Primary Mediastinal B-Cell Lymphoma Presenting As Chest Pain In A Young Woman And Treated With Epoch-R, Chloe Weidenbaum, Akanksha Kushwah, Ariana Geromes, Jonathan A. Abbas

Journal of Community Hospital Internal Medicine Perspectives

Primary mediastinal B-cell lymphoma (PMBCL) is a rare type of diffuse large B-cell lymphoma, constituting 2-4% of non-Hodgkin lymphomas. Here we present a case of PMBCL initially presenting as pleuritic chest pain in an otherwise healthy 33-year-old woman. PMBCL typically presents as a large, fast-growing tumor limited to the mediastinum, making the iliosacral involvement in this case unusual. R-CHOP is the most commonly used chemotherapy regimen, although more aggressive therapies like EPOCH-R can spare the need for consolidative radiation therapy. PMBCL represents one of the greatest diagnostic and therapeutic challenges in malignant hematology.

BACKGROUND: Targeted exercise training is a promising strategy for promoting cognitive function and preventing dementia in older age. Despite the utility of exercise as an intervention, variation still exists in exercise-induced cognitive gains and questions remain regarding the type of training (i.e., what), as well as moderators (i.e., for whom) and mechanisms (i.e., how) of benefit. Both aerobic training (AT) and resistance training (RT) enhance cognitive function in older adults without cognitive impairment; however, the vast majority of trials have focused exclusively on AT. Feedstuffs utilized in U.S. feedlot finishing rations incorporate high concentrations of N and P, with less than 15% of fed N and P retained by the animal. The remaining N and P are excreted in the manure, where the opportunity for manure N loss via ammonia (NH3) volatilization from the feedlot pen surface is a risk to the environment and lowers the value of manure as a fertilizer. Background: Access to biologically available nitrogen is a key constraint on plant growth in both natural and agricultural settings. Variation in tolerance to nitrogen deficit stress and productivity in nitrogen limited conditions exists both within and between plant species. The purpose of this phenomenological study is to understand the attitudes and perceptions of Hispanics living in Tampa Bay, Florida regarding foster parent roles. The theory guiding this study is the researcher’s axiological philosophical assumption. The philosophical assumption is applied within the social constructivist interpretive framework. In 1995, the Institute of Medicine (IOM) published Dental Education at the Crossroads: Challenges and Change, advocating for a revolution in dental education. Notably, it recommended that course content, structure, and delivery be modernized in response to graduating clinicians lacking the critical thinking and problem-solving skills required in a patient care setting. Infection of human peripheral blood cells by SARS-CoV-2 has been debated because immune cells lack mRNA expression of both angiotensin-converting enzyme 2 (ACE2) and transmembrane serine protease type 2 (TMPRSS2). Herein we demonstrate that resting primary monocytes harbor abundant cytoplasmic ACE2 and TMPRSS2 protein and that circulating exosomes contain significant ACE2 protein. Upon ex vivo TLR4/7/8 stimulation, cytoplasmic ACE2 was quickly translocated to the monocyte cell surface independently of ACE2 transcription, while TMPRSS2 surface translocation occurred in conjunction with elevated mRNA expression. Many transcription factors regulating the production, survival, and function of photoreceptor cells have been identified, but little is known about transcriptional co-regulators in retinal health and disease. Here, we show that BCL6 co-repressor (BCOR), a Polycomb repressive complex 1 factor mutated in various cancers, is involved in photoreceptor degenerative diseases. Using proteomics and transcription assays, we report that BCOR interacts with the transcription factors CRX and OTX2 and reduces their ability to activate the promoters of photoreceptor-specific genes. CUT&RUN sequencing further shows that BCOR shares genome-wide binding profiles with CRX/OTX2, consistent with a general co-repression activity. We also identify missense …

