serum and seminal plasma testosterone and estradiol levels. Results: Semen analysis parameters before and after treatment, respectively, were as follows: number of spermatozoa: 12.4 ± 4.1 million/mL and 22.3 ± 5.7 million/mL; motility: 33.4 ± 4.2% and 47.6 ± 7.4%; normal morphology: 5.4 ± 1.3% and 8.9 ± 2.7%. Differences between the values before and after treatment were statistically significant (P < 0.05). Serum testosterone level was 4.1 ± 1.2 ng/mL, estradiol level was 52.1 ± 9.4 pg/mL, and testosterone/estradiol ratio was 0.13 ± 0.03 at the beginning of treatment. These values were 3.2 ± 0.6 ng/mL, 68.4 ± 7.3 pg/mL, and 0.05 ± 0.001, respectively, in seminal plasma. Following 2 months of anastrozole treatment, testosterone and estradiol levels, and the testosterone/estradiol ratio showed statistically significant changes in serum and seminal plasma. While testosterone levels significantly increased, estradiol levels decreased (serum P_T = 0.001, P_{E2} = 0.001; seminal plasma P_T = 0.001, P_{E2} = 0.001). Conclusion: Aromatase inhibitors are a potential treatment method for infertile male patients with increased plasma estradiol levels and decreased plasma testosterone/estradiol ratios.

Materials and Methods: The study included a total of 156 IUI cycles performed in our center from January 2005 to December 2006 with the indication of male infertility. Aim: In this retrospective study, patients with vitamin D intoxication (VDI) were evaluated with respect to clinical and laboratory characteristics, treatment protocols and complications. Materials and Methods: The medical records of 27 children with VDI between 2003 and 2008 were investigated. Data included age, gender, nutritional status, dose and duration of prophylactic/therapeutic vitamin D, the reasons for vitamin D therapy prescription, clinical signs and symptoms, laboratory findings, therapy protocol for VDI, and complications. Aim: Acetylsalicylic acid (ASA, aspirin), which is one of the most frequently used drugs in the world, causes severe gastric mucosal injury. Nitric oxide (NO) is synthesized from L-arginine by nitric oxide synthase (NOS). NOS can be inhibited by Nω-nitro-L-arginine methyl ester (L-NAME) and stimulated by supplementing the diet with L-arginine (L-Arg). Aims: Detection of risk groups and infection sources and awareness of the routes of transmission of infections are important for prevention of hospital infections. In this study, we aimed to evaluate the knowledge level and the behavior models of hospital cleaning staff about nosocomial infections. Materials and Methods: A 21-item questionnaire about nosocomial infections was administered to the hospital cleaning staff. Results: Out of 290 hospital cleaning staff, 240 (82.7%, 122 male, 118 female, aged 36.2±8.7) were included in the study. Aims: To determine the effects of peer and group education on knowledge and beliefs about breast cancer and performance of breast self–examination (BSE). Materials and Methods: The study included a total of 193 female university students, of whom 59 were assigned to peer education and 134 to group education. Data collected at baseline and after six months included sociodemographic characteristics, knowledge of breast cancer and BSE and performance of BSE forms, and Champion´s Health Belief Model Scale. Aims: To determine viral causes of acute upper respiratory tract infections and clinical findings in children living in Istanbul, Turkey. Materials and Methods: This prospective study was carried out in 234 outpatient children (age range: 1-180 months) with acute upper respiratory tract infections, seen in the outpatient clinic at Istanbul Medical School Children´s Hospital. After clinical findings were recorded, nasal swab specimens were taken. In 201 specimens collected, influenza-A and -B, adenovirus, respiratory syncytial virus, and parainfluenza viruses were studied using direct fluorescence antibody, polymerase chain reaction (PCR), and cell culture techniques. We present herein a case in which salmonella infected a pre-existing chronic subdural hematoma. A one-year-old male infant was admitted to our hospital with complaints of loss of consciousness, seizure, and left hemiparesis. Irrigation and drainage through two burr-holes were performed to the left frontoparietal region with the diagnosis of chronic subdural hematoma according to CT findings. Operative findings revealed an infected subdural hematoma. Salmonella typhi grew in cultures of the subdural hematoma.
