ปัจจุบัน พ.ศ. 2562 จำนวนประชากรผู้สูงอายุของประเทศไทยมีมากกว่าประชากรวัยเด็ก และมีแนวโน้มสูงขึ้นทุกปี ทำให้อัตราส่วนของวัยแรงงานในการเลี้ยงดูผู้สูงอายุ 1 คนลดลงเรื่อยมา จากข้อมูลของสำนักงานสถิติแห่งชาติ ดังนั้น การดูแลโดยครอบครัวจึงไม่เพียงพออีกต่อไป สาเหตุส่วนหนึ่งเกิดจากอัตราการเกิดที่ลดลง โดยเหตุผลที่ครอบครัวสมัยใหม่มีบุตรน้อยลง เพราะวัฒนธรรมการทำงาน และขนาดครอบครัวที่เล็กลง ทำให้ขาดผู้ดูแลบุตรในเวลากลางวัน จากสถานการณ์ข้างต้น ภาครัฐจึงออกนโยบายหลายด้านเพื่อขานรับการเปลี่ยนแปลง รวมถึงกระตุ้นความร่วมมือจากภาคเอกชน มาตรการทางผังเมืองเป็นมาตรการหนึ่งที่สนับสนุนให้ภาคเอกชนร่วมจัดตั้งบริการทางสังคม โดยร่างผังเมืองรวมกรุงเทพฯ ฉบับปรับปรุงครั้งที่ 4 ได้เพิ่มเกณฑ์การจัดให้มีสถานดูแลผู้สูงอายุ และสถานเลี้ยงเด็กในเวลากลางวันในมาตรการเพิ่มพื้นที่อาคารรวมต่อพื้นที่ดิน (FAR Bonus) จึงเกิดคำถามในการนำกฎหมายดังกล่าวไปปฏิบัติจริง โดยเลือกศึกษาอาคารชุดพักอาศัยที่มีแนวโน้มเพิ่มขึ้นในกรุงเทพฯ แนวทางการศึกษาอาศัยความคิดเห็นของผู้เชี่ยวชาญเฉพาะด้าน เพื่อการคาดการณ์ที่น่าเชื่อถือ เทคนิคเดลฟาย (Delphi Technique) จึงถูกนำมาใช้ในการคัดเลือกผู้ให้สัมภาษณ์ การสัมภาษณ์ และการตอบแบบสอบถาม โดยมีกลุ่มประชากร 4 กลุ่ม คือ กลุ่มผู้พัฒนาโครงการอาคารชุดพักอาศัย ผู้เชี่ยวชาญด้านสถานดูแลผู้สูงอายุ และสถานเลี้ยงเด็กในเวลากลางวัน และผู้เชี่ยวชาญทางผังเมือง รวม 19 ท่าน สำหรับการศึกษาด้านความคุ้มค่าในการลงทุน อาศัยแบบจำลองการเพิ่มพื้นที่ของกรณีศึกษาอาคารชุดพักอาศัย จำนวน 11 โครงการ แบ่งตามระดับราคา เพื่อนำมาคำนวณหาผลตอบแทนและต้นทุนเมื่อพื้นที่เพิ่มขึ้น โดยดัดแปลงวิธีการคำนวณจาก อัตราส่วนกำไรสุทธิต่อต้นทุน (Return on Investment หรือ ROI) ผลที่ได้จึงนำมาวิเคราะห์ร่วมกับตัวแปรด้านระดับราคา และค่า FAR ของที่ตั้งโครงการ ผลการศึกษาความเห็นจากผู้พัฒนาโครงการอาคารชุดพักอาศัย และผู้เชี่ยวชาญแต่ละด้าน พบว่า มาตรการส่งเสริมดังกล่าวมีความเป็นไปได้ในการดำเนินงาน ขึ้นอยู่กับลักษณะโครงการ และกลุ่มเป้าหมาย กล่าวคือ โครงการระดับราคาปานกลาง มีความเหมาะสมในการเข้าร่วมเกณฑ์มากที่สุด เพราะที่ตั้งโครงการอยู่ในทำเลที่มีความต้องการเพิ่มพื้นที่อาคาร และขนาดของโครงการทำให้โอกาสที่จำนวนผู้ใช้บริการจะมีมากขึ้น ด้านความคุ้มค่าในการลงทุน ผลจากแบบจำลองการเพิ่มพื้นที่แสดงให้เห็นว่า เกณฑ์การจัดให้มีสถานดูแลผู้สูงอายุ และสถานเลี้ยงเด็กในเวลากลางวัน มีความคุ้มค่าในการลงทุน โดยมีสัดส่วนผลตอบแทนต่อต้นทุนมากที่สุดในโครงการระดับราคาสูง และลดลงในโครงการระดับราคาปานกลาง และระดับราคาต่ำ และยังมีแนวโน้มแปรผันตรงกับค่า FAR ของที่ตั้งโครงการ ด้านข้อจำกัดในการเข้าร่วมเกณฑ์ ครอบคลุมในเรื่องการเปลี่ยนแปลงการใช้งานพื้นที่ ความต้องการเพิ่มพื้นที่อาคาร ซึ่งสวนทางกับความต้องการสถานดูแลในเวลากลางวันเมื่อเปรียบเทียบระหว่างพื้นที่ในเมือง และชานเมือง รวมถึงการจัดหาผู้ประกอบการสถานดูแลในเวลากลางวันที่เชี่ยวชาญ

