Almost half of the 79.5 million forcibly displaced persons worldwide are youth under the age of 18, including refugees. Refugee youth face deliberate threats across all migration stages including violence, abuse, exploitation, poor living conditions, limited or no access to healthcare and education, interrupted family structure, and discrimination. Noteworthy, school psychologists who practice in host countries face new challenges as these diverse youth enroll in public schools. During the migration stage, humanitarian workers are a primary source of psychosocial and educational support for refugee youth and their families. Access to healthcare is a complicated public health challenge. Healthcare access is multi-dimensional and combines characteristics of individuals, their households, and their social and physical environments with system-level characteristics of healthcare delivery systems, organizations and healthcare providers. Access encompasses five dimensions: approachability, acceptability, availability, affordability and appropriateness. This dissertation investigates these five dimensions of access to healthcare in the context of two populations of interest: 1) persons who reside in rural areas and 2) persons who experience mental health challenges. Background: Postpartum depression (PPD) is a multifaceted and complex issue that impacts not only the mother, but also the infant and social system and may influence how healthcare is accessed. Compounding the effects of PPD are comorbidities such as perinatal substance use. Utilization of substances during pregnancy can increase feelings of anxiety and depression, result in cognitive and physiological consequences for the infant and impair relationships with healthcare providers.

Purpose: The studies completed for this dissertation revealed current concerns regarding PPD within the context of screening practices, perinatal substance use and rural practice implications.

Background: Traditionally, American Indian and Alaska Native (AIAN) communities and individuals held ecological knowledge that supported holistic wellness for their tribal members through a variety of medicines and ceremonies. Contemporary Western healthcare systems have largely alienated AIAN people from access to traditional ceremonial practices (TCPs). Despite decades of cultural adaptation of evidence-based practices (EBPs), the deleterious effects of historical and intergenerational trauma are widely documented. This includes disproportionately high rates of problem substance use in both reservation and urban AIAN community settings.

Purpose: Individuals who experience ongoing symptoms after sustaining a mTBI may not receive the help they need because the deficits they endorse on self-report measures are not identified on current standardized cognitive assessments. The purpose of the current investigation is to determine how to better document ongoing cognitive-communication deficits and to characterize the nature of how these deficits impact daily life and communicative participation, using a multidimensional assessment protocol.

Spirituality is not often discussed in the field of school psychology or the public school setting. However, it is understood to be a factor supporting resilience and well-being among children. Aspects of spirituality align with the core components of social-emotional learning (SEL), which relate to awareness of self and others, relationship skills, and responsible decision-making. Montana has had one of the highest suicide rates in the nation for half a century, and since 2000, it has risen almost 50%. Despite suicide’s alarming persistence in the state, there has been minimal academic study of suicide or mental health specifically in Montana, so this thesis attempts to answer a few questions: Why does Montana have such a high suicide rate? This qualitative study employed a phenomenological approach to examine how teachers, school leaders, and students rationalize their roles in the educational process when their institution is bounded by an educational system that openly strives against them. For a context, this study examined apartheid-era South Africa, from 1948 to 1994, which established social and administrative policies that deliberately curtailed the education of Indigenous and other South Africans, as a means of oppressing non-European ethnic groups.

Most research to date on integrated behavioral health (IBH) or integrated primary care (IPC) practices has examined primary care settings nested within academic medical centers or clinics within the U.S. Department of Veterans Affairs. This leaves clinics in rural and/or underserved communities without much representation in the literature. Likewise, provider job satisfaction and burnout have rarely been linked to the IPC model. Through an online cross-sectional survey of medical and behavioral health care providers in various primary care-based health care settings throughout Montana, this cross-sectional study demonstrates that degree of behavioral health integration significantly predicts greater job satisfaction and lower …

Background: Physical activity (PA) promotion is acknowledged as a critical component to improve public health among the growing population of older adults in the U.S. American Indian and Alaska Native (AIAN) older adults experience disproportionately high rates of chronic disease and disability compared with non-Hispanic White peers, and demonstrate lower levels of PA, indicating a need for examination of unique factors influencing their PA behavior.

Intent on more systematically developing the emerging professional identities of law students, the professional identity formation movement is recasting how we think about legal education. Notably, however, the movement overlooks the structural racism imbedded in American law and legal education. While current models of professional development value diversity and cross-cultural competence, they do not adequately prepare the next generation of legal professionals to engage in the sustained work of interrupting and overthrowing race and racism in the legal profession and system. Physical experiences with ancient art objects in museums are rare. Display paradigms in most public institutions continue to propagate systems of participant interaction that reinforces unequal power structures. The Montana Musuem of Art and Culture (MMAC) is the current custodian of an ancient, Rhodian wine amphora that provides an opportunity to examine a novel system of somatic participation. Objectives: Transgender individuals, often highly stigmatized and discriminated against, face challenging barriers regarding seeking routine- and transition-related physical and mental health care (James et al., 2016). With increasing age, older individuals face a higher likelihood of developing dementia. The rate of cognitive decline resulting from dementia is not equivalent for all, as some patients with dementia are able to function independently longer than others, despite having similar disease burden. The cognitive reserve (CR) theory provides one explanation for the differing rate of decline. CR suggests that there are factors — most notably, educational and occupational attainment — that can protect against the cognitive decline that is a hallmark of dementia.
