The purpose of this study was to explore the healthcare professional’s experience during the implementation of information technology. The focus is on the implementation of a computerized medication ordering, dispensing, and tracking system in acute care departments of a 600- bed public hospital. Spradley’s qualitative, ethnographic research methods were used to discover the personal meaning of the implementation. How do pharmacists and nurses feel about the implementation of an advanced technology system? Are there any barriers to implementation? This article describes how one health care executive charted her career course to achieve the advanced management competencies defined by Longest. To achieve success in the new and complex world of integrated health care delivery networks, aspiring executives are encouraged to develop a broad base of knowledge and experience that capitalizes on their unique skills, talents, and interests, rather than pursuing a narrowly defined linear career ladder. The importance of management team competency and selection of educational preparation are highlighted in the context of examples of advanced management competency development.

As a result of the ever changing and expanding role of the nursing home administrator in conjunction with the stricter legislation governing nursing facilities over the past decade and the graying of America, a study specific to this population was warranted. The purpose of this study is to determine the relative change in the self-reported occupational stressors of nursing home administrators over a five year period (December 1994 to June 1999) secondary to the increased regulatory climate of the nursing home industry.

Chemotherapy can be considered both a physiologic and a psychological stressor. Anxiety, nausea, emesis, and immunosuppression are commonly associated with chemotherapy, and may adversely affect the outcome or the process of treatment. Chemotherapy-related anxiety can exacerbate negative side-effects. Some psychological stressors, such as anxiety, can result in down-regulation of certain immune functions. Studies have demonstrated that behavioral interventions, such as specific relaxation strategies, can modify stress-related immune modulation. These behavioral interventions have also been shown to decrease the degree of nausea and vomiting associated with chemotherapy. การวิจัยเชิงพรรณนา (Descriptive Research) ครั้งนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาค่าใช้จ่ายของ ผู้บาดเจ็บจากอุบัติเหตุจราจรทางบกที่เข้ารับบริการโรงพยาบาลมหาราชนครศรีธรรมราช 2) เปรียบเทียบ ค่าใช้จ่ายในประเภทผู้บาดเจ็บที่เป็นคนเดินเท้า คนขับขี่ และคนโดยสาร 3) เปรียบเทียบค่าใช้จ่ายของ การบาดเจ็บที่เกิดจากการใช้ยานยนต์ที่ต่างกัน 4) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่คาดเข็มขัด นิรภัย และไม่คาดเข็มขัดนิรภัย 5) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่สวมหมวกนิรภัยและไม่สวม หมวกนิรภัย 6) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่ดื่มแอลกอฮอล์และไม่ดื่มแอลกอฮอล์ 7) เปรียบเทียบ ค่าใช้จ่ายในช่วงเวลาที่ได้รับอุบัติเหตุ กลุ่มตัวอย่าง คือ ผู้บาดเจ็บจากอุบัติเหตุจราจรทางบกที่เข้ารับบริการ รักษาที่โรงพยาบาลมหาราชนครศรีธรรมราช จํานวน 351 ราย โดยการสุ่มตัวอย่างแบบ Cluster Sampling โดยจับสลากผู้บาดเจ็บที่มารับบริการในระยะเวลาหนึ่งเดือน เครื่องมือที่ใช้ในการวิจัยคือ แบบบันทึกการเฝ้าระวังการบาดเจ็บระดับจังหวัด และแบบบันทึกค่ารักษาพยาบาลโรงพยาบาลมหาราช นครศรีธรรมราช วิเคราะห์ข้อมูลโดยหาค่าร้อยละ ค่าเฉลี่ยส่วนเบี่ยงเบนมาตรฐาน ทดสอบความแตกต่าง ของค่าเฉลี่ย โดยการทดสอบค่า ที และการวิเคราะห์ความแปรปรวนทางเดียว \nผลการวิจัยพบว่า ผู้บาดเจ็บที่ไม่นอนโรงพยาบาล จํานวน 251 คน มีค่าใช้จ่ายเฉลี่ย 200 บาทต่อคน ผู้บาดเจ็บที่เป็นคนเดินเท้าจะมีค่าใช้จ่ายมากกว่าผู้ขับขี่และผู้โดยสาร ผู้บาดเจ็บที่การบาดเจ็บเกิดจาก ยานพาหนะอื่น ๆ มีค่าใช้จ่ายสูงกว่าการบาดเจ็บที่เกิดจากรถจักรยานยนต์ ผู้บาดเจ็บที่ดื่มแอลกอฮอล์ มีค่าใช้จ่ายที่สูงกว่า ผู้บาดเจ็บที่ไม่ดื่มแอลกอฮอล์ อย่างมีนัยสำคัญที่ระดับ 0.05 ส่วนค่าใช้จ่ายเฉลี่ย ของผู้บาดเจ็บที่คาดและไม่คาดเข็มขัดนิรภัย, ผู้บาดเจ็บที่สวมและไม่สวมหมวกนิรภัยผู้บาดเจ็บที่เกิดเหตุ ในเวลาที่ต่างกัน ไม่มีความแตกต่างกัน ผู้บาดเจ็บที่นอนโรงพยาบาล จำนวน100 คน มีค่าใช้จ่ายเฉลี่ย 7,196 บาทต่อคน ไม่มีความแตกต่างกัน ในค่าใช้จ่ายเฉลี่ยของผู้บาดเจ็บในกลุ่มของประเภทผู้บาดเจ็บ ประเภทยานยนต์ ผู้บาดเจ็บที่คาดและไม่คาดเข็มขัดนิรภัย, ผู้บาดเจ็บที่สวมและไม่สวมหมวกนิรภัย, ผู้บาดเจ็บที่เกิดเหตุในเวลาที่ต่างกัน