ในปัจจุบัน โลกอยู่ในยุคของเศรษฐกิจฐานความรู้ (Knowledge-based economy) ที่มีความรู้เป็นตัวขับเคลื่อนหลักในการสร้างความเติบโตและความมั่งคั่งของเศรษฐกิจ โดยมีทุนมนุษย์เป็นหนึ่งในปัจจัยสำคัญในการขับเคลื่อนเศรษฐกิจ ในประเทศไทย รัฐบาลได้นำแนวคิด ประเทศไทย 4.0 มาเป็นวิสัยทัศน์เชิงนโยบาย ที่ต้องการเปลี่ยนเศรษฐกิจแบบเดิมไปสู่เศรษฐกิจฐานความรู้ที่ขับเคลื่อนด้วยนวัตกรรม แต่ทว่าทุนมนุษย์ของประเทศไทยยังมีอยู่จำกัด โดยเฉพาะการขาดแคลนทรัพยากรบุคคลในด้านเทคโนโลยีและทักษะชั้นสูงในด้านต่างๆ นอกเหนือจากการทำงานที่ต้องการความรู้และความเชี่ยวชาญชั้นสูงให้กับองค์กรและบริษัทต่างๆ แล้ว แรงงานต่างชาติทักษะสูง ยังเป็นกลุ่มบุคคลที่มีบทบาทในการพัฒนาเศรษฐกิจฐานความรู้ ด้วยการเผยแพร่แลกเปลี่ยนองค์ความรู้ (Knowledge spillover) ที่ช่วยเสริมสร้างความรู้และความเชี่ยวชาญให้กับทรัพยากรบุคคลในประเทศไทย เพิ่มศักยภาพให้กับทุนมนุษย์ของประเทศ ในช่วงหลายปีที่ผ่านมา ประเทศไทยได้ประสบกับภัยพิบัติหลากหลายรูปแบบ เช่น คลื่นสึนามิใน พ.ศ. 2547 อุทกภัยใน พ.ศ. 2554 และภัยพิบัติหมอกควัน รวมถึงจลาจลและความไม่สงบทางการเมือง ส่งผลต่อความมั่นใจในการลงทุนในประเทศไทยของนักลุงทุนต่างชาติ ที่อาจก่อให้เกิดความไม่มั่นใจในการเข้ามาทำงานในประเทศไทยของแรงงานต่างชาติทักษะสูง ซึ่งเป็นกลุ่มคนที่มักมีทุนทางสังคม (Social capital) ต่ำเมื่อเทียบกับคนไทยที่อยู่ในท้องถิ่นในด้านการเตรียมตัวและรับมือภัยพิบัติ งานวิจัยนี้จึงเป็นงานวิจัยชิ้นแรกๆ ในการศึกษาทุนทางสังคมที่เกี่ยวข้องกับการเตรียมตัวและรับมือภัยพิบัติของแรงงานต่างชาติทักษะสูง โดยใช้กรณีศึกษาของแรงงานต่างชาติทักษะสูงในกรุงเทพมหานคร โดยใช้การวิจัยเชิงคุณภาพ (Qualitative research) ด้วยการวิเคราะห์ทุนทางสังคมและวิเคราะห์โครงข่ายทางสังคม (Social network analysis) จากการทำการสัมภาษณ์เชิงลึกของกลุ่มตัวอย่าง 20 คนที่ทำงานในองค์กรระหว่างประเทศ สถานฑูต มหาวิทยาลัย และบริษัทข้ามชาติ ผลของงานวิจัยพบว่า ทุนทางสังคมของแรงงานต่างชาติทักษะสูงไม่ได้เกิดขึ้นในพื้นที่ชุมชนอยู่อาศัยในกรุงเทพมหานคร แต่เกิดขึ้นที่สถานที่ทำงานและสถานที่พักผ่อน ดังนั้นการวางแผนการเตรียมตัวและรับมือภัยพิบัติของแรงงานต่างชาติทักษะสูงจึงควรให้ความสำคัญกับพื้นที่บริเวณสถานที่ทำงาน ซึ่งแตกต่างกับการวางแผนการเตรียมตัวและรับมือภัยพิบัติของคนในท้องถิ่นที่มีทุนทางสังคมในชุมชนสูง

