International Review of Business and Economics
India as a developing economy is healthier nation because of proper and well-developed healthcare infrastructure. This leads to improvement in the life years and happiness of the population. The high life style increases the unhealthy practices of eating culture, fast food, no exercise and other. Even children’s are facing serious health issues in early ages. Due to this, every individual’s visiting the hospitals at least once in a month. Many get diseases while visiting hospitals where there is poor maintenance of various wastes, which leads to air pollution, waste water pollution, bio medical waste pollution, and soil pollution. Mycobacterium tuberculosis continues to cause devastating levels of mortality due to tuberculosis (TB). The failure to control TB stems from an incomplete understanding of the highly specialized strategies that M. tuberculosis utilizes to modulate host immunity and thereby persist in host lungs. Here, we show that M. tuberculosis induced the expression of indoleamine 2,3-dioxygenase (IDO), an enzyme involved in tryptophan catabolism, in macrophages and in the lungs of animals (mice and macaque) with active disease. In a macaque model of inhalation TB, suppression of IDO activity reduced bacterial burden, pathology, and clinical signs of TB disease, leading to increased host …

Parasitic nematodes infect over one quarter of the population worldwide, causing morbidity in over one billion people. Current anthelmintic drugs are beginning to lose effectiveness due to the presence of resistant strains. We are interested in the role of neuropeptides, which regulate behaviors in all organisms, as another possible target for anthelmintic drugs. FMRFamide-related peptides (FaRPs) are a family of neuropeptides that are conserved throughout the animal kingdom. In particular, nematodes contain the largest family of FaRPs identified thus far and many of these FaRPs are identical among different nematode species; FaRPs in nematodes are collectively referred to as FLPs …

Objective. HIV and tuberculosis represent diseases of major public health importance worldwide. Very little is known about HIV-TB coinfection among pregnant women, especially from industrialized settings. In this study, we examined the association between TB, HIV, and HIV-TB coinfection among pregnant mothers and obstetric complications, alcohol use, drug abuse, and depression. Method. We examined inpatient hospital discharges in the United States from January 1, 2002, through December 31, 2014. We employed multivariable survey logistic regression to generate adjusted estimates for the association between infection status and study outcomes. Results. We analyzed approximately 57 million records of pregnant women and their …

Children and adolescents have the tendency to develop osteomyelitis in metaphyseal sinusoids because turbulent blood flow increases the probability of bacterial growth. Osteomyelitis in children is frequently misdiagnosed owing to lack of symptoms and specific findings from laboratory tests. Without rapid antibiotic treatment of osteomyelitis, complications such as subperiosteal abscesses may occur. We describe a 13-year-old boy who underwent irrigation and debridement to treat a subperiosteal abscess of the right distal radius and ulna, caused by acute osteomyelitis. After debridement, the patient was placed on a continuous 6-week course of nafcillin infusion through a peripherally inserted central catheter (PICC). The …

The prevalence of antibiotic resistance has grown steadily over the past few decades. Hundreds of thousands of people are dying each year due to the unavailability of previously effective antibiotic regimens. A major influence in the continued progression of this resistance can be attributed to the overuse and inappropriate prescribing of antibiotics. This study explored the factors that influence medical providers to use or prescribe antibiotics in situations when they are not indicated. The study was conducted through a semi-structured interview to providers from Minnesota and Wisconsin who work or have previously worked in urgent cares or emergency rooms. Providers …

Attitudes Towards Vaccinations In The Minnesota Slavic Community, Maksim Kozak, Hannah Margaret Roy, Rachel Van Heest
All Electronic Theses and Dissertations
The incidence of vaccine preventable illnesses is increasing in the United States. Negative attitudes towards vaccinations, or vaccine hesitancy is fueling the decreasing vaccination rates. Ascertaining the level of vaccine hesitancy in susceptible populations is essential in combatting the rise of vaccine preventable illnesses in the United States. Very little research has previously been compiled on vaccine attitudes in Slavic immigrants. Slavic immigrants identify racially as “white” on surveys, masking differences in the Slavic population from the much larger Caucasian population in the United States. In order to better understand the level of vaccine hesitancy in the Minnesota Slavic immigrant …

Clostridium difficile (C. difficile) is a life threatening bacteria spread in spore form through diarrhea. C. difficile effects approximately 500,000 people annually (CDC, 2016). From 2009-2016, the incidence rate of C. difficile in Minnesota* has increased from 50 to 227/1000,000 persons, with 58% of cases considered community associated** (CA), 17% healthcare facility onset (HCFO), and 25% community onset-HCFO (MDH, 2016). MDH 2016 data indicates C. difficile is being transmitted within healthcare and community settings and among patients primarily greater than 65 years of age, with 54% of CA cases using antibiotics during the 12 weeks prior to positive specimen (2016).

