The purpose of this study was to discover if a relationship existed between the most common safety concerns and the most common UAS accidents with regards to the integration of military unmanned aerial systems (UAS) into the National Airspace System (NAS). This study examined the most common causes of UAS accidents over a five-year period, the level of safety concerns and common concerns from UAS military pilots and sensor operators. A shortage of skilled craft labor in the North American construction industry has been an unfortunate cyclic trend since the late 1980s. This shortage has been reported and discussed frequently by numerous past studies in the context of construction industry. The 2008 U.S. recession was at least one period when the craft shortage temporarily improved, as witnessed by spikes in construction unemployment rates above 20% due to the work slowdowns. The studies conducted in this thesis evaluate the safety benefits of a blind spot warning (BSW) application and a do not pass warning (DNPW) application at low, medium, and high market penetrations (MP) of connected vehicles (CV) using a high-fidelity driving simulator. Using vehicle-to-vehicle (V2V) communication, CVs can exchange information to alert drivers of potential safety hazards and reduce potential crashes during lane change and overtaking maneuvers. This project evaluates a salvaged process control skid for potential refurbishment and educational re-use. In-lab work evaluated the existing skid. Several process control elements have been recommended for replacement, including the process tank (due to corrosion), the pressure transmitter (operability/corrosion), the flow meters (mechanical) and the control system (age). The control valves, temperature elements and transmitters, pump, holding tanks, and the electrical heat exchanger should be salvaged. An I/P converter and pressure relief valve should be added to the pneumatic feed. Enclosing of all electrical components should be prioritized. Currently, U.S. air carriers do not provide equipment or training necessary to mitigate the risk posed by surface-to-air fire (SAFIRE) threats. These threats consist of self-guided weapons (infrared shoulder-fired surface-to-air missiles), manually-aimed threats (small arms, recoilless grenade launchers, rockets, and light anti-aircraft artillery), and hand-held lasers. Technological solutions to counter infrared shoulder-fired missiles have been explored, but were rejected due to prohibitive equipment and maintenance costs. A lower cost option, providing air-carrier pilots with SAFIRE risk-reduction training, has not been formally addressed by the air-carrier industry or the U.S. federal government. Coal is the most abundant fossil fuel in the United States and remains an essential source of energy. While more than half of coal production comes from surface mining, nearly twice as many workers are employed by underground operations.
One of the key pieces of equipment used in underground coal mining is the continuous mining machine. These large and powerful machines are operated in confined spaces by remote control. Wrong-way crashes are a major cause for safety concern along freeways and limited access facilities. This study investigated three different aspects of the use of Rectangular Rapid Flashing Beacons (RRFBs): 1) different settings of red RRFBs and their perceived effectiveness on reducing wrong-way driving based on a public opinion survey, 2) the effect of active red RRFBs on the behavior of drivers traveling on adjacent roadways via a before-and-after study, and 3) the effectiveness of red RRFBs on alleviating wrong-way driving on freeway off-ramps by analyzing the percentage of vehicles turning back after observing the red RRFB activation. This study aimed at improving the methodology for developing statewide non-motorized safety performance functions (SPFs). Due to lack of pedestrian and bicyclist counts, the methodology proposed a procedure for developing statewide surrogate non-motorized exposure measures using data that are available at statewide level. Eleven years non-motorized crashes at signalized urban intersections joining Arterial and Collector roads in Michigan were used to test the procedure.

The Highway Safety Manual (HSM) lists four different methods for determining the change in crash frequency in order of reliability. Currently, the Utah Department of Transportation (UDOT) uses the fourth reliable method. The goal of this research was to develop a tool that the most reliable method mentioned in the HSM could be used to perform life-cycle benefit-cost analyses. A spreadsheet program was built that performs the HSM's Part C Predictive Method for 11 different roadway segment types mentioned in HSM using Excel macros and Visual Basic for Applications (VBA) programming. Loss of control is a serious problem in aviation that primarily affects General Aviation. Technological advancements can help mitigate the problem, but the FAA certification process makes certain solutions economically unfeasible. This investigation presents the design of a generic adaptive autopilot that could potentially lead to a single certification for use in several makes and models of aircraft. The autopilot consists of a conventional controller connected in series with a robust direct adaptive model reference controller. In a follow-up to our previous study, the current work examines the gust-induced “cone of uncertainty” in a small unmanned aerial vehicle’s (UAV) flight trajectory addressed in the context of safety assessments of UAV operations. Such analysis is a critical facet of the integration of unmanned aerial systems (UAS) into the National Airspace System (NAS), particularly in terminal airspace. A recent alleged “drone” collision with a British Airways Airbus A320 at Heathrow Airport highlighted the need to understand civil Remotely Piloted Aircraft Systems (RPAS) accidents and incidents (events). This understanding will facilitate improvements in safety by ensuring efforts are focused to reduce the greatest risks. One hundred and fifty two RPAS events were analyzed. The data was collected from a 10-year period (2006 to 2015). Results show that, in contrast to commercial air transportation (CAT), RPAS events have a significantly different distribution when categorized by occurrence type, phase of flight, and safety issue. This paper quantifies the human harm, in the form of fatalities and skull fractures, which could occur as a result of an unmanned aircraft falling from a height. The analysis is used to establish the maximum height at which an unmanned aircraft can be flown over people to achieve a level of safety consistent with the rate of ground fatalities from General Aviation. The maximum height is dependent on the aircraft mass and the population density of people on the ground below.

The risk of fatigue on pilot performance and safety is well recognized in aviation. In response to increased safety concerns, the Federal Aviation Administration addressed fatigue risk for passenger airline pilots in Title 14 Code of Federal Regulations Part 117, which included accommodations to address fatigue related risk-factors such as circadian rhythm and the phenomenon commonly described as jet-lag. The rule became effective in January 2014. A survey was conducted in March and April of 2014 to better understand pilot perceptions of the new Part 117 rule. The current issue in the process of transporting a patient from the operating room (OR) to the intensive care unit (ICU) is the risks involved with the IV stand being top-heavy due to two or three IV pumps weighing approximately 15 pounds each. This can lead to injury to the patient, or other personnel, if an instance occurred where the IV stand were to tip over. Having a single person to manage both the IV stand and gurney during transport can increase this risk.

The extraction of Bakken crude oil in the Williston Basin of North Dakota is transforming how petroleum is shipped. Before 2008, North American oil was carried primarily via pipeline from source regions to refineries along the Gulf Coast or to metropolitan areas close to consumers. The current pipeline system is ill-equipped to accommodate massive volumes of oil from this previously low producing region. Lack of infrastructure and high demand for “light tight oil” (LTO) led to the use of railroads as a means of moving the product to refineries across the nation. With the legalization of marijuana in states such as Colorado, proximity of these marijuana dispensaries has become another factor. The Normalized Difference Vegetation Index (NDVI) can be used as a measure of vegetation, and to determine areas with high levels of irrigated vegetation.