การวิจัยนี้มีวัตถุประสงค์ เพื่อศึกษาความสัมพันธ์ระหว่าง ความเครียด ความเหนื่อยล้าในการเห็นอกเห็นใจ ความพึงพอใจในการเห็นอกเห็นใจ และสุขภาวะของนักวิชาชีพผู้ให้บริการด้านสุขภาพจิต กลุ่มตัวอย่างคือ นักวิชาชีพผู้ให้บริการด้านสุขภาพจิตจำนวน 149 คนอายุเฉลี่ย 33.81 ± 8.79 ปี เครื่องมือที่ใช้ในการวิจัยได้แก่ 1) มาตรวัดการรับรู้ความเครียด 2) มาตรวัดความเหนื่อยล้าในการเห็นอกเห็นใจ 3) มาตรวัดความพึงพอใจในการเห็นอกเห็นใจ และ 4) มาตรวัดสุขภาวะแบบ 5 องค์ประกอบ วิเคราะห์ข้อมูลโดยค่าสถิติสหสัมพันธ์แบบเพียร์สันและ การวิเคราะห์การถดถอยพหุคูณ ผลการวิจัยพบว่า ความเครียด และความเหนื่อยล้าในการเห็นอกเห็นใจ มีค่าสหสัมพันธ์ทางลบกับสุขภาวะของนักวิชาชีพสุขภาพจิตอย่างมีนัยสำคัญทางสถิติที่ .001 (r = -.47 และ -.34 ตามลำดับ, p < .001) ในขณะที่ความพึงพอใจในการเห็นอกเห็นใจมีค่าสหสัมพันธ์บวกกับสุขภาวะของนักวิชาชีพสุขภาพจิตอย่างมีนัยสำคัญทางสถิติที่ .001 (r = .67, p < .001) โดยที่ความเครียด ความอ่อนล้าในการเห็นอกเห็นใจ และความพึงพอใจในการเห็นอกเห็นใจ สามารถร่วมกันทำนายสุขภาวะของนักวิชาชีพทีให้บริการด้านสุขภาพจิตอย่างมีนัยสำคัญทางสถิติ และสามารถอธิบายความแปรปรวนของสุขภาวะในนักวิชาชีพทีให้บริการด้านสุขภาพจิตได้ ร้อยละ 47.8 (R2 = .478, p < .001) ในขณะที่ตัวแปรความเหนื่อยล้าในการเห็นอกเห็นใจ ไม่สามารถทำนายสุขภาวะของนักวิชาชีพสุขภาพจิตได้อย่างมีนัยสำคัญทางสถิติ (R2 = .478, p = .346) ซึ่งความเครียด มีน้ำหนักในการทำนายสูงที่สุด (β = -.81, p < .01) ตามมาด้วย ความพึงพอใจในการเห็นอกเห็นใจ (β = .59, p < .001)

