Background: Hypoxaemia predicts mortality at all levels of care, and appropriate management can reduce preventable deaths. However, pulse oximetry and oxygen therapy remain inaccessible in many primary care health facilities. We aimed to develop and validate a simple risk score comprising commonly evaluated clinical features to predict hypoxaemia in 2-59-month-old children with pneumonia.

Methods: Data from seven studies conducted in five countries from the Pneumonia Research Partnership to Assess WHO Recommendations (PREPARE) dataset were included. Readily available clinical features and demographic variables were used to develop a multivariable logistic regression model to predict hypoxemia (oxygen saturation < 90%) at presentation to care. The adjusted log coefficients were transformed to derive the PREPARE hypoxemia risk score and its diagnostic value was assessed in a held-out, temporal validation dataset. The model and risk score were analysed by evaluating the area under the receiver operating characteristic curve (AUC), sensitivity and specificity.

The arrangement of neurons into ordered layers underlies circuit function in many nervous system regions. This is particularly true in the mammalian retina. Here, fate-committed retinal ganglion cells (RGCs) migrate from the apical to the inner retina, where they form connections that enable vision. The mechanisms that permit ganglion cell migration and whether distinct ganglion cell types use different migration modes are unknown. We show that the serine/threonine kinase LKB1 regulates ganglion cell migration and nuclear positioning. In the absence of LKB1, many ganglion cells remain in the apical retina. Misplaced cells show modified morphologies and display altered cytoskeletal proteins. …

Background: Catheter ablation procedures with transseptal punctures (usually for atrial fibrillation) are often associated with migraine-related visual auras, but the mechanism remains unknown. Whether this phenomenon is mediated by the creation of an atrial septal defect from transseptal puncture or by silent acute brain emboli detected on magnetic resonance imaging related to the procedure remains to be investigated.

Objective: This study aimed to evaluate whether randomization to a transseptal puncture during catheter ablation for ventricular arrhythmias is associated with postprocedural visual auras and assess the relationship between occipital and parietal lobes acute brain emboli and migraine-related visual auras.

This paper studies the interaction of Song, Story, Time, and Fate in the context of the Elves' resistance to change by means of song and enchantment, specifically in Lothlórien until the arrival of the Fellowship of the Ring. Once Galadriel renounces the Ring and the resistance to change and the fading of the Elves, the mythic power of the Great Tales transforms the ability of Frodo and Sam to meet the fate that awaits them in Mordor.

Background: Nurse-to-nurse bullying is a critical issue within the nursing profession, impacting close to half of all nurses. This phenomenon, articulated by the phrase "nurses eat their young," detrimentally influences employee retention, job satisfaction, morale, and ultimately, patient care. Furthermore, nurse-to-nurse bullying significantly affects the well-being of nursing professionals, particularly targeting novice nurses. July 6, 2025, marks the 140-year anniversary of the first successful treatment of the rabies virus, discovered by Louis Pasteur. Louis Pasteur’s contributions and reputation are ever prevalent today, with “pasteurized” products being the signature of his legacy in microbiology. Through the creation of the “pasteurization” process (the gentle heating of food and drink products to eliminate pathogens), Louis Pasteur eloquently stated his motive for scientific discovery: “Man’s first look at the universe discovers only variety, diversity, multiplicity of phenomena. Despite widespread adoption of no-fault divorce, many U.S. states still consider adultery in the allocation of alimony and property. In sixteen of those states, adultery remains a criminal offense—enabling spouses accused of infidelity to invoke the Fifth Amendment privilege against self-incrimination during divorce proceedings. This Article explores how such invocations obstruct discovery, delay litigation, and create strategic advantages for wealthier or culpable spouses, especially in fault-based or hybrid divorce regimes. Zoom Fatigue (ZF), or physical and neuropsychological responses to excessive videoconferencing (VC), increased during the COVID-19 pandemic. Like burnout, ZF can compromise patient safety by causing decreased job satisfaction, social isolation, disrupted focus, and difficulty empathizing with patients. This Plan-Do-Study-Act quality improvement project aimed to assess feelings about VC, raise awareness of the adverse effects of overuse, and empower primary care providers (PCPs) to improve their work experiences. In multi-messenger astrophysics, signals of multiple types (e.g., gravitational waves, neutrinos, electromagnetic waves) are combined in an effort to learn more about the observed phenomena of interest. The Advanced Particle-astrophyics Telescope (APT) is a mission concept for a space-borne instrument that detects gammaray bursts (GRBs) omnidirectionally, facilitating multi-messenger observations by identifying and localizing celestial events of interest. Here, we describe the on-instrument computations for APT and its Antarctic Demonstrator (ADAPT) as well as techniques for follow-up observations of transient events.

In June 2025, U.S. employment rose by 147,000, with gains in state government and health care, while the overall unemployment rate remained steady at 4.1%. Veteran unemployment slightly increased overall, though trends varied by age, gender, and race, with younger veterans and Hispanic veterans seeing improvements, while older veterans and Post-9/11 veterans experienced higher unemployment rates.

