Preparing Healthcare Education For An Ai-Augmented Future, Jiajie Zhang, Susan H Fenton
Faculty, Staff and Student Publications
Artificial intelligence (AI) fundamentally transforms healthcare education as a knowledge enterprise, creating a distributed cognitive system composed of the human brain, which remains relatively unchanged, and AI-based knowledge and cognitive functions, which have accelerated exponentially in scale and power. Education must focus on developing skills to collaborate with AI and on achieving outcomes like problems solved and discoveries made. Curriculum and education policies also need to adapt to this transformation.

Introduction: While incidentally discovered covert cerebrovascular diseases (id-CCD) are associated with future stroke, it is not known if patients with id-CCD are prescribed statins.
Methods: Patients age ≥50 with id-CCD on neuroimaging from 2009 to 2019 with no prior ischaemic stroke, transient ischaemic attack or dementia were identified using natural language processing in a large real-world cohort. Robust Poisson multivariable regression was used to assess statin prescription among patients without prior statins.
Results: Among 2 41 050 patients, 74 975 patients (31.1%; 4.7% with covert brain infarcts (CBI); 29.0% with white matter disease (WMD)) had id-CCD. 53.5% (95% CI 53.2 …

Housing is an environmental social determinant of health that is linked to mortality and clinical outcomes. We developed a lexicon of housing-related concepts and rule-based natural language processing methods for identifying these housing-related concepts within clinical text. We piloted our methods on several test cohorts: a synthetic cohort generated by ChatGPT for initial infrastructure testing, a cohort with substance use disorders (SUD), and a cohort diagnosed with problems related to housing and economic circumstances (HEC). Our methods successfully identified housing concepts in our ChatGPT notes (recall = 1.0, precision = 1.0), our SUD population (recall = 0.9798, precision = 0.9898), …

To uniformly test and benchmark the secure evaluation of transformer-based models, we designed the iDASH24 homomorphic encryption track dataset. The dataset comprises a protein family classification model with a transformer architecture and an example dataset that is used to build and test the secure evaluation strategies. This dataset was used in the challenge period of iDASH24 Genomic Privacy Competition, where the teams designed secure evaluation of the classification model using a homomorphic encryption scheme. Combined with the benchmarking results and companion methods, iDASH24 dataset is a unique resource that can be used to benchmark secure evaluation of neural network models.

Faculty, Staff and Student Publications
In the realm of lung cancer treatment, where genetic heterogeneity presents formidable challenges, precision oncology demands an exacting approach to identify and hierarchically sort clinically significant somatic mutations. Current Next-Generation Sequencing (NGS) data filtering pipelines, while utilizing various external databases for mutation screening, often fall short in comprehensive integration and flexibility needed to keep pace with the evolving landscape of clinical data. Our study introduces a sophisticated NGS data filtering system, which not only aggregates but effectively synergizes diverse data sources, encompassing genetic variants, gene functions, clinical evidence, and an extensive body of literature. This system is distinguished by a …

The results of clinical trials are a valuable source of evidence for researchers, policy makers, and healthcare professionals. However, online trial registries do not always contain links to the publications that report on their results, instead requiring a time-consuming manual search. Here, we explored the application of pre-trained transformer-based language models to automatically identify result-reporting publications of cancer clinical trials by computing dense vectors and performing semantic search. Models were fine-tuned on text data from trial registry fields and article metadata using a contrastive learning approach. The best performing model was PubMedBERT, which achieved a mean average precision of 0.592 …

Clinical research data visualization is integral to making sense of biomedical research and healthcare data. The complexity and diversity of data, along with the need for solid programming skills, can hinder advances in clinical research data visualization. To overcome these challenges, we introduce VisualSphere, a web-based interactive visualization system that directly interfaces with clinical research data repositories, streamlining and simplifying the visualization workflow. VisualSphere is founded on three primary component modules: Connection, Configuration, and Visualization. An end-user can set up connections to the data repositories, create charts by selecting the desired tables and variables, and render visualization dashboards generated by …

Genetically modified (GM) crops that have been engineered to express transgenes have been in commercial use since 1995 and are annually grown on 200 million hectares globally. These crops have provided documented benefits to food security, rural economies, and the environment, with no substantiated case of food, feed, or environmental harm attributable to cultivation or consumption. Despite this extensive history of advantages and safety, the level of regulatory scrutiny has continually increased, placing undue burdens on regulators, developers, and society, while reinforcing consumer distrust of the technology. CropLife International held a workshop at the 16th International Society of Biosafety Research …