การศึกษาวิจัยเรื่อง "การคุ้มครองสิทธิและการเข้าถึงความยุติธรรมของคนไร้ที่พึ่งในสถานคุ้มครองคนไร้ที่พึ่ง" มีวัตถุประสงค์เพื่อศึกษาค้นคว้า กฎหมาย หลักการ แนวปฏิบัติเพื่อคุ้มครองสิทธิ สภาพปัญหา สาเหตุ อุปสรรค ผลกระทบ การเยียวยา สถานการณ์การละเมิดสิทธิและการเข้าไม่ถึงความยุติธรรม และเสนอแนะแนวทางการปฏิบัติที่เหมาะสมต่อการคุ้มครองสิทธิและการเข้าถึงความยุติธรรมของคนไร้ที่พึ่งในสถานคุ้มครองคนไร้ที่พึ่ง การวิจัยครั้งนี้เป็นการศึกษาโดยใช้วิธีวิจัยแบบผสานวิธี (Mixed method) คือเชิงปริมาณและเชิงคุณภาพ รวมถึงการศึกษาทบทวนเอกสารงานวิจัยที่เกี่ยวข้อง (Documentary research) ตามแนวคิดการคุ้มครองสิทธิและการเข้าถึงความยุติธรรม ผลการวิจัยสรุปสาระสำคัญได้ 2 ประการดังต่อไปนี้ ประการแรก การถูกละเมิดสิทธิ (Rights Infringement) พบปัญหาหลายประการทั้งด้านกฎหมาย นโยบาย การปฏิบัติและองค์ความรู้ เช่น (1) พ.ร.บ คุ้มครองคนไร้ที่พึ่ง พ.ศ. 2557 กีดกันคนบางกลุ่มออกจากสถานคุ้มครองคนไร้ที่พึ่งเนื่องจากกฎหมายคุ้มครองเฉพาะกลุ่มที่เกี่ยวข้อง (2) สถานคุ้มครองคนไร้ที่พึ่งมีกฎระเบียบมากเกินไป ขาดการเชื่อมโยงฐานข้อมูลและขาดระเบียบวิธีการปฏิบัติงานที่ชัดเจนระหว่างหน่วยงานที่เกี่ยวข้อง (3) คนไร้ที่พึ่งส่วนใหญ่ขาดการรับรู้เรื่องสิทธิของตนเอง (4) เจ้าหน้าที่และบุคลากรไม่เพียงพอ ขาดองค์ความรู้ และทักษะการปฏิบัติงานด้านสังคมสงเคราะห์ ประการที่สอง การเข้าไม่ถึงความยุติธรรม (Inaccessibility to justice) พบปัญหาหลายประการ เช่น (1) เจ้าหน้าที่ตำรวจใช้วิธีการล่อลวงและควบคุมตัวคนไร้ที่พึ่งเพื่อเข้าสู่กระบวนการคัดกรองคุณสมบัติโดยปราศจากความยินยอม (2) การเลือกปฏิบัติของเจ้าหน้าที่ในบางสถานคุ้มครองคนไร้ที่พึ่ง (3) สถานคุ้มครองคนไร้ที่พึ่งยังขาดมาตรการ หลักเกณฑ์และกองทุนการเยียวยากรณีคนไร้ที่พึ่งที่ได้รับการปฏิบัติที่ไม่เหมาะสม (4) คนไร้ที่พึ่งไม่กล้าร้องเรียนปัญหาเนื่องจากเกรงผลกระทบต่อการใช้บริการ ผลการวิจัยในครั้งนี้สามารถนำไปใช้ในการปรับปรุงกฎหมาย นโยบาย ระเบียบ กลไกลและหลักการปฏิบัติในการคุ้มครองสิทธิและการเข้าถึงความยุติธรรมของคนไร้ที่พึ่งในอนาคต

Alcoholics Anonymous (AA) meeting attendance and spiritual practices are established predictors of abstinence. This study utilized longitudinal data from two studies of AA to investigate 1) how perceived within meeting social dynamics in AA meetings affect later AA attendance, and thus exposure to the emphasis of spiritual practices and 2) influence the extent that spiritual gains are mobilized, beyond AA attendance. Findings revealed that greater group cohesion was associated with lower AA attrition, and that expressiveness, or openness, of the group predicted reported practice of spiritual behaviors. Motivational Interviewing (MI) is a known effective intervention for alcohol use disorder (AUD). MI's mechanisms of action remain inconsistently substantiated, and research in this area has been reliant on identifying relationships through strength of association rather than experimental manipulation of active ingredients. In two previous studies, a pilot and a larger replication study, we disaggregated MI into its hypothesized active ingredients by creating three conditions: MI, Spirit Only MI (SOMI, in which evocation of change talk was proscribed), and a non-therapy condition (NTC). Research has shown undeniable evidence of mental illness stigma. Stigma has been shown to reduce treatment seeking and negatively impact emotion and cognition in individuals with mental illness (Livingstone & Boyd, 2010). By discovering the driving forces behind stigma, treatment seeking and quality of life can be improved for individuals with mental illness. This study investigates the effect of knowledge, disorder type, and causal attribution on mental illness stigma. Specifically, participants were assigned to one of two conditions, knowledge or no knowledge. This paper reviews the issues confronting chaplaincy/spiritual care in the 21st century. It looks at how faith communities are changing their view of chaplaincy as well how institutions respond. The paper looks at two qualitative studies and what can be learned from them in confronting the questions raised at the beginning. It concludes with the question of how the evidence base can be expanded to make chaplaincy/spiritual care more relevant over the next few years.