Graduate students' mental health and well-being is a prominent concern across various disciplines. However, early predictors of mental health and well-being in graduate education, specifically doctoral education, have rarely been studied. The present study evaluated both the underlying latent classification of individuals' mental well-being and predictors of those classifications. Results estimated two latent classes of students' mental health and well-being: one class with generally high levels of mental well-being and one with lower levels of mental well-being. Long-duration spaceflight poses multiple hazards to human health, including physiological changes associated with microgravity. The hemodynamic adaptations occurring upon entry into weightlessness have been associated with retrograde stagnant flow conditions and thromboembolic events in the venous vasculature but the impact of microgravity on cerebral arterial hemodynamics and function remains poorly understood. The objective of this study was to quantify the effects of microgravity on hemodynamics and wall shear stress (WSS) characteristics in 16 carotid bifurcation geometries reconstructed from ultrasonography images using computational fluid dynamics modeling. Documenting genome diversity is important for the local biomedical communities and instrumental in developing precision and personalized medicine. Currently, tens of thousands of whole-genome sequences from Europe are publicly available, but most of these represent populations of developed countries of Europe. The uneven distribution of the available data is further impaired by the lack of data sharing. Recent whole-genome studies in Eastern Europe, one in Ukraine and one in Russia, demonstrated that local genome diversity and population structure from Eastern Europe historically had not been fully represented. An aggressive form of neuroblastoma (NB), a malignant childhood cancer derived from granule neuron precursors and sympathoadrenal lineage, frequently comprises MYCN amplification/elevated N-Myc expression, which contributes to the development of neural crest-derived embryonal malignancy. N-Myc is an oncogenic driver in NB. Persistent N-Myc expression during the maturation of SA precursor cells can cause blockage of the apoptosis and induce abnormal proliferation, resulting in NB development. An insulinoma-associated-1 (INSM1) zinc-finger transcription factor has emerged as an NB biomarker that plays a critical role in facilitating tumor cell growth and transformation. Pediatric autoimmune neuropsychiatric disorder associated with streptococcal infection (PANDAS) starts as the child’s body has an inappropriate reaction to the immune response for streptococcal infection. PANDAS is characterized by the sudden onset of neuropsychiatric disorders, including obsessive-compulsive disorder (OCD), anxiety, chorea movements, and tics. Previous research has drawn conflicting conclusions on PANDAS treatment efficacy and long-term treatment outcomes leaving remission rates unclear. Minimal Covid-19 Quieting Measured In The Deep, Offshore Waters Of The U.S. Outer Continental Shelf, Jennifer L. Miksis-Olds, S. Bruce Martin, Kim Lowell, Christopher Verlinden

Using a 2-year time series (2019–2020) of 1-min sound pressure level averages from seven sites, the extension of COVID-related quieting documented in coastal soundscapes to deep (approximately 200–900 m) waters off the southeastern United States was assessed. Sites ranged in distance to the continental shelf break and shipping lanes. Sound level decreases in 2020 were observed at sites closest to the shelf break and shipping lanes but were inconsistent with the timing of shipping changes related to a COVID-19 slowdown. These observations are consistent with increased numbers of vessel tracks in 2020 compared to 2019 at a majority of sites.

Drosophila melanogaster has been extensively used in different types of research as a model organism due to its high reproductivity, short life span, and easy maintenance. In this paper, we examined drosophila as a model for ethanol toxicity in the male reproductive system. We developed three assays: The DAPI assay, sperm viability assay, and tubulin staining assay. The DAPI assay is a nuclear staining assay that assesses the effects of various ethanol concentrations in late spermiogenesis in which increased detrimental effects to the sperm bundles were observed. The purpose of this quantitative, quasi-experimental study was to understand the difference in the posttest detectability scores, as measured by the Conners Kiddie Continuous Performance Test 2nd edition, among early childhood students participating in various activity types, including ATNR integration exercises, running exercises, or sedentary work, when controlling for pretest detectability scores at schools in rural north Texas using a nonequivalent control-group, pretest-posttest design.
