The world has now entered the third decade of the AIDS epidemic. Men-who-have-sex-with-men continue to be disproportionately affected by HIV/AIDS. The United States still struggles in its response to this ongoing crisis in many areas: disease prevention, treatment, and HIV related stigma, prejudice and discrimination. Much of the information reported on MSM living with HIV has come from urban population centers, but only a few studies have focused on HIV positive MSM living in rural areas. Human C-reactive protein (CRP) increases survival of and decreases bacteremia in mice infected with Streptococcus pneumoniae. Such protection of mice against pneumococcal infection is seen only when CRP is administered into mice 6 hours before to 2 hours after the injection of pneumococci, but not when CRP is given to mice at a later time. Our first aim was to define the mechanism of CRP-mediated initial protection of mice against infection. It was proposed that CRP binds to phosphocholine (PCh) moieties present in the cell wall and activates the complement system on the pneumococcal surface that kills the pathogen. …

A four-year longitudinal cohort and geographic cluster study in rural Thailand was conducted to characterize the clinical spectrum of dengue virus (DENV) infection. Symptomatic DENV infections in the cohort were detected by active school absence-based surveillance that triggered cluster investigations around ill cohort children. Data from 189 cohort children with symptomatic DENV infection and 126 contact children in the clusters with DENV infection were analyzed. Of infected contacts, only 19% were asymptomatic; 81% were symptomatic, but only 65.9% reported fever. Symptom-based case definitions were unreliable for diagnosis. Background: The emergence of carbapenem-resistant Acinetobacter baumannii (CRAB) was observed in a Kenyan hospital from 2009 to 2010. Further investigation of the dissemination of CRAB isolates and the molecular characterization of associated resistance determinants were therefore performed.

Methods: Antibiotic susceptibilities were determined by broth microdilution and Etest. Metallo-blactamases were detected by Etest method. Clonal relationships were studied by pulsed-field gel electrophoresis (PFGE) and multilocus sequence typing (MLST). b-Lactam and aminoglycoside resistance determinants and the clonal relatedness to widespread European clones were studied by PCR and sequencing.

BACKGROUND:

The incidence of HIV among persons who inject drugs (PWIDU) has fallen in many nations, likely due to successes of clean needle/syringe exchange and substance abuse treatment and service programs. However in Pakistan, prevalence rates for PWID have risen dramatically. In several cities, prevalence exceeded 20% by 2009 compared to a 2003 baseline of just 0.5%. However, no cohort study of PWID has ever been conducted.

METHODS:

The paleopathological record of neoplastic conditions in the past is considered scarce. The detection of tumours in ancient populations is hindered by the quality and quantity of signs visible on the skeleton, the methodological approach, the preservation of remains, and by difficulties of differential diagnosis. The aims of this paper are to report the extensive and multiple osteolytic lesions observed in an adult male and to discuss the possible etiology of these lesions. The individual, a 71-year-old male who died in 1932, is part of the Coimbra Identified Skeletal Collection. Records indicate that he died of a ‘heart lesion’. The …

Background: Cytokine-mediated endothelial activation pathway is a known mechanism of pathogenesis employed by Plasmodium falciparum to induce severe disease symptoms in human host. Though considered benign, complicated cases of Plasmodium vivax are being reported worldwide and from Pakistan. It has been hypothesized that P.vivax utilizes similar mechanism of pathogenesis, as that of P.falciparum for manifestations of severe malaria. This study investigated the relationship between Anopheles abundance, collection sites and environmental variables in the Lower Rio Grande Valley, South Texas. In addition, species composition in the Lower Rio Grande Valley was determined in 2011. A total of 6772 female mosquitoes were identified to six genera and 27 species. The most prevalent genera collected were Culex (53.9%), Ochlerotatus (25.6%) and Aedes (13.6%). Anopheles mosquitoes were collected using resting boxes during summer 2012 at multiple locations along the Lower Rio Grande Valley. The safety of fresh produce is often challenged by the sporadic incidences of outbreaks owing to Salmonella Typhimurium (S.T.) and Escherichia coli O157:H7 (E.c.). Washing and current disinfection strategies do not completely decontaminate leafy greens such as spinach and iceberg lettuce. In this Article I consider whether the existing international legal human rights regime enjoys political authority over sovereign states. In particular, I explore whether, just as states can cite their role as the primary institutions that protect human rights in order to justify their claim to authority over their citizens, perhaps the current human rights regime might plausibly cite its secondary role in securing human rights in order to ground its authority over these states.

