Advancing Tka Biomechanics: From Joint Simulator To Boundary Condition Development, Yashar Ali Behnam

This dissertation addresses four specific aims that collectively attempted to advance the experimental and computational analysis of total knee arthroplasty (TKA) components. The first study focused on the development of a novel whole knee joint simulator capable of simultaneous tibiofemoral and patellofemoral knee loading. This simulator employs custom fixturing to facilitate dynamic, unconstrained, muscle-driven PF articulation alongside controlled TF contact mechanics. Validation against experimental measurements showed strong agreement, demonstrating the simulator's potential as a valuable tool for future TKA design and surgical technique investigations.

Herniated discs in the spine are a significant patient burden, with potential links to lower back and leg discomfort and a considerable impact on daily life. These discs, located between spinal vertebrae, are comprised of the annulus fibrosus (AF) and the nucleus pulposus (NP). Herniations happen when the NP protrudes through a full-thickness annular tear, possibly compressing spinal nerves. The mechanical factors underlying herniated discs are poorly understood, necessitating research into these mechanisms and accessible diagnostic techniques. Our study employs high-resolution ultrasound and texture correlation to quantify strain patterns in intervertebral discs during dynamic loading.

In the Unites States, approximately 1.5 million people currently have a spinal cord injury and suffer permanent sensory and motor loss due to the disruption of the spinal cord. Due to the significant morbidity, it is vital to understand the functional impact of disrupting neural descending pathways that modulate spinal neurons involved in intermuscular coordination critical for gait behaviors. Tasks that are more difficult require additional input from these neural pathways; therefore, fourteen feline subjects were familiarized with level overground locomotion and stair descent gait tasks. The objective of this study was to investigate associations between injury-related kinematic parameters and overweight measures for youth baseball pitchers. The injury-related kinematic parameters considered were measurements 1) at foot contact: stride length, front foot position, shoulder external rotation, shoulder abduction, and elbow flexion; 2) between FC and ball release: peak knee extension; and 3) at BR: shoulder abduction. Wearable sensors were leveraged to develop two methods for computing hip joint angles and moments during walking and stair ascent that are more portable than the gold standard. The Insole-Standard (I-S) approach replaced force plates with force-measuring insoles and achieved results that match the curvature of results from similar studies. Peaks in I-S kinetic results are high due to error induced by applying the ground reaction force to the talus. The Wearable-ANN (W-A) approach combines wearables with artificial neural networks to compute the same results. Compared against the I-S, the W-A approach performs well (average rRMSE = 18%, R2 …

Ankle foot orthoses (AFOs) are commonly prescribed to provide stability and foot clearance for patients with weakened or injured musculature. The Dynamic Response AFO (DRAFO) was designed to improve proprioception at heel strike. The design includes a rigid outer shell with a cut out heel and a soft inner lining; it is typically aligned in plantarflexion and may incorporate external heel wedges. The goal of this study was to evaluate biomechanical differences between healthy subjects and those with ankle instability during the gradual onset of lower extremity fatigue from a landing activity. An understanding of these differences is needed in order to prevent future injury to or further debilitation in individuals with ankle instability. A functional fatiguing activity was designed to focus fatigue on the quadriceps muscles, as those are the muscles most frequently fatigued during sport. Adolescent Idiopathic Scoliosis (AIS) is a three dimensional deformity of the spine that causes abnormal curvatures in the coronal and sagittal planes and abnormal vertebral rotations. The gold standard for diagnosing and tracking the progression of the deformity is through the use of repeated radiography and a patient can receive upwards of fifteen radiographic series over the course of treatment, which exposes this population to an increased risk of radiation-dependent complications.

Hallux valgus is a common condition characterized by lateral deviation of the large toe and medial deviation of the first metatarsal. While some gait analyses of patients with hallux valgus have been performed using plantar pressures, very little is known about the kinematics of gait in this population. The purpose of this study was to evaluate triplanar kinematics in patients with hallux valgus using a multisegmental foot model.