The prevalence of edentulism is common worldwide. While improvements in access to healthcare and dental care are reducing the prevalence rate of edentulism, the rapidly growing number of elderly as a percent of the global population will sustain a need for denture therapy for the foreseeable future. While denture use has positive impacts on the quality of life, their use is associated with some problems and risks. Denture stomatitis, a chronic infection-related inflammatory disorder of the oral mucosa, is extremely common and has been reported to occur in up to two-thirds of denture wearers. Importantly, epidemiology studies have shown edentulism …

Honduras is a country with great healthcare needs. Within health care, the largest percentage of mortality is due to conditions occurring in the perinatal period, including prenatal and maternal health, delivery and postnatal care (PAHO, 2017a). Additionally, malnutrition continues to affect infants as they growth throughout their childhood (Gray, Cossman, & Powers, 2006). Education is shown to produce positive outcomes in low-income populations (Perkins, et al, 2011). The purpose of this project was to identify Honduran healthcare needs and provide effective and sustainable intervention. A needs assessment with a Honduran-Canadian health organization reiterated evidence found in the literature, illustrating the …

Cystic fibrosis (CF) is an autosomal recessive disease associated with recurrent lung infections that can lead to morbidity and mortality. The impact of antibiotics for treatment of acute pulmonary exacerbations on the CF airway microbiome remains unclear with prior studies giving conflicting results and being limited by their use of 16S ribosomal RNA sequencing. Our primary objective was to validate the use of true single molecular sequencing (tSMS) and PathoScope in the analysis of the CF airway microbiome. Three control samples were created with differing amounts of Burkholderia cepacia, Pseudomonas aeruginosa, and Prevotella melaninogenica, three common bacteria found in cystic …

Assessment And Optimization Of Respiratory Syncytial Virus Prophylaxis Regimens In Connecticut, 1995-2013, Ben Artin
Yale School of Medicine Physician Associate Program Theses
BACKGROUND - Respiratory syncytial virus (RSV) causes seasonal respiratory infection with potentially serious complications in children, and leads to hospitalization rates as high as 50% in high-risk infants. Safe and effective immunoprophylaxis (palivizumab) is available, but costly. The American Academy of Pediatrics (AAP) recommends palivizumab only for infants at high risk of complications, and only during the RSV season. Although the current AAP guidelines acknowledge the existence of spatial and temporal variation in RSV incidence, they do not recommend spatial or temporal adjustments to immunoprophylaxis regimen outside of Florida. In this study, we investigate the value of using spatial and …

Power Calculation For Cross-Sectional Stepped Wedge Cluster-Randomized Trials With Variable Cluster Sizes, Linda J. Harrison, Tom Chen, Rui Wang
Harvard University Biostatistics Working Paper Series
Standard sample size calculation formulas for Stepped Wedge Cluster Randomized Trials (SW-CRTs) assume that cluster sizes are equal. When cluster sizes vary substantially, ignoring this variation may lead to an under-powered study. We investigate the relative efficiency of a SW-CRT with varying cluster sizes to equal cluster sizes, and derive variance estimators for the intervention effect that account for this variation under the assumption of a mixed effects model; a commonly-used approach for analyzing data from cluster randomized trials. When cluster sizes vary, the power of a SW-CRT depends on the order in which clusters receive the intervention, which is …

Altered response to the intestinal microbiota strongly associates with inflammatory bowel disease (IBD); however, how commensal microbial cues are integrated by the host during the pathogenesis of IBD is not understood. Epigenetics represents a potential mechanism that could enable intestinal microbes to modulate transcriptional output during the development of IBD. Here, we reveal a histone methylation signature of intestinal epithelial cells isolated from the terminal ilea of newly diagnosed pediatric IBD patients. Genes characterized by significant alterations in histone H3-lysine 4 trimethylation (H3K4me3) showed differential enrichment in pathways involving immunoregulation, cell survival and signaling, and metabolism. Interestingly, a large subset …
