Objective: The main objective of this study was to determine functional and neuromuscular outcomes of stroke patients using their non-preferred hand with and without a 3D printed passive exoskeleton compared to controls using their non-preferred hand with and without the passive exoskeleton. Methods: Adults at least six months post stroke (Stroke, n = 5) and age- and sex- matched healthy controls (Control, n = 5) performed nine trials of a gross motor task while having their brain activity measured. Despite the frequency of musculoskeletal injuries such as sprains, broken bones and torn ligaments1, treatment options are often costly, time-consuming and ill-fitted.2,3 Additive manufacturing (“3D-printing) allows for the production of highly-customized and inexpensive assistive devices4, which suggests potential efficacy in the prescription of splints and casts for musculoskeletal injury.3 In the present study, a parametric, customizable splint/cast was created using a computer-aided design (CAD) package (Fusion 360, Autodesk, San Rafael, CA, USA) and produced with low-cost, desktop 3D printing (Ultimaker 2+ Extended, Ultimaker, Geldermalsen, Netherlands). We propose to isolate the contributions of continuous OF to the orthogonal relationship between balance and gait control during treadmill walking using a series of conditions. Manipulating the direction of OF using a CAREN (Motek Medical, Amsterdam, Netherlands) virtual reality treadmill environment will isolate these visual contributions in human subjects as they walk. We plan to tease out the effect of OF direction on the relationship between gait and balance control by having healthy young adults perform a series of treadmill walking trials while immersed in a VR environment. Peripheral arterial disease (PAD) is a vascular disease where atherosclerotic blockages restrict blood flow to muscles in the lower extremities. Diabetes is a common co-morbid condition in PAD and has been shown to increase the risk of lower extremity amputation compared to PAD without diabetes. Although there is a clear pathological relationship between diabetes and PAD, the gait mechanics involved remain indistinct. Therefore, the purpose of this study was to determine whether the presence of diabetes in addition to PAD results in greater functional impairment during ambulation. Twenty-three subjects with PAD and twelve controls consented to participate in the research. …

Indirect calorimetry provides the average cost of a stride cycle and prevents from identifying which part of the gait cycle causes increased metabolic cost in patients, however, recent simulation methods allow estimating the time profile of metabolic cost within the stride cycle. In this study, we compare the estimations of the time profile of the metabolic cost of two simulation methods for level and uphill walking. Flexor hallucis longus (FHL) is a multiarticular muscle that possesses the potential to alter foot’s functional and mechanical behavior, since it performs multiple significant functions during walking (e.g supports the medial longitudinal arch of the foot).The purpose of this study was to determine if the functional changes of the FHL are associated with the foot’s adaptations to the mechanical demands of walking. Backward walking is an experimental method capable of altering how the forces propagate underneath the foot and disrupting the mechanical function of the FHL muscle. Air pollution is the primary environmental cause of premature death in the EU (European Commission, 2013) and the most problematic pollutants across Europe have consistently been oxides of nitrogen (e.g. nitrogen dioxide (NO2)), particulate matter (e.g. PM10, PM2.5) and ozone (O3). While measurements form an important aspect of air quality assessment, on their own they are unlikely to be sufficient to provide an accurate spatial and temporal description of the pollutant concentrations for exposure assessment and moreover they cannot provide information regarding future air quality. There are no current second-line antibiotics for children with Multidrug Resistant Tuberculosis (MDR TB). Current methods to treat pediatric MDR TB consist of splitting up adult formulated pills in accordance to the WHO’s recommended dosages, which depend on the weight and age of the child. This process is very inconsistent, tedious, and time consuming when using the state-of-the-art tools (kitchen knives, standard pill cutters). Under the sponsorship of Health Advocacy Innovations (HAI), we developed a mechanical tool that makes ingesting pills for child patients more convenient, consistent, and swift. OBJECTIVES: Extracorporeal membrane oxygenation provides short-term cardiopulmonary life support, but is associated with peripheral innate inflammation, disruptions in cerebral autoregulation, and acquired brain injury. We tested the hypothesis that extracorporeal membrane oxygenation also induces CNS-directed adaptive immune responses which may exacerbate extracorporeal membrane oxygenation-associated brain injury.
DESIGN: A single center prospective observational study.
SETTING: Pediatric and cardiac ICUs at a single tertiary care, academic center.
PATIENTS: Twenty pediatric extracorporeal membrane oxygenation patients (0-14 yr; 13 females, 7 males) and five nonextracorporeal membrane oxygenation Pediatric Logistic Organ Dysfunction score matched patients.