Background: Rapid growth in global health activity among US medical specialty education programs has lead to heterogeneity in types of activities and global health training models. The breadth and scope of this activity is not well chronicled.

Methods: Using a standardized search protocol, we examined the characteristics of US medical residency global health programs by number of programs, clinical specialty, nature of activity (elective, research, extended curriculum based field training), and geographic location across seven different clinical medical residency education specialties. Penarik becak dayung dan becak bermesin, pengatur lalu lintas, pedagang asongan, dan pedagang kaki lima banyak terpapar dengan polusi timbal dari udara ambien yang merupakan ancaman terhadap para pekerja pinggir jalan. Penelitian ini bertujuan mengetahui hubungan karakteristik responden dengan kadar timbal dalam darah. Sampel dalam penelitian ini berjumlah 109 orang terdiri dari 58 orang penarik becak dayung, 30 orang penarik becak bermesin dan 21 orang pedagang kaki lima yang ditarik secara consecutive sampling. Pengendalian tuberkulosis telah meningkatkan angka kesembuhan dan menyelamatkan banyak jiwa, tetapi kurang berhasil menurunkan insiden tuberkulosis. Oleh karena itu, pengendalian tuberkulosis menekankan pada kebijakan determinan sosial karena determinan sosial secara langsung dan melalui faktor risiko tuberkulosis berpengaruh terhadap tuberkulosis. Hasil telaah literatur menunjukkan bahwa stratifikasi determinan sosial menyebabkan clustering tuberkulosis, berupa pengelompokkan penderita tuberkulosis menurut lokasi geografis yang secara statistik signifikan. Perkembangan perokok di kalangan anak-anak dan remaja semakin meningkat, baik secara kuantitas maupun kualitas. Data Global Youth Tobacco Survey terakhir di tahun 2009, menunjukkan 20,3% anak sekolah 13 - 15 tahun merokok. Perokok pemula usia 10 - 14 tahun naik 2 kali lipat dalam 10 tahun terakhir dari 9,5% pada tahun 2001 menjadi 17,5% pada tahun 2010. Angka perokok pada usia remaja yang tinggi meningkatkan risiko penyakit. Berdasarkan penelitian, para perokok yang terus merokok dalam jangka panjang memiliki risiko kematian tiga kali lebih tinggi daripada mereka yang bukan perokok. Masih banyak ditemukan resep obat antituberkulosis anak dengan kombinasi beberapa obat dalam racikan puyer yang tidak sesuai standar program pemberantasan tuberkulosis (TB) paru Kementerian Kesehatan Republik Indonesia. Studi ini bertujuan untuk mengetahui situasi dan permasalahan berhubungan praktik peresepan puyer sebagai obat anti tuberkulosis (OAT). Pada periode Mei hingga Desember tahun 2009, penelitian diawali dengan pengukuran persentase peracikan OAT dalam bentuk puyer, dilanjutkan dengan penelitian kualitatif eksploratif. Data dikumpulkan dari rumah sakit, puskesmas, apotek dan dinas kesehatan di Jakarta, Bandung, Medan, dan Makassar. Tikus liar dan domestikasi mempunyai potensi faktor risiko bagi kesehatan masyarakat. Mereka dapat membawa mikroorganisme yang dapat ditransmisi melalui kontak dengan urine maupun feses atau melalui ektoparasit yang ada di tubuh mereka. Pada penelitian ini, diamati prevalensi mikroorganisme zoonotik pada tikus yang tertangkap di Pasar Kota Kabupaten Banjarnegara. Bakteri Salmonella paratyphi B dan Salmonella paratyphi C menunjukkan prevalensi yang paling tinggi (masing-masing 20%). Spesies bakteri lain yang ditemukan antara lain Salmonella typhimurium, Citrotobacter, Citrotobacter frendii, Enterotobacter cloacae, Escherichia coli (E. coli) dan Proteus miriabilis. Sampai dengan tahun 2013 dilaporkan ada 13 kasus leptospirosis di Kabupaten Banyumas.Terjadinya peningkatan kasus selama dua tahun terakhir perlu mendapat perhatian dari berbagai pihak agar kasus leptospirosis bisa segera ditangani. Tujuan penelitian ini adalah memetakan kasus leptospirosis dan menganalisis faktor risiko lingkungan dan perilaku yang memengaruhi leptospirosis. Penelitian ini menggunakan desain studi observasional kasus kontrol. Pengumpulan data dilakukan dengan wawancara, observasi, dan pengukuran. Analisis data menggunakan analisis spasial, analisis univariat, dan bivariat. Riwayat pemberian Air Susu Ibu (ASI), karakteristik ibu dan anak serta pola asuh berpengaruh terhadap kecerdasan anak. Penelitian ini bertujuan untuk mengetahui proporsi tingkat kecerdasan anak, riwayat pemberian ASI, karakteristik ibu dan anak, serta faktor dominan yang berhubungan dengan tingkat kecerdasan anak pada siswa SDSN Pekayon Jaya VI Kota Bekasi. Penelitian kuantitatif dengan desain potong lintang dengan metode systematic random sampling dengan jumlah sampel sebesar 166 responden (siswa/i yang berumur 7 - 9 tahun) beserta ibunya. Background: A rigorous comparison of cervical cancer screening methods utilizing data on immune status, antiretroviral therapy (ART) and colposcopy-directed biopsy has not been performed among HIV-positive women.

