Mutations and/or overexpression of various transporters are known to confer drug resistance in a variety of organisms. In the malaria parasite Plasmodium falciparum, a homologue of P-glycoprotein, PfMDR1, has been implicated in responses to chloroquine (CO), quinine (ON) and other drugs, and a putative transporter, PfCRT, was recently demonstrated to be the key molecule in CO resistance. However, other unknown molecules are probably involved, as different parasite clones carrying the same pfcrt and pfmdr1 alleles show a wide range of quantitative responses to CO and ON. This pilot project, conducted under the auspices of the Forced Migration and Refugees Studies Program (FMRS) at the American University in Cairo, arose as part of a continued commitment to outreach projects of direct benefit to the refugee community. Maintaining healthy behaviors and accessing adequate health care are particular challenges for refugees living in large urban areas such as Cairo. Young refugees are at risk for unplanned pregnancy, sexually transmitted diseases, and other health-related problems such as nutritional deficits and stress-related disorders. Campylobacter jejuni, a leading cause of bacterial gastroenteritis, has different age distribution and disease expression in developing and developed countries, which may be due to the endemnicity of infection and the age of acquisition of immunity. Differences in disease expression are not solely dependent on the C jejuni strain or virulence attributes. Another modulating factor in developing countries may be endemic nematode infections such as Trichuris, which drive type 2 cytokine responses and down-regulate type I immune responses. Some Loans Are More Equal Than Others: Third-Party Originations And Defaults In The Subprime Mortgage Industry, William P. Alexander, Scott D. Grimshaw, Grant R. Mcqueen, Barrett A. Slade

We show how agency problems between lenders (principals) and third-party originators (TPO; agents) imply that TPO-originated loans are more likely to default than similar retail-originated loans. The nature of the agency problem is that TPOs are compensated for writing loans, but are not completely held accountable for the subsequent performance of those loans. Using a hazard model with jointly estimated competing risks and unobserved heterogeneity, we find empirical support for the TPO/default prediction using individual fixedrate subprime loans with first liens secured by residential real estate originated between January 1, 1996, and December 31, 1998. The article critically examines the Andrea Yates trial, arguing that it highlights significant flaws in the legal system, media coverage, and societal attitudes toward mental illness and gender roles. The trial underscores the ambiguity in legal definitions of insanity, the unequal treatment of mothers who commit infanticide, and the problematic intersection of law and media, which can undermine justice and perpetuate harmful stereotypes. The case serves as a cautionary tale about the failure of the legal system to address mental health adequately and the media's role in sensationalizing tragedy for public consumption.

Agricultural sustainability is enhanced by management practices that optimize the performance of multiple agroecosystem functions. The performance of western Corn Belt cropping systems was evaluated based on four agroecosystem functions: food production, raw materials production, nutrient cycling, and greenhouse gas regulation. A simple multiattribute ranking procedure was used to quantify agroecosystem performance using data from a long-term cropping systems experiment near Mead, NE. In late July 2002, a special session of the Nevada Legislature passed medical malpractice reform legislation. The expressly-stated purpose of this statute is remedying, or at least ameliorating, the "serious threat to the health, welfare and safety of [Nevada] residents" which is posed by the state's "extreme difficulties attracting and maintaining a sufficient network of physicians to meet [residents'] needs." Moreover, the measure emphasizes substantive reforms that are primarily intended to limit the potential liability of certain health care providers for negligent actions. Diabetic neuropathy develops as a result of hyperglycemia- induced local metabolic and microvascular changes in both type I and type II diabetes mellitus. Diabetic neuropathy shows slower impulse conduction, axonal degeneration, and impaired regeneration. Diabetic neuropathy affects peripheral, central, and visceral sensorimotor and motor nerves, causing improper locomotor and visceral organ dysfunctions. The pathogenesis of diabetic neuropathy is complex and involves multiple pathways. Lack of success in preventing neuropathy, even with successful treatment of hyperglycemia, suggests the presence of early mediators between hyperglycemia-induced metabolic and enzymatic changes and functional and structural properties of Schwann cells (SCs) and axons. Human N-acetyltransferase Type I (NAT1) catalyses the acetylation of many aromatic amine and hydrazine compounds and it has been implicated in the catabolism of folic acid. The enzyme is widely expressed in the body, although there are considerable differences in the level of activity between tissues. A search of the mRNA databases revealed the presence of several NAT1 transcripts in human tissue that appear to be derived from different promoters. In a sub-Saharan African context, limited natural resources, infectious diseases, including those transmitted by arthropod vectors, and chronic exposure to food contaminated with mycotoxin-producing fungi which, among others, are vectored by insects, are among the major constraints to human health. Thus, pest control should be an important component in human health improvement projects. It appears that the advantages of preventive over curative methods are rarely recognised in Africa, with more emphasis being given to the search for the 'silver bullet' than to integrated control approaches. Ring A-Seco Mosquito Larvicidal Limonoids From Turraea Wakefieldii, Mary Ndung'u, Ahmed Hassanali, Antony M. Hooper, Sumesh Chhabra, Thomas A. Miller, Rowena L. Paul, Baldwyn Torto

Five novel limonoids, 1-5, were isolated from the root bark of Turraea wakefieldii and were characterized as tecleaninoid derivatives. This is the first report of the natural occurrence of tecleanin-type limonoids with a five-membered-ring A-seco structure for which we propose the name neotecleanins. The relative stereochemical structures of compounds 1-5 were established on the basis of NMR spectroscopy. The absolute stereochemical structure of 5 was confirmed by X-ray diffraction methods. In mosquito larvicidal assays, compounds 1, 2 and 4 showed dose-dependent larvicidal activity against larvae of Anopheles gambiae s.s. © 2003 Elsevier Ltd. All rights reserved.

Objective: The primary objective of this research is to examine how changing levels of nursing home staffing adequacy and variations in rehabilitation orientation have affected facility deficiencies and the quality of patient care as facilities responded to the Balanced Budget Act of 1997.

The metabolic syndrome, a clinical condition linked to diabetes and cardiovascular disease is a powerful predictor for overall mortality, and is present in more than 20% of the US. population (Ford, Giles, & Dietz, 2002). This study examines gender differences as well as other factors associated with metabolic syndrome as defined by the Adult Treatment Panel III of the National Cholesterol Education Program.

A sample of 10,134 adults between 20-64 years of age was selected from the Third National Health and Nutrition Survey. Metabolic syndrome was present in 19.6 % of this sample. An ecological model of health services was …

Diabetes mellitus, a serious and costly disease, is a public health challenge. Diabetes is controllable, yet non-adherence to prescribed medications causes diabetesrelated complications resulting in hospital admissions and readmissions that may be prevented. A cross-sectional, descriptive-correlational study was conducted to investigate the relationship of metabolic control (A1C) to hardiness, self-efficacy, and perceived medication adherence. Health-Related Hardiness Scale (Pollock, 1990); Long-Term Medication Behavior Self-Efficacy Scale (De Geest et al., 1994); and Perceived Medication Adherence Scale (Allison, 2000) were administered to 215 participants. Oxidative stress, hyperlipidemia, neutrophil activation and endothelial cell dysfunction are characteristic of women with preeclampsia. We used in vitro experiments to test if a combination of oxidative stress and linoleic acid favors a mechanism for neutrophil transendothelial migration. We used linoleic acid because it is one of the fatty acids elevated in preeclampsia and the precursor for arachidonic acid and its inflammatory metabolites.