Background: Fat grafting is a growing field within plastic surgery. Adipose-derived stem cells (ASCs) and stromal vascular fracture (SVF) may have a role in fat graft survival. Our group previously demonstrated a detrimental effect on ASC survival by the lidocaine used in tumescent solution. Sodium bicarbonate (SB) buffers the acidity of lidocaine. The purpose of this study was to determine whether SB buffering is a practical method to reduce ASC and SVF apoptosis and necrosis seen with common lidocaine-containing tumescent solution. Extracorporeal life support (ECLS) is a class of technologies used to support or replace the function of failing organs. During ECLS, blood is withdrawn from systemic circulation and circulated through an artificial organ or “treatment membrane” that performs the function of the failing organ, prior to return to systemic circulation. While ECLS provides life-saving therapy to wide patient populations from pre-term infants to combat-wounded soldiers, this therapy is limited due to secondary thrombotic and bleeding complications that result from: 1) exposure of blood to the foreign surfaces in the device circuit and 2) administration of anticoagulant drugs to prevent clot …

Autism spectrum disorder (ASD) research has yet to leverage "big data" on the same scale as other fields; however, advancements in easy, affordable data collection and analysis may soon make this a reality. Indeed, there has been a notable increase in research literature evaluating the effectiveness of machine learning for diagnosing ASD, exploring its genetic underpinnings, and designing effective interventions. This paper provides a comprehensive review of 45 papers utilizing supervised machine learning in ASD, including algorithms for classification and text analysis. Background: Foot orthoses have proven to be effective for conservative management of various pathologies. Pathologies of the lower limb can be caused by abnormal biomechanics such as abnormal foot structure and alignment, leading to inadequate support. Objectives: To compare biomechanical effects of different foot orthoses on the medial longitudinal arch (MLA) during dynamic gait using skeletal kinematics. Ligands that accelerate nanoceria dissolution may greatly affect its fate and effects. This project assessed the carboxylic acid contribution to nanoceria dissolution in aqueous, acidic environments. Nanoceria has commercial and potential therapeutic and energy storage applications. It biotransforms in vivo. Citric acid stabilizes nanoceria during synthesis and in aqueous dispersions. In this study, citrate-stabilized nanoceria dispersions (∼4 nm average primary particle size) were loaded into dialysis cassettes whose membranes passed cerium salts but not nanoceria particles. The cassettes were immersed in iso-osmotic baths containing carboxylic acids at pH 4.5 and 37 °C, or other select agents. Cerium atom material …

Optical coherence tomography (OCT) is used to provide anatomical information of biological systems but can also provide functional information by characterizing the motion of intracellular structures. Dynamic light scattering OCT was performed on intact, control MCF-7 breast cancer cells and cells either treated with paclitaxel to induce apoptosis or deprived of nutrients to induce oncosis. Autocorrelations (ACs) of the temporal fluctuations of OCT intensity signals demonstrate a significant decrease in decorrelation time after 24 h in both the paclitaxel-treated and nutrient-deprived cell groups but no significant differences between the two groups. Background: Although multilingual interfaces are preferred by most users when they have a choice, organizations are often unable to support and troubleshoot problems involving multiple user languages. Software that has been structured with multiple languages and data interlinking considerations early in its development is more likely to be easily maintained. We describe the process of adding multilingual support to the CAPO international Cohort study database using REDCap.

Ensemble methods, such as the traditional bagging algorithm, can usually improve the performance of a single classifier. However, they usually require large storage space as well as relatively time-consuming predictions. Many approaches were developed to reduce the ensemble size and improve the classification performance by pruning the traditional bagging algorithms. In this article, we proposed a two-stage strategy to prune the traditional bagging algorithm by combining two simple approaches: accuracy-based pruning (AP) and distance-based pruning (DP). Oxygen is vital to the function of all tissues including the liver and lack of oxygen, that is, hypoxia can result in both acute and chronic injuries to the liver in vivo and ex vivo. Furthermore, a permanent oxygen gradient is naturally present along the liver sinusoid, which plays a role in the metabolic zonation and the pathophysiology of liver diseases. Accordingly, here, we introduce an in vitro microfluidic platform capable of actively creating a series of oxygen concentrations on a single continuous microtissue, ranging from normoxia to severe hypoxia. Today's web-enabled and virtual approach to medical education is different from the 20th century's Flexner-dominated approach. Now, lectures get less emphasis and more emphasis is placed on learning via early clinical exposure, standardized patients, and other simulations. This article reviews literature on virtual patients (VPs) and their underlying virtual reality technology, examines VPs' potential through the example of psychiatric intake teaching, and identifies promises and perils posed by VP use in medical education.