ศึกษาระดับแอลฟา-ฟีโตโปรตีน (AFP) และฮอร์โมนฮิวแมน คลอริโอนิค โกนาโดโทรปิน (HCG) ในสตรีมีครรภ์ที่เข้ารับการตรวจครรภ์ที่โรงพยาบาลสมุทรปราการ โดยทำการเจาะเลือดที่ช่วงอายุครรภ์ \nระหว่าง 14-18, 22-26 และ 30-34 สัปดาห์ ตามลําดับ พบว่าระดับ AFP มีค่า 61.42 ± 41.81 IU/ml (74.32 ± 50.59 ng/ml), 208.54 ± 72.62 IU/ml (252.33 ± 87.87 ng/ml) และ 260.41 ± 96.08 IU/ml (315.10 ± 116.26 ng/ml) ตามลำดับ และระดับฮอร์โมน HCG มีค่า 35,533.35 ± 200000.45 mIU/ml(3,826.59 ± 2153.85 ng/ml), 24,150.62 ±13191.36 mIU/ml (2,600.78 ± 1420.58 ng/ml) และ 351917.35 ± 22697.51 mIU/ml (3,867.94 ± 2444.29 ng/ml) ตามลำดับ ส่วนน้ำหนัก เฉลี่ยของทารกแรกเกิดมีค่า 3,121.74 + 363.64 กรัม

การประเมินความพึงพอใจของผู้รับบริการและผู้ให้บริการในคลินิกเวชปฏิบัติครอบครัว กลุ่มงานเวชกรรมสังคม โรงพยาบาลหาดใหญ่ (Program Evaluatlion On The Satisfaction Of Clients And Health Teams At Family Medicine Clinic Department Of Social Medicine, Hat Yai Hospital), นวลตา อาภาคัพภะกุล

