Abstract

The article discusses complacency and the five hazardous attitudes in the context that the five hazardous attitudes are a subset of complacency. On the list of 12 most common contributors to aircraft accidents, the FAA lists complacency as the second most often contributor. Complacency implies that there is a level of satisfaction with the present circumstances without any consideration of future unseen potential dangers. On May 15th, 2017, N452DA, a Learjet 35A operated by Trans-Pacific Air Charter, LLC, departed from Philadelphia to Teterboro, New Jersey (TEB) on a positioning flight under 14 Code of Federal Regulations § 91 (Part 91) in Visual Meteorological Conditions (VMC). While circling to land Runway 01 at TEB after executing the Instrument Landing System (ILS) Runway 06 approach, N452DA stalled and crashed one-half mile south of the approach end of runway 01. Eye movement characteristics might provide insights on pilots' mental fatigue during prolonged flight. The visual entropy, eye fixation numbers, and eye fixation durations of ten novice pilots and ten expert pilots were analyzed for a four-hour simulated flight task consisting of four consecutive flight legs. Each flight leg lasted approximately one hour and contained five flight phases: takeoff, climb, cruise, descend, and landing. The pilots maneuvered the simulated B-52 aircraft following instrument flight rules (IFR) in a moderate-fidelity Microsoft Flight Simulator environment. The research in this report intends to highlight the severe impact of 5G C-band implementation on the air transportation industry. This project aimed to improve the efficiency of flight planning of regular airlines regarding the addition of extra fuel due to weather forecasts with low probability occurrence regarding rain and thunderstorm events (30 and 40% chances).

Recommendations:

  • Expand the scope/sample of analyzed flights to include other significant strategic airports.

  • Involve Operations Control Center, Air Operations Engineering, and Safety Department in future studies in order to create an effective policy for extra fuel supply (on planning and flight execution).

Past accidents have indicated that first officers (FO) are less likely to identify and correct captain errors than captains are to correct FO errors. Crew resource management (CRM) training was introduced in the late 1970s to improve captain teamwork skills to utilize the FO more effectively and to increase FO willingness to interject to preserve safety. We conducted an investigation into non-fatal helicopter emergency medical service accidents from January 26, 1991 to April 26, 2018 via the National Transportation Safety Board aviation accident database. Over this 28-year timeframe 247 accidents results in 251 fatalities and 179 non-fatal injuries. Exploratory analysis of the data indicate that more non-fatal injuries occurred in September compared to any other month during the study timeframe. Exploratory correlational analysis via elastic net logistic regression concluded that no linear relationship of NTSB accident database data provide insights into what factors are correlated with an increased likelihood of non-fatal injuries. As demand for unmanned aerial vehicle (UAV) operations increases, it is vital to understand its effects on air traffic controllers and the safety of the national airspace system. This study’s primary purpose is to determine how UAVs that operate in controlled airspace would influence air traffic controllers’ occupational stress and performance. In a within-subject experimental research design, 24 participants sampled from a university’s undergraduate Air Traffic Management (ATM) program completed three different air traffic control (ATC) scenarios on an en route ATC simulation system. The most frequent obstacle of an aircraft evacuation is the passengers carrying baggage while evacuating. Passengers who insist on taking their carry-on baggage during an emergency evacuation not only slow down the evacuation process but also act as a significant risk to the safety of other passengers. This study investigated the factors that affect passengers’ behavioral intention to evacuate with carry-on baggage and the effects of evacuating with carry-on baggage on the total evacuation time. Overall, two studies were conducted to provide an outline of the factors that affect and affected by carry-on baggage.