Depression, anxiety, and perceived stress are common comorbidities in patients with inflammatory bowel disease (IBD) and may negatively influence the disease course. Likewise, severe IBD may contribute to the development or worsening of psychiatric symptoms. Despite the established relevance of the gut-brain axis and frequent use of psychotropic medications in IBD patients, limited evidence exists regarding the effects of psychiatric treatments on gastrointestinal disease activity. Therefore, the aim of this systematic review is to evaluate the effectiveness of psychiatric therapies on gastrointestinal symptoms and disease activity in patients with IBD. Background/Objectives: Dopamine partial agonists are drugs initially developed to treat schizophrenia, seeking a double effect of increased dopaminergic transmission in the prefrontal cortex and decrease in the accumbens/striatum. Of these drugs, aripiprazole, brexpiprazole, and cariprazine are currently marketed and used in schizophrenia spectrum and mood disorders. It is debated whether patients with psychiatric disorders becoming pregnant should discontinue or continue their antipsychotic treatment despite some risks for the fetus, i.e., whether it is worse to have an untreated disorder or treating it with drugs. In Elaine McMillion Sheldon’s documentary King Coal (2023) coal is being referred to as a matter that is “not dirty or clean, but elemental.” The acclaimed film centers on the complex histories and cultures of coal in Central Appalachia, based on the perspective of Sheldon, a coal miner’s daughter, and a West Virginia native. The elemental nature of coal is explored in the film by situating coal less as a resource and more as a mythic foundation for the community's cultural history. Hellbenders are prehistoric salamanders that have thrived in cold rivers since before the continents divided. Once found throughout Appalachian watersheds, due to exploitation and habitat degradation only a few dozen healthy populations survive. Monstrous yet fascinating, hellbenders were often reviled historically, but in recent decades they have gained iconic status and are now celebrated as charismatic indicators for clean rivers. Hellbenders are symbols of the wild because of their ancient history, elusive nature, and contribution to ecosystem resilience. Treatments for generalized anxiety disorder (GAD) often aim to address maladaptive approach-avoidance behavior patterns. Approach-avoidance conflict (AAC) offers a potential framework for identifying treatment outcome predictors and informing optimization of GAD treatment. The current study examined whether pre-treatment neurobehavioral AAC indices predict symptom improvement in behavioral activation (BA) and exposure therapy (EXP) for GAD. Treatment-seeking adults meeting criteria for GAD completed a randomized clinical trial with pre-treatment blinding, conducted from 2016-2021. Participants were randomized to complete 10 manualized sessions of BA or EXP. Participants completed an AAC task during functional magnetic resonance imaging pre-treatment. This study used an equity ethic framework and a STEM identity model to contextualize and understand Latine students’ perceptions of STEM identity development. The purpose of this study was to investigate how Latine undergraduate students who engaged in STEM coursework perceived their learning experiences and stories of resilience through an equity ethic framework. Data were collected through interviews with 19 Latine college students attending a Hispanic-Serving Institution. Findings revealed the following themes related to Latine students’ STEM identity development and lived experiences in STEM coursework: implicit biases, imposter syndrome, self-efficacy, and support system and resources. Background: Hikikomori syndrome involves voluntary withdrawal from social life, school and work, with onset typically in young adulthood. Hikikomori risk has not been examined in the UK, and these studies aimed to validate and refine the Hikikomori Risk Inventory-24 (HRI-24) screening tool in UK young adults. Methods/Results: In Study 1, participants (n = 341) completed the HRI-24. Item analysis and confirmatory factor analysis resulted in a shorter 13-item HRI (HRI-13), which demonstrated a perfect correlation with the full HRI-24. Both the HRI-13 and HRI-24 showed strong convergent and divergent validity, correlating with depression, anxiety, avoidant coping and negative early life …

This article describes the creation and evaluation designed to support student success through their inclusion in a variety of professional and technical training programs. The opportunity to develop a leadership academy specifically for engineering students allowed faculty program coordinators to deliver content that was normally not available to students at this Hispanic-Serving Institution (HSI). We implemented and evaluated the leadership academy using a mixed-methods approach involving multimedia interviews and two traditional survey data collection techniques. BACKGROUND: Takotsubo syndrome (TTS) is considered as a transient acute heart failure syndrome predominant in females. However, data on Chinese TTS patients are limited. Therefore, this study aimed to investigate the clinical characteristics of Chinese TTS patients, identify prognostic markers.

METHODS: A total of 258 TTS patients were enrolled. Data on clinical characteristics and in-hospital major adverse cardiovascular events (MACE) were collected and analyzed. The latter included severe heart failure, shock, malignant arrhythmia, and death.

RESULTS: The average age of TTS patients was 59.3 ± 19.2 years. Female patients accounted for 68.9%. MACE occurred in 41.4% of patients during hospitalization. …
