Coordinating Care For People With Multiple Chronic Conditions: The Guided Care Model And Other Innovative Approaches, Judith D. Moore, Carol O'Shaughnessy

National Health Policy Forum

The Forum sponsored two site visits to explore the Johns Hopkins University's research and demonstration program on Guided Care, in April and August 2009. Guided Care is a patient-centered medical home model that uses an interdisciplinary approach to coordinating health care for older people with multiple chronic conditions. Guided Care nurses located in primary care practices work with Medicare patients on a long-term basis, coordinate their care among various providers, provide transitional care, and assist patients with self-management skills. Site visit participants learned about the complexities of caring for frail Medicare patients, some of whom see multiple physicians and other …

Ceramide Metabolism Regulates A Neuronal Nadph Oxidase Influencing Neuron Survival During Inflammation, Brian M. Barth

Chemistry and Biochemistry

Inflammation is a major component of acute and chronic pathologies of the central nervous system, including psychiatric disorders. Microglia respond to pathogens, injury, and toxins by secreting inflammatory mediators including pro-inflammatory cytokines in an event known as neuroinflammation. This thesis research investigated a link between neuroinflammation and oxidative stress, and ultimately neurodegeneration. The cytokine tumor necrosis factor alpha was shown to stimulate a neuronal NADPH oxidase (NOX), specifically by stimulating the production of ceramide and ceramide-1-phosphate via Mg 2+-neutral sphingomyelinase (Mg2+-nSMase) and ceramide kinase. Intriguingly, glucosylceramide blocked NOX activation, linking ceramide neutralization directly to a decline in oxidative stress. Most …

Arvicoline rodents (voles and lemmings) inhabit high-latitude environments and undergo pronounced seasonal changes in their physiology and behavior. They are an important prey resource in circumpolar regions, and their population numbers can affect the survival and reproductive fitness of many predator and secondary prey species. I studied the effects of seasonality and environmental factors on reproduction and energy allocation in the northern red-backed vole (Myodes rutilus), an arvicoline rodent in Alaska known to have bred in winter. Marine mammals are dietary staples among many indigenous peoples of the Arctic, but these foods sometimes contain high levels of mercury, a toxic heavy metal that can cause nerve and brain damage. Because mercury can be released into the environment by both industrial and natural processes, prehistoric marine mammal consumers may have been exposed to this toxicant, but little is known about preindustrial mercury levels. This research examined the potential for using the mercury concentration of archaeological bone as a biomarker of mercury exposure. In the search for solutions to environmental and human health problems linked to the dominant global food system, too much attention has been given to the scale of food systems and too little attention given to the specific practices and outcomes of various food system components and initiatives. The community of Fairbanks, Alaska is used to examine whether local-food system initiatives can improve an urban food system's social equity and environmental sustainability. Existing populations of Steller sea lions (Eumetopias jubatus ) have declined precipitously over the last half-century. Investigations into the cause of this downward trend have focused on many different possible factors. Toxicity caused by the accumulation of organochlorines (OCs), such as polychlorinated biphenyls (PCBs) and 1,1,1-trichloro-2,2-bis(p-chlorophenyl)ethane or dichlorodiphenyltrichloroethane (DDT), has been demonstrated in marine mammals and was considered here as one possible factor that may have contributed to the decline of Steller sea lions or their failure to recover. This thesis describes investigations on bowhead whales (Balaena mysticetus) conducted over the past three decades, specifically on age, growth, morphology and energetics. The examined whales were harvested by Inupiat Eskimo whale hunters primarily in Barrow, Alaska. Bowheads are robust cetaceans reaching 19 m in length and inhabit the ice-covered waters of the circumpolar Arctic. They have the thickest blubber (? 35 cm) of any cetacean and the longest (>4 m) and most extensive baleen apparatus. Bowheads are ~4 m at birth and grow rapidly to ~8 m in their first year; they then experience a 2-3 year growth pause …

I examined the interactions of key components of a hunting system of Sitka black-tailed deer (Odocoileus hemionus sitkensis) on Prince of Wales Island, Alaska to address concerns of subsistence hunters and to provide a new tool to more effectively monitor deer populations. To address hunter concerns, I documented local knowledge and perceptions of changes in harvest opportunities of deer over the last 50 years as a result of landscape change (e.g., logging, roads). To improve deer monitoring, I designed an efficient method to sample and survey deer pellets, tested the feasibility of identifying individual deer from fecal DNA, and used …

Mammalian predation, avian predation, female body condition and food availability on the breeding ground are likely the main factors influencing nesting success in tundra-nesting waterfowl. These driving factors are mediated by the primary life history characteristics; incubation behavior, female body size, nesting associations, and nest site selection. I created a conceptual model illustrating how these factors are inter-related and how they impact nest success through a variety of pathways to better understand the evolution of a species' nesting strategy and patterns observed in the field. When adult salmon return to their natal streams to spawn they deliver energy in the form of carcass tissues and eggs. Currently, the effect of this marine-derived energy on the growth and energy allocation strategies of juvenile salmonids is unknown. This thesis examined the effects of marine-derived energy on the growth and energy allocation strategies of juvenile coho salmon and resident Dolly Varden. Fatty acid analysis was developed as a tool for monitoring the flow of marine-derived lipids and hence energy from carcass tissues to consumers in laboratory and field settings. Power Training: Can It Improve Functional Performance In Older Adults? A Systematic Review, John Rice†, Justin Keogh‡

Older adults’ reduced performance in functional activities of daily living (ADL) such as gait, sit to stand or stair climbing may reflect age-related declines in muscular power more so than strength. Therefore, this review was conducted to determine if power training is effective in improving the functional ability of older adults, and if so, if it was more effective than strength training. The review was performed using Medline (PubMed), CINAHL, Sports Discus, ProQuest 5000 International and Google Scholar with the keywords “power training”, “older adult” and “elder” and all derivatives.
