Flight training accidents constitute 14% of general aviation accidents. Herein we determined the rates, injury severity, and phase of flight for primary student solo accidents/incidents (mishaps) in Cessna 172 aircraft.
Mishaps over the period spanning 1994–2013 were identified from the NTSB database. Student population data were from the FAA. Statistics employed proportion tests, Poisson distribution, and Mann-Whitney tests.
Across the study period, 598 mishaps were identified. While the mishap rate increased nearly two-fold between 1994/1997 and 2002/ 2005, a 35% decline was evident thereafter. Nevertheless, no statistical difference in mishap rates was evident between the initial and current periods. Over …

Loss of control is a serious problem in aviation that primarily affects General Aviation. Technological advancements can help mitigate the problem, but the FAA certification process makes certain solutions economically unfeasible. This investigation presents the design of a generic adaptive autopilot that could potentially lead to a single certification for use in several makes and models of aircraft. The autopilot consists of a conventional controller connected in series with a robust direct adaptive model reference controller. Flight data recorders (FDRs, or black boxes) generate data that is collected on an embedded memory device. A well-known difficulty with these devices is that the embedded memory device runs out of space. To avoid getting into this problematic situation, the software of the FDR is designed to operate in a watchful mode, constantly working to minimize the use of memory space; otherwise a larger FDR would be needed. The purpose of this qualitative study was to investigate the relationship between the elements/processes of safety management systems (SMSs) and their impact on safety culture at collegiate flight training institutions. Research questions addressed the following: different approaches to developing and implementing an SMS, different approaches to the assessment of safety culture, and the relationship between elements/processes of an SMS and a strong safety culture. A semistructured interview protocol was used. The researcher interviewed five safety professionals at U.S. collegiate flight training institutions of various sizes. The FAA’s release of 14 CFR 107 establishes a new Remote Pilot certification for operation of small unmanned aircraft system (UAS) platforms. Remote Pilot privileges are valid for two years, during which the operator must complete a recurrent aeronautical knowledge test covering the seven topical areas of knowledge identified by 14 CFR 107.73(b). While the existing recertification methods allow for validation of remote pilot knowledge, they fail to engage this new group of aviators in establishing positive safety culture. Probabilistic Risk Assessment (PRA) is a staple in the engineering risk community, and it has become to some extent synonymous with the entire quantitative risk assessment undertaking. Limitations of PRA continue to occupy researchers, and workarounds are often proposed. After a brief review of this literature, we propose to address some of PRA׳s limitations by developing a novel framework and analytical tools for model-based system safety, or safety supervisory control, to guide safety interventions and support a dynamic approach to risk assessment and accident prevention. A recent alleged “drone” collision with a British Airways Airbus A320 at Heathrow Airport highlighted the need to understand civil Remotely Piloted Aircraft Systems (RPAS) accidents and incidents (events). This understanding will facilitate improvements in safety by ensuring efforts are focused to reduce the greatest risks. One hundred and fifty two RPAS events were analyzed. The data was collected from a 10-year period (2006 to 2015). Results show that, in contrast to commercial air transportation (CAT), RPAS events have a significantly different distribution when categorized by occurrence type, phase of flight, and safety issue. The use of small unmanned aircraft systems (sUAS) has increased significantly in the past year. Geographic fencing (geo-fencing) is software built into most medium-cost consumer sUAS. This software is typically used to limit the altitude above launch point, the flight distance from the transmitting controller, and/or restrict flight inside a no-fly zone. This paper quantifies the human harm, in the form of fatalities and skull fractures, which could occur as a result of an unmanned aircraft falling from a height. The analysis is used to establish the maximum height at which an unmanned aircraft can be flown over people to achieve a level of safety consistent with the rate of ground fatalities from General Aviation. The maximum height is dependent on the aircraft mass and the population density of people on the ground below.

Obesity affects over 25% of Americans; however, prescribed FAA standard passenger weights for US airlines are based on data compiled 15 years ago. Since increased passenger weight degrades aircraft performance and may lead to a loss of control, the hypothesis herein is that passenger weight under-estimation for states with high obesity rates could potentially lead to a runway overrun or the inability to out climb rising terrain. In terms of the employed methodology, current person weights for the ten most obese states were determined using nationwide data adjusted for state ethnicity.
