Riset Kesehatan Dasar tahun 2013 menyebutkan angka insiden diare pada balita di Indonesia sebesar 6,7%. Aceh merupakan provinsi dengan insiden diare tertinggi, mencapai 10,2%. Profil Kesehatan Aceh menunjukkan bahwa secara umum terjadi peningkatan penyakit infeksi seperti influenza, tuberkulosis, dan diare dalam kurun waktu tujuh tahun (2006 - 2012). Penyakit tersebut dapat dicegah dengan pemberian ASI yang berperan dalam peningkatan kekebalan tubuh. Penelitian ini dilakukan untuk mengetahui hubungan riwayat pemberian ASI dengan penyakit infeksi pada balita. Penelitian ini menggunakan data sekunder dari Badan Pusat Statistik Provinsi Aceh, yaitu data Survei Sosial dan Ekonomi Nasional tahun 2012 dengan jumlah sampel 3.486 balita. …

Malaria merupakan penyakit menular yang menjadi perhatian global. Kecamatan Arongan Lambalek merupakan daerah rawan malaria di Kabupaten Aceh Barat dengan kategori Medium Incidence Area dengan nilai Annual Parasite Incidence (API) 3,67 per 1000 penduduk masih di atas target API Nasional pada fase eliminasi API kurang dari 1 per 1000 penduduk tahun 2013. Penelitian ini bertujuan menganalisis karakteristik, sosial ekonomi, perilaku, dan kondisi lingkungan rumah yang berpengaruh terhadap kejadian malaria di Kecamatan Arongan Lambalek, Kabupaten Aceh Barat. Kebijakan pusat pelayanan kesehatan masyarakat (puskesmas) sebagai Badan Layanan Umum Daerah (BLUD) diimplentasikan untuk meningkatkan kualitas pelayanan kesehatan dasar. Kebijakan BLUD puskesmas telah diterapkan di Kabupaten Gianyar sejak tahun 2010 dan berlaku pada puskesmas perawatan maupun nonperawatan. Pelaksanaan BLUD puskesmas tidak selalu meningkatkan mutu layanan. Penelitian ini bertujuan mengetahui gambaran mutu pelayanan puskesmas perawatan yang berstatus BLUD di Kabupaten Gianyar. Penelitian ini merupakan penelitian potong lintang dengan pendekatan kuantitatif dan kualitatif. Tempat dan waktu penelitian ini dilakukan di Kabupaten Gianyar, Agustus hingga Desember 2013. Kejadian luar biasa hepatitis A di Kabupaten Ciamis telah terjadi beberapa kali, satu di antaranya terjadi di Pondok Pesantren X. Penelitian ini bertujuan untuk menganalisis kondisi kesehatan lingkungan serta perilaku hidup bersih dan sehat dengan kejadian hepatitis A siswa/siswi di Pondok Pesantren X Kabupaten Ciamis. Rancangan penelitian menggunakan desain kasus kontrol dengan jumlah kasus 40 orang dan kontrol 80 orang, total sampel 120 orang. Populasi penelitian adalah siswa/siswi madrasah tsanawiyah dan madrasah aliyah negeri yang tinggal di asrama pondok pesantren. Multimorbiditas adalah kehadiran dua atau lebih penyakit kronis pada satu orang. Prevalensi multimorbiditas meningkat dengan usia, terutama pada lanjut usia (lansia). Penelitian ini bertujuan untuk mengetahui prevalensi dan determinan multimorbiditas pada lansia di Indonesia. Data diambil dari survei keempat Indonesian Family Life Survey (IFLS) yang diadakan pada tahun 2007. IFLS adalah survei sosial ekonomi dan kesehatan longitudinal di Indonesia. Sampel yang digunakan dalam analisis adalah 2.960 lansia (³ 60 tahun). Regresi logistik dilakukan untuk menentukan prevalensi dan determinan multimorbiditas pada lansia. Penelitian ini bertujuan untuk mengetahui hubungan pemakaian kelambu berinsektisida dengan kejadian malaria pada anak usia 0-4 tahun di wilayah Puskesmas Galang Kecamatan Galang Kota Batam tahun 2013. Desain penelitian adalah potong lintang pada 132 responden. Hasil penelitian membuktikan bahwa pada tingkat signifikansi 5% terdapat hubungan bermakna antara jenis kelambu (OR = 4,6), lama pemakaian kelambu (OR = 2,9), cara pencucian kelambu (OR = 3,6), cara menjemur kelambu (OR = 2,8), dan pencelupan ulang kelambu (OR = 3,6) memiliki hubungan yang bermakna dengan kejadian malaria. Recent advances in informatics technology has made it possible to integrate, manipulate, and analyze variables from a wide range of scientific disciplines allowing for the examination of complex social problems such as health disparities. This study used 589 county-level variables to identify and compare geographical variation of high and low preterm birth rates. Data were collected from a number of publically available sources, bringing together natality outcomes with attributes of the natural, built, social, and policy environments. Singleton early premature county birth rate, in counties with population size over 100,000 persons provided the dependent variable. Graph theoretical techniques were used …

Background Over the past ten years, there has been an explosion of microbiome research. Many software packages for analyzing microbial sequences such as the 16S gene from 454 sequencers and Illumina platforms are available. But for a new researcher, it is difficult to know which package to choose. We present a systematic review of packages for the analysis of molecular sequences used to describe and compare microbial communities. This review gives students and researchers information to help choose the best analytic pipeline for their project. To the best of our knowledge, this is the first review of such software.

