The study conceptualized Safety Management System (SMS) initiative, self-efficacy, and transformational safety leadership as constructs that relates to safety behavior (measured by safety compliance and safety participation) when mediated by safety motivation using a quantitative approach. Structural equation modeling techniques was used to derive a final measurement model that fit the empirical data and was used to test the study hypotheses. Utilizing a sample of 282 collegiate flight students and instructors from a large public university in the US, a 46-item survey was used to measure respondent’s perceptions on the study variables. Unmanned aircraft activity is becoming more common within the National Airspace System (NAS) and is expected to dominate the NAS in the near future. Specific procedures for adopting unmanned aircraft into the National Airspace System (NAS).
A variation of the Military Grid Reference System was developed and digitally overlaid onto the radar display. To incorporate this grid system, a customized flight plan database was created for the storage of operator submitted flight plans. Instead of verbal communication, a computer chat system is used for communication because of the low altitude operations in the field.

Unmanned Aircraft Systems (UAS) are expected to dominate the National Airspace System (NAS) in the near future. One particular barrier preventing the integration of unmanned aircraft into the NAS is the lack of standardized procedures for distinguishing and communicating with remote UAS operators. In preparation for adopting unmanned flight operations into a complex control system, it is important to identify solutions to effectively control UAS in the NAS. Commercial unmanned aircraft systems (UAS) are expected to dominate the National Airspace System (NAS) in the years to come. One particular barrier preventing integration of UAS into the NAS is the lack of standardized procedures for separating aircraft and communicating with ATC. In preparation for adopting unmanned flight operations into a complex control system, it is important to identify solutions to effectively control UAS in the NAS.

An archival study was conducted to determine the influence of stall warning system performance on aircrew decision-making outcomes during airborne icing encounters. A Conservative Icing Response Bias (CIRB) model was developed to explain the historical variability in aircrew performance in the face of airframe icing. The model combined Bayes’ Theorem with Signal Detection Theory (SDT) concepts to yield testable predictions that were evaluated using a Binary Logistic Regression (BLR) multivariate technique applied to two archives: the NASA Aviation Safety Reporting System (ASRS) incident database, and the National Transportation Safety Board (NTSB) accident databases, both covering the period January 1, 1988 …

Currently, U.S. air carriers do not provide equipment or training necessary to mitigate the risk posed by surface-to-air fire (SAFIRE) threats. These threats consist of self-guided weapons (infrared shoulder-fired surface-to-air missiles), manually-aimed threats (small arms, recoilless grenade launchers, rockets, and light anti-aircraft artillery), and hand-held lasers. Technological solutions to counter infrared shoulder-fired missiles have been explored, but were rejected due to prohibitive equipment and maintenance costs. A lower cost option, providing air-carrier pilots with SAFIRE risk-reduction training, has not been formally addressed by the air-carrier industry or the U.S. federal government. The use of small unmanned aircraft systems (sUAS) has increased significantly in the past year. Geographic fencing (geo-fencing) is software built into most medium-cost consumer sUAS. This software is typically used to limit the altitude above launch point, the flight distance from the transmitting controller, and/or restrict flight inside a no-fly zone. While the concept of a geo-fence is simplistic, the human-computer-interface (HCI) varies drastically among platforms, and even between software iterations on the same platform. For expedient airfield runway repair, the US Air Force has developed a folded fiberglass mat to cover craters repaired with a well-compacted granular base material. The objective of this study was to evaluate the adequacy of using polymer plugs to anchor the mat to a repaired asphalt pavement for heavy aircraft operations. The effort consisted of materials testing, field experiments and analytical modeling. An 89,800-kg (198,000-pound) load cart having the footprint of a single C-5 main gear was pulled on a mat with wheels locked to simulate full braking forces. Undesirable hot surface ignition of flammable liquids is one of the hazards in ground and air transportation vehicles, which primarily occurs in the engine compartment. In order to evaluate the safety and sustainability of candidate replacement fuels with respect to hot surface ignition, a baseline low lead fuel (Avgas 100 LL) and four experimental unleaded aviation fuels recommended for reciprocating aviation engines were considered. In addition, hot surface ignition properties of the gas turbine fuels Jet-A, JP-8, and JP-5 were measured. Examining How Breakdowns In Pilot Monitoring Of The Aircraft Flight Path, Robert Sumwalt, David Cross, Dennis Lessard

Aircraft accident and incident data reveal that serious safety consequences can arise when flight crewmembers fail to properly monitor the aircraft flight path. This research study reviewed human factors literature to better understand why pilots fail to properly monitor, and analyzed accident and incident data. Recommendations for improving monitoring performance were formulated, which may be useful to air carriers in implementing a new Federal Aviation Administration requirement that calls for specific training in monitoring.

A wide range of legislation has been proposed or put into place that restricts the use of unmanned systems. These actions by legislators and regulators will stifle the growth of this technology and the associated surrounding industry. The largest obstacle to the proliferation of UAS in the U.S. is the FAA. The FAA has designated the location of six test sites that are anticipated to allow for less restrictive and formative research to assess the technologies that the FAA has claimed need to exist in order to integrate UAS into the NAS. Commercial aviation and commercial space similarly launch, fly, and land passenger vehicles. Unlike aviation, the U.S. government has not established maintenance policies for commercial space. This study conducted a mixed methods review of 610 U.S. space launches from 1984 through 2011, which included 31 failures. An analysis of the failure causal factors showed that human error accounted for 76% of those failures, which included workmanship error accounting for 29% of the failures. With the imminent future of commercial space travel, the increased potential for the loss of human life demands that changes be made to the standardized procedures, training, and …

A task overload condition often leads to high stress for an operator, causing performance degradation and possibly disastrous consequences. Just as dangerous, with automated flight systems, an operator may experience a task underload condition (during the en-route flight phase, for example), becoming easily bored and finding it difficult to maintain sustained attention. When an unexpected event occurs, either internal or external to the automated system, the disengaged operator may neglect, misunderstand, or respond slowly/inappropriately to the situation. In the situation of integrating unmanned aerial systems (UASs) into the National airspace, UASs outside of terminal areas would generally be permitted to fly their preferred routes, and self-separate, with minimal intervention from air traffic control.