การศึกษานี้วัตถุประสงค์ เพื่อประเมินความพึงพอใจของผู้ให้บริการและผู้รับบริการคลินิกเวชปฏิบัติ ครอบครัว โรงพยาบาลศูนย์หาดใหญ่ โดยมีกลุ่มตัวอย่าง ดังนี้ 1) ผู้ป่วยที่มารับบริการ จำนวน 200 คน 2) แพทย์ผู้ให้บริการประจำคลินิกเวชปฏิบัติครอบครัว 2 คน 3) พยาบาลและเจ้าหน้าที่ จำนวน 27 คน 4) แพทย์ทั้งในและนอกคลินิกเวชปฏิบัติครอบครัว 4 คน 5) หัวหน้ากลุ่มงานเวชกรรมสังคม และ 6) ผู้อำนวยการโรงพยาบาลศูนย์หาดใหญ่ การเก็บรวมรวมข้อมูล โดยใช้แบบสอบถามและสัมภาษณ์ เจาะลึก ผลการศึกษาพบว่า 1) ผู้รับบริการส่วนใหญ่เป็นกลุ่มผู้ประกันสุขภาพ มีอาชีพค้าขายและรับจ้าง รายได้เฉลี่ย 6,600 บาทต่อเดือนต่อครอบครัว มีการศึกษาระดับประถมศึกษาตอนต้น มาใช้บริการเฉลี่ย 6 ครั้งต่อปี ผู้รับบริการส่วนใหญ่พักอาศัยอยู่ในชุมชนใกล้เคียงที่ต้องใช้เวลาในการเดินทางมารับบริการเฉลี่ย ประมาณ 18 นาที รวมเวลาที่ใช้ในการมารับบริการที่คลินิกประมาณ 37 นาที และคิดว่าจะยังคงมาใช้บริการ \nที่คลินิกเวชปฏิบัติครอบครัวเพราะมีความพอใจในบริการที่ได้รับและไม่เสียเวลาในการรอรับบริการมากกว่า เหตุผลอื่น 2) โดยเฉลี่ยมีความพึงพอใจในระดับมาก ในเรื่องการประชาสัมพันธ์ห้องรอตรวจ แพทย์ ห้องยา ห้องทำหัตถการ และบรรยากาศทั่วไป แต่ที่พอใจน้อยที่สุดคือ คุณภาพของยาที่ได้รับ 3) พยาบาลและ เจ้าหน้าที่ มีความพึงพอใจต่อการให้บริการและเห็นความสำคัญของการจัดให้มีคลินิกเวชปฏิบัติคู่การดูแล ทั้งชุมชนได้ดี 6) ผู้บริหารระดับกลางและสูงพอใจในระดับหนึ่ง การขยายงานต่อไป คือ การเป็นแม่ขาย ในการดำเนินงาน และเป็นแหล่งความรู้ของนักศึกษาแพทย์ พยาบาล และบุคลากรสาธารณสุข คลินิกเวชปฏิบัติครอบครัวจะยังคงดำเนินนโยบายและให้การสนับสนุนต่อไป และเน้นที่การเป็นแม่ข่าย ในการดำเนินงาน และเป็นแหล่งความรู้

National surveys to determine trends in contraceptive use in the United States have occurred every 6 to 10 years since 1955.1,2,3 Since that time, there has been a rapid decline in the use of natural methods of family planning; a sharp rise and slow decline in the use of oral contraceptives and, in the past ten years, a major reliance on surgical sterilization for contraception. The growing feminization of the migration of ethnic indigenous women, such as Mixtecs, Zapotecs and Triquis from the southern Mexican state of Oaxaca, are altering the dynamics and configuration of traditional Mexican migration, making it especially significant for the health professions. Informed by a postcolonial ecofeminist paradigm, this bi-national grounded theory study explored the migration and health experiences of indigenous Oaxacan immigrant/migrant women. Data collection using participant observation and interviewing occurred in Oaxaca, Mexico, along the US-Mexico border, and the interior of California. Dimensional analysis was used as the method of data analysis to generate theoretical understandings of these phenomena. …

The purpose of this study was to describe and assess certified nurse-midwives’ (CNMs) knowledge and promotion of two modalities for child spacing, natural family-planning (NFP) and the lactational amenorrhea method (LAM). One thousand two hundred CNMs were randomly selected from a national membership list and mailed a 24-item questionnaire on NFP and LAM. Of the 514 respondents (42.8% return rate), 450 (87.5%) were currently practicing as CNMs. Respondents had an average age of 46 years, with an average of 10 years of practice. Swimmers, like all athletes, require a well-balanced diet with adequate energy intake for normal daily activities and the demands of training and competition. Investigators have typically studied nutritional status of athletes during heavy or peak training (1), (2). These studies generally show low levels of energy intake relative to expenditure, and carbohydrate intake also well below recommendations. Living In A House Of Cards: Family Experiences With Long-Term Childhood Technology Dependence, Maureen O'Brien

Families' experiences of providing long-term home care for the child who is technology dependent were explored via parental interviews. Families identified frequent change, uncertainty, and unpredictability in their lives, a phenomenon described as "living in a house of cards." Attempts to increase stability involved the use of vigilance, advocacy, and reframing. Areas of challenge, change, and growth included making sense of life, managing daily life with technology, and maintaining a functioning family. Increased understanding of family experiences with long-term childhood technology dependence can provide nurses with additional strategies for providing optimal care to this population.

This secondary analysis evaluated the effect of a mutual goal setting nursing approach on the level of self-efficacy to manage disease in general in patients with congestive heart failure (CHF). The conceptual frameworks utilized were King's theory of goal attainment and Bandura's theory of self-efficacy. The primary study used a convenience sample of 54 patients who had been admitted to two home healthcare agencies with the primary diagnosis of CHF.