Epilepsy is the most common neurological problem that affects people with learning disabilities. The high seizure frequency, resistance to treatments, associated skills deficit and co-morbidities make the management of epilepsy particularly challenging for people with learning disabilities. The Books Beyond Words booklet for epilepsy uses images to help people with learning disabilities manage their condition and improve quality of life. Our aim is to conduct a randomized controlled feasibility trial exploring key methodological, design and acceptability issues, in order to subsequently undertake a large-scale randomized controlled trial of the Books Beyond Words booklet for epilepsy.

Background: As genome-wide sequence analyses for complex human disease determinants are expanding, it is increasingly necessary to develop strategies to promote discovery and validation of potential disease-gene associations.
Findings: Here we present a dynamic web-based platform – GWATCH – that automates and facilitates four steps in genetic epidemiological discovery: 1) Rapid gene association search and discovery analysis of large genome-wide datasets; 2) Expanded visual display of gene associations for genome-wide variants (SNPs, indels, CNVs), including Manhattan plots, 2D and 3D snapshots of any gene region, and a dynamic genome browser illustrating gene association chromosomal regions; 3) Real-time validation/replication …

Naphthenic acids (NAs) are water-soluble components of petroleum. The characterization and quantification of NAs by analytical methods have proved quite challenging, whilst the toxic effects of these water-soluble compounds on a variety of organisms adversely affecting reproduction and steroid production is becoming apparent. In this study, we report a fluorescence-based competitive binding method for rapid sensing of the presence of NAs using cellulosic peptide array strips as sensors. The peptide array was designed from sequences derived from the estrogen receptor (ER). Several of these peptides were able to detect the presence of NAs at low micromolar (∼5 mg L−1 …

In this paper, we describe a system that distinguishes be- tween legitimate and malicious database transactions per- formed by application programs. Our system is particularly useful for protecting against code-modification attacks performed by insiders who have access to and can change the programs' source code to make them execute different queries than those they are expected to execute. Our system works with any type of DBMS and requires minimum modification to application programs.

We present a general method for estimating the effect of a treatment on an ordinal outcome in randomized trials. The method is robust in that it does not rely on the proportional odds assumption. Our estimator leverages information in prognostic baseline variables, and has all of the following properties: (i) it is consistent; (ii) it is locally efficient; (iii) it is guaranteed to match or improve the precision of the standard, unadjusted estimator. To the best of our knowledge, this is the first estimator of the causal relation between a treatment and an ordinal outcome to satisfy these properties. We …

Over the last few decades, the notion of sustainability has become an interdisciplinary buzz word. Sustainability has been an integrative concept that includes three constructs or pillars: a) social; b) economic; and c) ecological. Until recently, theoretical approaches based on the three pillars approach have operated in silos rather than exploring the interconnectedness of the constructs. Few models have moved beyond the idea that logical relations exist among the constructs (social, economic, and ecological) to consider factors external to communities, nor have they examined the internal socio-economic factors that influence positive outcomes. Many studies aim to assess whether a therapy has a beneficial effect on multiple outcomes simultaneously relative to a control. Often the joint null hypothesis of no difference for the set of outcomes is tested using separate tests with a correction for multiple tests, or using a multivariate T2-like MANOVA or global test. However, a more powerful test in this case is a multivariate one-sided or one-directional test directed at detecting a simultaneous beneficial treatment effect on each outcome, though not necessarily of the same magnitude. Background
Although funding has supported the scale up of routine, opt-out HIV testing in the US, variance in implementation mechanisms and barriers in high-burden jurisdictions remains unknown.
Methods
We conducted a survey of health care organizations in Washington, DC and Houston/Harris County to determine number of HIV tests completed in 2011, policy and practices associated with HIV testing, funding mechanisms, and reported barriers to testing in each jurisdiction and to compare results between jurisdictions.
Results
In 2012, 43 Houston and 35 DC HIV-testing organizations participated in the survey. Participants represented 85% of Department of Health-supported testers in DC and 90% …

Despite staggering investments made in unraveling the human genome, current estimates suggest that as much as 90% of the variance in cancer and chronic diseases can be attributed to factors outside an individual’s genetic endowment, particularly to environmental exposures experienced across his or her life course. New analytical approaches are clearly required as investigators turn to complicated systems theory and ecological, place-based and life-history perspectives in order to understand more clearly the relationships between social determinants, environmental exposures and health disparities. Aminoglycosides are a critical component of multidrug-resistant tuberculosis (MDR-TB) treatment but data on their efficacy and adverse effects in Botswana is scarce. We determined the effect of amikacin on treatment outcomes and development of hearing loss in MDR-TB patients. Patients started on MDR-TB treatment between 2006 and 2012 were included. Multivariate analysis was used to determine the effect of amikacin on treatment outcomes and development of hearing loss.
