Introduction: Electromyography (EMG) and mechanomyography (MMG) provide non-invasive assessments of neuromuscular function and fatigue during dynamic exercise. This study examined neuromuscular fatigue and cycling performance through changes in power output (PO), total work (TW), EMG and MMG, amplitude (AMP) and mean power frequency (MPF) during three, heart rate (HR)-Clamp trials in the vigorous range (77-95% HRmax). Introduction: Insurance coverage may significantly affect access to timely and advanced cardiovascular therapeutic interventions within the United States. Among individuals experiencing acute myocardial infarction (AMI) accompanied by cardiogenic shock (CS), variations based on the primary payer may play a role in subsequent in-hospital outcomes. We conducted a retrospective analysis to determine whether insurance status correlates with in-hospital mortality and other outcomes among patients with AMI and CS, utilizing the NIS database.

Background: People living with HIV (PLWH) experience nearly double the risk of premature atherosclerotic cardiovascular disease and often present with acute coronary syndromes at younger ages [1]. Chronic inflammation, immune activation, and drug–drug interactions between antiretrovirals and antiplatelet agents further contribute to accelerated atherosclerosis and poor outcomes after percutaneous coronary intervention (PCI) [2,3]. Importantly, in-stent restenosis (ISR) remains disproportionately frequent in PLWH despite modern drug-eluting stents, with medication nonadherence compounding risk [4]. We present a young woman with HIV and recurrent myocardial infarction due to rapid ISR, underscoring the impact of inflammation, adherence, and pharmacologic interactions on secondary prevention.

Background: Screening guidelines for breast cancer typically begin at age 40, leaving younger women at risk for delayed detection. The survival outcomes and tumor site patterns by subtype in this younger population remain underexplored. Our analysis leverages comprehensive national data to fill this critical knowledge gap, offering one of the most detailed evaluations to date of molecular subtype distribution, prognostic disparities, and anatomical presentation in women under 40.

Introduction: Cold autoimmune hemolytic anemia (AIHA) is an uncommon, complement-mediated hemolysis typically driven by IgM autoantibodies; secondary forms are often infection-related. Severe, transfusion-refractory presentations are rare in older adults. We present an elderly patient with Mycoplasma pneumoniae and Epstein–Barr virus co-infection complicated by life-threatening cold agglutinin hemolysis.
Case report: Upon admission, a 79-year-old male patient presented with a one-week history of fatigue, pyrexia, nasal congestion, and dyspnea; examination findings revealed pallor, bibasilar crackles, and mild splenomegaly. Introduction: Paroxysmal nocturnal hemoglobinuria (PNH) is a clonal hematopoietic stem-cell disorder driven by loss of GPI-anchored complement regulators (CD55, CD59), leading to complement-mediated intravascular hemolysis, cytopenias, and thrombosis. We report a new PNH diagnosis in a young woman with severe pancytopenia who achieved hematologic improvement on oral iptacopan, requiring dose adjustments for mild liver enzyme elevation.
Case Report: On February 27, 2025, a 38-year-old female patient exhibited symptoms of progressive fatigue, dyspnea, and jaundice. Background: The G protein–coupled estrogen receptor (GPER) mediates rapid non-genomic estrogen signaling and has been implicated in breast cancer progression and aggressiveness. However, its expression patterns across molecular subtypes and its direct functional contribution to tumor behavior remain incompletely defined. The objective of this study was to comprehensively characterize GPER expression in breast cancer tissues and to functionally validate its role in regulating tumor cell proliferation and migration using genetic modulation approaches.

Background: Febrile neutropenia (FN) remains a high-acuity complication of myelosuppressive chemotherapy, yet outcomes in Hispanic patients—particularly in South Texas—are underrepresented. We compared outcomes of FN admissions among Hispanic adults with solid tumors versus hematologic malignancies at a community hospital in the Rio Grande Valley.
Methods: We conducted a retrospective cohort study of adult hospital admissions for FN from January 2022 through January 2025. Inclusion criteria were age ≥18 years, FN documented in the emergency department, active chemotherapy for a solid or hematologic malignancy, and Hispanic ethnicity. Background: Heart disease mortality has declined nationwide, but Texas continues to experience higher rates than the United States overall. At the same time, hypertensive heart disease (HHD) has become more prominent within cardiovascular mortality statistics. Whether this shift reflects a growing hypertension-driven contribution to the Texas–U.S. gap has not been well quantified. We compared long-term trends in HHD mortality and the presence of HHD on death certificates among heart disease deaths in Texas versus the United States.

Background: The G protein–coupled estrogen receptor (GPER) mediates rapid estrogen signaling that promotes proliferation, migration, and therapeutic resistance in breast cancer, including in hormone receptor–negative subtypes. Notably, standard endocrine therapies may inadvertently activate GPER, highlighting the need for direct targeting of this pathway. The objectives of this study were to evaluate GPER as a therapeutic target using pharmacologic modulators and to determine how GPER-driven tumor behavior relates to tumor microbiome composition across breast cancer subtypes.

