OBJECTIVE: The objective of this study was to evaluate the impact of newer therapies on the highest risk patients with congenital diaphragmatic hernia (CDH), those with agenesis of the diaphragm.

SUMMARY BACKGROUND DATA: CDH remains a significant cause of neonatal mortality. Many novel therapeutic interventions have been used in these infants. Those children with large defects or agenesis of the diaphragm have the highest mortality and morbidity.

METHODS: Twenty centers from 5 countries collected data prospectively on all liveborn infants with CDH over a 10-year period. The treatment and outcomes in these patients were examined. Patients were followed until death …

Fragmenting Work And Fragmenting Organizations: The Contract Of Employment And The Scope Of Labour Regulation, Judy Fudge

Osgoode Hall Law Journal

This article diagnoses the conceptual and normative crisis of the scope of labour protection as resulting from the conception of employment as a personal and bilateral contract between an employee and a unitary employer that is characterized by the employee's subordination. It argues that the related fragmentation of organizations and fragmentation of work reveals the extent of the problem with this legal conceptualization of employment. The article offers an approach to reconceptualizing the scope of labour protection that is based on an understanding of personal work arrangements and enterprises as activities. Institutional Oversight Of Clinical Trials And The Drug Approval Process, Paul B. Miller

The institutional and federal bodies responsible for regulatory review and oversight of clinical trials in Canada serve distinct yet complementary functions in ensuring that clinical trials provide scientifically rigorous and ethically sound evaluation of new therapeutic products. To date, academics and reformers alike have discussed reform priorities for federal and institutional review in isolation, as if their guiding purposes are distinct. This article identifies the overlapping objectives of federal and institutional review, argues for the importance of coordination of institutional and federal oversight structures, and identifies potential points of coordination.

BACKGROUND: Renal involvement is a serious manifestation of systemic lupus erythematosus (SLE); it may portend a poor prognosis as it may lead to end-stage renal disease (ESRD). The purpose of this study was to determine the factors predicting the development of renal involvement and its progression to ESRD in a multi-ethnic SLE cohort (PROFILE).

METHODS AND FINDINGS: PROFILE includes SLE patients from five different United States institutions. We examined at baseline the socioeconomic-demographic, clinical, and genetic variables associated with the development of renal involvement and its progression to ESRD by univariable and multivariable Cox proportional hazards regression analyses. In recent years, health practitioner educators and researchers have devoted an increasing amount of attention to the improvement of communication in the clinical setting. One aspect of communication--organizational communication--occupies relatively little space in practitioner education and research. Yet, contemporary clinical practice relies heavily upon organizational structures to facilitate the coordination of care. Understandably, overcrowded practitioner curricula may not accommodate the addition of explicit pedagogical material related to organizational communication. Purpose – Crisis planning, which symbolizes an organization's crisis preparedness and often conceptualized at the corporate headquarters, is increasingly decentralized to regional centers of global companies. These centers, in turn, synchronize their crisis master plans with its national units for expeditious management of localized crises. Significant investments will be required to maintain, operate, upgrade, and expand California's transportation infrastructure if the state is to retain its economic position in the global economy and accommodate a projected near doubling of the current population by 2040. At the same time, available funding for transportation will decline significantly over the next 15 years if the current transportation finance system remains unchanged. This report analyzes a range of alternative sources of revenue, as well as different finance options. Hypertension is a leading cause of stroke, coronary artery disease, heart attack, and heart and kidney failure in the United States. In Georgia, the percentage of those with hypertension and related diseases remain above the national average. The aim of this paper is to offer a basic review of hypertension including physical complications of the disease and to provide statistics regarding the scope of hypertension in the state of Georgia. Recent neuroimaging research shows that older adults exhibit recruitment, or increased activation on various cognitive tasks. The current study evaluated whether a similar pattern also occurs in semantic memory by evaluating age-related differences during recognition of Recent (since the 1990s) and Enduring (1950s to present) famous names. Fifteen healthy older and 15 healthy younger adults performed the name recognition task with a high and comparable degree of accuracy, although older adults had slower reaction time in response to Recent famous names. In recent years, many academic institutions have implemented more stringent academic qualification standards by increasing research requirements for faculty. This paper analyzes the impact of changing research requirements in terms of faculty research output. We model the academic research pipeline including review and revision processes as a queue network and study the stationary behavior of the research pipeline. Both a theoretical model of the publication process and a simulation showing numerous combinations of submission strategies and publication requirements are presented. Problem — Various demographic and familial risk factors have been linked to runaway behavior. To date, there has not been a systematic investigation of the impact of size of community on runaway behavior. This study will compare runaways from smaller cities and rural areas to their urban counterparts.

Methods — A convenience sample of 602 adolescents was interviewed between 1995 and August of 1996 in Missouri, Iowa, Nebraska, and Kansas, USA. While the number of job openings within the academic ranks of sport management continues to prosper, the number of women filling these positions remains low. Grappendorf and Lough (2003) reported that women represent only 25 % of professors who hold membership in NASSM, and within doctoral sport management programs, Jisha and Pitts (2004) reported that most students are Caucasian males in their early thirties. Likewise, Morin and Grappendorf (2004) found doctoral sport management students to be 65 % men and 62 % Caucasian. A large amount of research has been done in the area of wireless sensor networks (WSN), but not much work has been done in modeling the fault tolerance and reliability of these networks. In this thesis, the fault tolerance of a WSN to node failures is studied and an analytical reliability model of the network is derived. A valid reliability model of a network could reveal an estimate of the network's performance before it is deployed.

Nanosecond pulsed electric fields are able to manipulate internal cell organelles by taking advantage of the small but finite time required for charge to establish across a capacitive barrier. Pulsed electric fields also affect macromolecular structures such as enzymes and proteins. Application of a single nanosecond pulsed electric field decreases the fluorescence of a commercially available biological dye Bodipy FL C5 Ceramide, used to observe the Golgi apparatus of living cells. The effect is seen when the fluorophore is exposed to an electric field both in vitro and when incorporated into the Golgi of a living Jurkat cell. Observation …

Vibrio cholerae serotypes 01 and 0139 are responsible for world-wide epidemics of cholera. These pandemic causing strains must possess genes that encode for the cholera toxin (CTX) and toxin co-regulated pili (TCP) in order to infect their hosts. In this study, 284 isolates of Vibrio cholerae from ballast and coastal waters were serotyped, with 11 % testing positive for serotype 01 and 21 % testing positive for serotype 0139. PCR assays were used to detect the presence of ctxA and tcpA genes in all positive isolates, none of which contained ctxA while 2% of the isolates contained tcpA. The 01 …

Challenge courses have become increasingly popular in recent years. Benefits examined and researched through the use of challenge courses include increased self-esteem, group cohesion, leadership skills, and general self-efficacy (Hart & Silka, 1994; Hatch & McCarthy, 2005; Moseley, Reinke, & Bookout, 2002; Paxton, 2000; Propst & Koesler, 1998). The purpose of this study was to explore the effects of vital tooth whitening on tooth color satisfaction, beliefs about dentofacial appearance, and self-esteem in a population 50 years of age and older. A two-group, randomized, pre-test, post-test, single-blind design was utilized following !RB-approval. Sixty-two participants were enrolled. Fifty-three participants (N=53) completed the study with no adverse events reported. Both the control and experimental groups received instructions for a threeweek oral self-care regimen, a toothbrush, toothpaste, and floss. The experimental group also received a tooth whitening product (independent variable) to be used during the same three-week period. Dependent variable measures …