Background: Despite family meetings being widely used to facilitate discussion among patients, families, and clinicians in palliative care, there is limited evidence to support their use. This study aims to assess the acceptability and feasibility of Patient-Centred Family Meetings in specialist inpatient palliative care units for patients, families, and clinicians and determine the suitability and feasibility of validated outcome measures from the patient and family perspectives.

In recognition of the ongoing health disparities experienced by Aboriginal and Torres Strait Islander peoples (hereafter Aboriginal), this scoping review explores the role and impact of the clinical communication process on Aboriginal healthcare provision. A medical education lens is applied, looking at the utility of a tailored clinical communication framework to assist health practitioners work more effectively with Aboriginal peoples and communities. The initial framework, building on existing communication guides, proposes four domains: content, process, relational and environmental. Aim: To explore the impact of cultural factors on the provision of end-of-life care in a geriatric inpatient rehabilitation setting.

Background: Australia’s ageing population is now also one of the most culturally diverse. Individuals from culturally and linguistically diverse backgrounds may have specific care needs at the end of life according to various aspects of their culture.

Design: A mixed method approach using a retrospective audit of existing hospital databases, deceased patients’ medical records, and in-depth interviews with clinicians.

Despite the potentially devastating effects of a death on the lives of adolescents, little is known about their help-seeking experiences. We interviewed by telephone 39 bereaved adolescents on their help-seeking experiences. Thematic analysis resulted in three themes: Formal support, Informal support and School-related support. Participants provided a critical appraisal of positive and negative experiences, and noted barriers and facilitators for help-seeking. As adolescents bereaved through suicide may receive less social support, professional help is a much needed auxiliary. Parental encouragement is important in accessing adequate professional help.

Objective: To understand the clinical and psychosocial journey of culturally and linguistically diverse (CALD) palliative care patients.

Method: This study was conducted at a subacute hospital with a specialist palliative care unit and a community palliative care service in a metropolitan region of New South Wales, Australia. Medical records of 100 deceased patients from CALD backgrounds over a 12-month period from 2014 to 2015 were recorded on a data mining tool. The cohort had transitioned to either community or inpatient palliative care services with a life-limiting illness. This study aimed to establish whether Pastoral Care (PC) visits were an effective component of a hospitalized patient’s overall health experience. Outcomes of PC visits were reported by 369 patients in 7 sites across Australia. The patient reported outcomes of PC visits included: the patients felt they could be honest with themselves, with a sense of peace, a better perspective of their illness, less anxiety, and felt more in control. Five factors of the PC visit significantly related to higher patient’s overall outcomes: (a) having more Pastoral Care visits (p < 0.5 0 R 0.778, CI 0.17–1.38); (b) the patient was able to talk about what was on their mind (p < 0.01, OR1.48, CI 0.58–2.37); (c) they had something to be hopeful about (p < 0.01 OR1.18, CI 0.51–1.85); 4); the visit focused on decisions about the patient’s health care (p < 0.05, OR0.70, CI 0.05–1.35); and (d) a belief in God/Higher Being (p < 0.01, OR 1.01, CI 0.43–1.71).

PROBLEM:

Preparing health professional students for interprofessional collaborative practice, especially at a distance where provider shortages prevail remains difficult.

APPROACH:

A two-week interprofessional education (IPE) immersion experience preparing students from 11 disciplines and four universities was implemented. Week-one, using online technology, students develop/present an interprofessional careplan for a complex patient. Students then meet face-to-face to conduct group interviews with two standardized patient dyads. Week-two, students develop a website for use of the patient dyads. Websites are presented to faculty and fellow students via an online virtual meeting space.

From 1998-2018, over 6,000 migrants have been found dead after attempting to cross into the United States through its southern border; most of the deaths are due to harsh environmental conditions found through the crossing areas. Migrant remains are often found with no belongings or evidence to use to identify the deceased. The purpose of this research was to conduct a systematic investigation to determine whether the use of client feedback measures enhance the mental health treatment of youth in private practice settings. Participants were randomly assigned to one of two groups, treatment as usual or the Client Feedback condition. Clients who were assigned to the Feedback condition completed the feedback measures routinely at each session. The Pediatric Symptom Checklist (PSC) and/or Pediatric Symptom Checklist – Youth Version (PSC-Y) were completed at intake and at each 3-month progress review summary, and/or closure per normal office standards. Mobile technologies (e.g., smartphones and associated social media accessed through downloadable mobile applications) have changed the social landscape of adolescent communication. Intertwined with this communication are risks, many of which are sexual. Evidence suggests online risks associated with mobile technology use can be mitigated by (a) parental awareness of online activities; (b) protective practices regarding content and contacts; and (c) mediation aimed at regulating access and use.