Methods: Between June and November 2009, 500 HIV-positive women were enrolled at an HIV treatment clinic in Nairobi, Kenya, and underwent Papanicolau (Pap) smear, visual inspection with acetic acid (VIA), human papillomavirus (HPV) and colposcopydirected biopsy (gold standard). Positive Pap smear (ASCUS, LSIL, HSIL), VIA, HPV and their combinations were compared with CIN2/3+. Human pygmy populations inhabit different regions of the world, from Africa to Melanesia. In Asia, short-statured populations are often referred to as "negritos." Their short stature has been interpreted as a consequence of thermoregulatory, nutritional, and/or locomotory adaptations to life in tropical forests. A more recent hypothesis proposes that their stature is the outcome of a life history trade-off in high-mortality environments, where early reproduction is favored and, consequently, early sexual maturation and early growth cessation have coevolved. Early growth cessation and reproduction are predicted to maximize fitness under conditions of high adult mortality, factors that could explain the pygmy phenotype of many rainforest hunter-gatherers. This life-history hypothesis is elegant but contentious in part because it lacks a clear biological mechanism. One mechanism stems from the field of human immunological ecology and the concept of inflammation "memory" across the life cycle and into subsequent generations. The "negrito" hypothesis predicts that a shared phenotype among various contemporary groups of hunter-gatherers in Southeast Asia - dark skin, short stature, tight curly hair - is due to common descent from a region-wide, pre-Neolithic substrate of humanity. The alternative is that their distinctive phenotype results from convergent evolution. The core issues of the negrito hypothesis are today more relevant than ever to studies of human evolution, including the out-of-Africa migration, admixture with Denisovans, and the effects of environment and ecology on life-history traits. Background While several studies have documented the importance of hand washing in the university setting, the added role of environmental hygiene remains poorly understood. The purpose of this study was to characterize the personal and environmental hygiene habits of college students, define the determinants of hygiene in this population, and assess the relationship between reported hygiene behaviors, environmental contamination, and health status. Methods 501 undergraduate students completed a previously validated survey assessing baseline demographics, hygiene habits, determinants of hygiene, and health status. Background: In HIV-infected children, viral diversity tends to increase with age in the absence of antiretroviral treatment (ART). We measured HIV diversity in African children (ages 6–36 months) enrolled in a randomized clinical trial comparing two ART regimens (Cohort I of the P1060 trial). Children in this cohort were exposed to single dose nevirapine (sdNVP) at birth. Methods: HIV diversity was measured retrospectively using a high resolution melting (HRM) diversity assay.