Lifelong Direct Error-Driven Learning For Uav Altitude Estimation In Different Weather Conditions, Shirin Nasr-Esfahani, Jagannathan Sarangapani
Electrical and Computer Engineering Faculty Research & Creative Works
While deep neural networks achieve remarkable visual perception capabilities for UAV position and orientation estimation, their resilience to different weather conditions still needs improvement. These models often suffer from catastrophic forgetting when adapted to new environments, losing previously acquired knowledge. Lifelong learning methods aim to balance learning flexibility and memory stability. In this paper, we present an image-based approach to estimate the relative altitude of a UAV using 2D images under varying weather conditions, including sunny, sunset, and foggy scenarios. Our experiments demonstrate significant performance degradation when the model is trained sequentially on different weather datasets, especially when new images …

Lifelong Safe Optimal Adaptive Tracking Control Of Nonlinear Strict-Feedback Discrete-Time Systems, Behzad Farzanegan, S. Jagannathan
Electrical and Computer Engineering Faculty Research & Creative Works
This paper presents a comprehensive approach for achieving multi-task safe optimal adaptive tracking (MSOAT) for a class of nonlinear discrete-time systems, particularly those in strict-feedback form, utilizing a multi-layer neural network (MNN)-based framework. To begin, a cost function with a novel Barrier function (BF) term is introduced for each subsystem to address the weak safely reachable problem, serving as a crucial tool for guiding the system's trajectory toward the safe set while avoiding unwanted sets. To deal with the tracking problem, the Hamilton-Jacobi-Bellman (HJB) framework is used through the actor-critic MNN-based backstepping technique to estimate the solution of the value …

Despite a long presence in the contiguous United States (US), the distribution of invasive wild pigs (Sus scrofa × domesticus) has expanded rapidly since the 1980s, suggesting a more recent evolutionary shift towards greater invasiveness. Contemporary populations of wild pigs represent exoferal hybrid descendants of domestic pigs and European wild boar, with such hybridization expected to enrich genetic diversity and increase the adaptive potential of populations. Our objective was to characterize how genetic enrichment through hybridization increases the invasiveness of populations by identifying signals of selection and the ancestral origins of selected loci. Our study focused on invasive …

The global outbreak of highly pathogenic avian influenza (HPAI) H5N1 Eurasian lineage goose/Guangdong clade 2.3.4.4b virus that was detected in North America in 2021 is the largest in history and has significantly impacted wild bird populations and domestic poultry across the continent. Synanthropic birds may play an important role in transmitting the virus laterally to other wild bird species and domestic poultry. Understanding the dynamics of HPAI in atypical, or nonreservoir, wild bird hosts may help inform management decisions and potential risk factors to both wild and domestic bird populations. Following the confirmation of infections of HPAI H5N1 in domestic …

Raptor‐aircraft collisions (bird strikes) pose a serious safety risk to civil aircraft. Even smaller raptors, such as American kestrels (Falco sparverius), can be problematic within airport environments. Given widespread declines in kestrel populations, public interest, logistical and financial constraints, and other factors, managing kestrels at airports presents some unique challenges. I conducted a study to determine which biological (e.g., age and sex of the bird) and logistical factors (e.g., month and translocation distance) might influence the return of American kestrels live‐captured and translocated from 42 civil airports and military airbases within the contiguous USA during 2016–2021. American kestrels …

Invasive species, particularly omnivorous rodents, have damaged island ecosystems around the world. The unique flora and fauna of island systems have typically not evolved with omnivorous rodents and are heavily impacted by their introduction. Wake Atoll of the southern Pacific Ocean is a 707-hectare coral island complex of three islands managed by the United States Department of Defense (DoD). DoD restricts civilian access to the atoll but aircraft and seacraft regularly visit the island to transport military equipment and to sustain the year-round military and contractor personnel stationed on the atoll. Wake Atoll had been invaded by Rattus rattus complex …