Introduction: Delayed hemolytic transfusion reaction (DHTR) in adults with sickle cell disease (SCD) is frequently mistaken for vaso-occlusive crisis (VOC) or evolving acute chest syndrome (ACS). New alloantibodies may be absent or delayed, so reflex transfusion can worsen hemolysis. Programs now emphasize routine post-transfusion surveillance and a protocolized, transfusion-sparing response when DHTR is suspected.
Case Presentation: A 35-year-old man with hemoglobin SS SCD, hypertension, chronic kidney disease, and recurrent priapism presented with severe left-leg pain two weeks after receiving two units of packed red blood cells for pre-operative optimization. Background: Optic neuritis is a common neuro-ophthalmic emergency and may be the first clinical manifestation of inflammatory demyelinating disease. In the acute setting, clinicians must rapidly distinguish multiple sclerosis (MS) from important mimics, particularly when optic nerve involvement is accompanied by spinal cord lesions. A structured approach integrating phenotype, MRI lesion distribution and activity, and cerebrospinal fluid (CSF) markers can support timely diagnosis and guide counseling regarding long-term management. We report a young man presenting with unilateral optic neuritis and prior transient neurologic episodes, highlighting a practical diagnostic framework applicable to emergency and inpatient care.

Background: Monkeypox (Mpox) is a global health threat, particularly among men who have sex with men (MSM). Despite the predominance of urban cases in Thailand, rural–urban disparities in Mpox-related attitudes and behaviors remain underinvestigated. This study aimed to compare Mpox-related attitudes and preventive behaviors among urban (Chonburi) and rural (Nakhon Nayok) MSM and identify their influencing factors.

Pharmacy graduates must be prepared to address health disparities and the structural and social drivers of health (SSDH) that shape patient outcomes. These skills require intentional introduction, practice, and scaffolding. One approach to operationalizing this within case-based instruction is the Structural Analysis (SA) framework.Educational activity
Three Doctor of Pharmacy programs piloted the SA framework as a structured method for integrating SSDH into patient case discussions. A central goal in cognitive neuroscience is to map mental processes onto their physical substrates. Traditionally, this goal is pursued by associating different mental processes with spatially-based anatomic localization. On the contrary, much less is known about the temporal dynamics of neural activity, particularly at the scale of the whole brain. School-based policy functions as a critical lever for reducing health disparities among LGBTQ+ youth, who face elevated risks of suicidality, bullying, unsafe school environments, and housing instability. An extensive body of research identifies inclusive school practices—such as affirming curricula, supportive staff, and student organizations—as key protective factors that foster belonging and improve mental health outcomes. However, recent political and legal developments threaten the implementation and consistency of these protections.

Facial plastic and reconstructive procedures represent a small but relatively well-reimbursed component of ENT Medicare billing. Of 860 otolaryngologists identified in the 2023 CMS Physician and Other Practitioners Public Use File, 104 (12.1%) billed at least one facial plastic CPT code, with a mean of 16.9 services per participating provider. A total of 1,756 facial plastic services were rendered, representing 0.16% of total ENT Medicare volume, with a cumulative reimbursement of $584,834. Mean reimbursement per facial plastic provider-CPT combination was $435, compared to $104 across all ENT services, a 4.2-fold difference. Plasma membrane (PM) localization of KRAS requires specific glycosphingolipids in the outer leaflet and phosphatidylserine (PS) in the inner leaflet. PM PS content is controlled by lipid transport proteins ORP5 and ORP8, which operate at ER-PM membrane contact sites (MCSs). Using high-resolution imaging, we now show that GSLs, including GM3 and SM4, are required to maintain ORP5 and ORP8 localization to MCSs. Genetic deletion or pharmacologic inhibition of enzymes required for the biosynthesis of GM3 or SM4 displaces PI4-kinase Type IIIα (PI4KIIIα) and its adaptor EFR3A from the PM, thereby reducing PM phosphatidylinositol 4-phosphate (PI4P) content. BACKGROUND: Patients with type 2 diabetes mellitus (T2DM) and chronic limb-threatening ischemia (CLTI) undergoing lower extremity revascularization (LER) face high risks of major adverse cardiovascular events (MACE) and major adverse limb events (MALE), despite guideline-directed lipid-lowering therapy. Apolipoprotein B100 (ApoB100), reflecting total atherogenic particle number, may identify residual risk beyond LDL-cholesterol (LDL-C). Therefore, we investigated whether baseline ApoB100 levels independently predict MACE and MALE beyond conventional risk factors in this high-risk population.

This study aimed to evaluate the applicability of a multiplex real-time PCR (rPCR) kit, originally developed for bovine mastitis, for detecting mastitis pathogens in small ruminants and reducing bacterial identification time compared with conventional bacteriological culture (BC). Milk samples were obtained from 175 sheep and 124 goats and included cases of clinical mastitis (n = 57) and subclinical mastitis (n = 242). With BC, bacterial growth was detected in 125 of 299 milk samples (41.8%), yielding a total of 169 bacterial isolates. The objective of the experiment was to compare the dominant follicle size, growth rate of the dominant follicle, estrus response, plasma progesterone concentration, and pregnancy in Nili Ravi buffaloes synchronized by controlled internal drug release (CIDR; 1.38 g) and medroxyprogesterone acetate sponge (MPAS; 250 mg) in the ovulation synchronization (OvSynch) protocol. The present study aimed to investigate the hematological and biochemical blood parameters, along with selected egg quality traits, of Turkish Sultan chickens. A total of 90 blood samples collected from 60-week-old hens and roosters and 129 eggs collected from 50 hens were analyzed. Ticks are obligate hematophagous ectoparasites that play a major role in the transmission of pathogens affecting both animals and humans. Investigations into tick biology, pathogen transmission, and the development of effective control strategies have traditionally depended on the extensive use of laboratory animals. Consequently, the development of reliable in vitro feeding systems is a valuable alternative approach with the potential to reduce animal use in tick research.
