However, despite widespread attempts to reduce the frequency of runway incursions, the rate at which these events occur in the U.S. has steadily risen over the past several years. This dissertation investigated the relationship among ethical leadership, an ethical workplace climate, safety culture, safety behaviors, and measured safety outcomes of workers in the high reliability organizations of aviation and healthcare. The primary objective was to develop a model linking these factors and assess their fit within the model. A secondary objective was to examine differences between the two populations. In this study, a 101-item instrument was used to collect and analyze employee responses on ten factors comprising the model. A variety of challenges to the successful assimilation of Unmanned Aircraft Systems (UASs) into the National Airspace System (NAS) exists. Aside from technical and legislative challenges, another setback has recently surfaced when the FAA suspended its selection process for UAS test sites due to privacy concerns. According to the National Transportation Safety Board (NTSB), loss of control in-flight is the greatest cause of general aviation accidents. The purpose of this study was to determine the most frequently occurring probable causes and contributing factors from loss of control in-flight. This study used the Pareto principle and methodology developed by the General Aviation Joint Steering Committee to analyze accidents retrieved from the NTSB's Aviation Accident Database. In 2009, the crash of Colgan Flight 3407 in Buffalo, NY raised concerns regarding the adequacy of current pilot qualification standards for entering air carrier operations. In response, the U.S. Congress enacted Public Law 111-216, which requires a pilot to hold an Airline Transport Pilot (ATP) certificate in order to be qualified as a flight crewmember under 14 CFR, part 121, and also allows the Federal Aviation Administration (FAA) to modify the requirements to obtain an ATP certificate. New flight deck technology designed to mitigate runway incursions may not be effective in triggering a flight deck alert to avoid high speed surface collisions for runway incursions classified as serious by legacy metrics. This study demonstrated an innovative method of utilizing expert raters and actual high-risk incidents to identify shortcomings of using legacy metrics to measure the effectiveness of new technology designed to mitigate hazardous incidents. In October, 2009, the Aircraft Certification Service (AIR) chartered a team to conduct the Manufacturers' Safety Management Systems (MSMS) pilot project. The pilot project began in January, 2011, and encourages voluntary implementation of Safety Management Systems (SMS) among aviation Design and Manufacturing (D&M) organizations in order to allow the FAA to assess how to provide oversight of these systems. Eleven companies representing the breadth of organizations that AIR oversees participated.

The persistence of faulty decision making as a primary cause of accidents indicates a need to train pilots to make better decisions. The purpose of this study was to analyze scenario-based training's effectiveness at improving the aeronautical decision making of collegiate flight students. The researcher scored each participant's aeronautical decision making as they completed simulated flights in an advanced aviation training device. The Flight Training Department at Embry-Riddle Aeronautical University, Daytona Beach, has been contemplating implementing an Aviation Safety Action Program (ASAP) in order to help protect students and instructors from certificate action. This feasibility study analyzed what motivated instructor pilots to submit safety reports, and if an ASAP would increase reporting from instructor pilots. It also identified what concerns existed among instructors and managers regarding an ASAP. A survey was given to instructors and managers; also instructors in leadership roles were interviewed. Commercial aviation and commercial space similarly launch, fly, and land passenger vehicles. Unlike aviation, the U.S. government has not established maintenance policies for commercial space. This study conducted a mixed methods review of 610 U.S. space launches from 1984 through 2011, which included 31 failures. An analysis of the failure causal factors showed that human error accounted for 76% of those failures, which included workmanship error accounting for 29% of the failures. Emergency Locator Transmitters (ELT) help search crews to locate aircraft in distress and to rescue survivors. This study analyzed ELT data from U.S. General Aviation accidents during the period 2006 to 2010. This study examined the effectiveness of ELTs in terms of ELT Success Rate (ESR) and False Negative Rate (FNR) based on ELT-Aided. This study found a significant difference between ELT-Operated and ELT-Aided. The ESR was found to be 38.58% whereas the FNR was found to be 61.42 %. This study utilized discrete event simulation (DES) and queuing networks to investigate the effects of baggage volume and alarm rate at the Security Screening Checkpoint (SSCP) of a small origin and destination airport. A Jackson queuing network was considered for a theoretical assessment to SSCP performance. A DES model using Arena version 12 was utilized for an empirical approach. Introduction. Over the last 20 years, nearly 40,000 general aviation (GA) aircraft were involved in accidents, roughly 20% of which were fatal. To address this safety concern, scientists have often relied on accident data. Because of the rare nature of accidents, commercial aviation incident and near miss data may prove to be useful sources of safety information. In one such study, the National Transportation Safety Board interviewed GA pilots that were flying near a weather-related accident in pursuit of a different perspective than that of the accident pilot. Several FOQA parameters that could enhance air carrier safety have not been recorded and/or analyzed. Airlines have traditionally chosen a limited number of parameters for analysis (thousands have been available for recording). No study in the U.S. had addressed FOQA parameters, leading to an exploratory mixed methods study. A conference call comprising FOQA industry experts was planned and completed to (a increase researcher knowledge and (b) discuss a list of improved safety FOQA parameters. Incorrect maintenance information has also been cited as a contributing factor in a number of recent aircraft mishaps. To date no one has studied the types of errors found in aircraft maintenance manuals published by manufacturers.