Use of a contraceptive (i.e., fertility control) is attractive for rodent management where lethal control is unwanted. Although population reduction is generally unachievable with small-scale, short-term contraceptive use, reduced juvenile recruitment is achievable. The injectable immunocontraceptive vaccine GonaCon (active ingredient: gonadotropin releasing hormone [GnRH]) was registered in 2022 by the United States Environmental Protection Agency (EPA) for controlling fertility of female prairie dogs (Cynomys, a type of ground squirrel) in urban/suburban settings. Here we: 1) describe past research, including a replicated field study in Colorado (GonaCon treatment vs. control sites) testing efficacy of GonaCon in prairie dogs, which …

Wild pigs (Sus scrofa) are one of the most destructive invasive species in the United States, known for causing extensive damage to agricultural commodities, natural resources, and property, and for transmitting diseases to livestock. Following the establishment of the National Feral Swine Damage Management Program (NFSDMP) in 2014, the expansion of wild pig populations has been successfully slowed. This paper combines two modeling approaches across eight separate models to characterize the expansion of wild pig populations in the absence of intervention by the NFSDMP and forecasts the value of a subset of resources safeguarded from the threat of …

Management of the raccoon rabies virus variant in North America is conducted primarily using oral rabies vaccination (ORV). When a sufficient proportion of the population is vaccinated (60%), rabies transmission can be eliminated. To date, ORV programs have successfully controlled and eliminated raccoon rabies in rural areas, but there has been less success in urban areas. We studied the proportions of rabies virus neutralizing antibodies (RVNA) in a raccoon (Procyon lotor) population during a 3-year ORV trial in developed areas of Burlington, Vermont, United States. We used a modified N-mixture model to estimate raccoon abundance, RVNA seroprevalence, and …

Oral baits containing vaccines, medicants or toxicants have been delivered to a broad range of wildlife species to protect against or treat disease, or suppress populations. Ethyl-Iophenoxic acid (Et-IPA) is used as a biomarker to determine oral bait consumption by wildlife species to assess and refine baiting strategies. Et-IPA is a persistent biomarker in many eutherian mammal species but not in the two marsupial species, swamp wallaby (Wallabia bicolor) and brushtail possum (Trichosurus vulpecula), tested to date. Et-IPA has not previously been evaluated in carnivorous marsupials. The Tasmanian devil (Sarcophilus harrisii), the largest extant …

1. Diseases occurring at low prevalence in free-ranging wildlife can be difficult to study and predict at a landscape scale. This problem is compounded by imperfect pathogen detection.
2. We used data from multiple surveillance methods to determine factors influencing spread of a low-prevalence disease, bovine tuberculosis (bTB), among white-tailed deer (Odocoileus virginianus) in a bTB endemic area over a 10-year period. We fit dynamic occupancy models to predict spatial spread of bTB through a county in Michigan, USA, within the bTB endemic area, while accounting for imperfect detection and a suite of ecological processes potentially driving bTB …

Angiostrongylus cantonensis is a globally distributed nematode and the leading cause of eosinophilic meningitis in humans. As a global hotspot for this disease, Hawaii’s agricultural exports may be contributing to the spread of A. cantonensis. Phytosanitary irradiation doses of 150 or 400 Gy provide quarantine security against multiple insect pests. We evaluated the in vitro and in vivo effects of phytosanitary irradiation on infectious, third-stage A. cantonensis juveniles. In vitro experiments directly exposed juveniles to irradiation doses ranging from 200 to 1,000 Gy. Results showed low mortality and no dose response across all treatments 27 days post-irradiation. …

We introduce a new method for the classification of Gram-stained WSIs. As a test for the diagnosis of blood infections, Gram stains are highly relevant to informing patient treatment. Rapid analysis of Gram stains has been shown to be positively associated with better clinical outcomes, indicating the need for better tools to aid in automatic Gram stain analysis. To date, this area of research has been underexplored, with previous studies relying on the manual patch-level annotation of WSIs to generate training data. This is the first application of a transformer-based model to Gram-stain WSI classification, an approach that is far …

DNA computing is an ever-growing field with scientists trying to design structures that optimize logic gate communication to develop fast, biologically compatible, computational structures. We hypothesize that by using the principles of DNA computing, it is possible to design a DNA tile capable of studying localized DNA hybridization that can differentiate between oligonucleotides of different structural conformations. This includes synthetically manipulating DNA into a nanostructure that can perform Boolean logic functions to calculate the different rates of hybridization. To test our hypothesis, we designed a DNA Tile that incorporated a 4WJ using YES logic. Linear and hairpin single-stranded (ss) DNA …
