Background: In the United States, 52.8% of Americans aged 18 and older consume alcohol, with 29.5 million suffering from alcohol use disorder (AUD). In Los Angeles, 69.8% of Angelinos aged 12 and older suffer from AUD, and 33% of annual arrests are due to alcohol and other drugs. Alcohol withdrawal syndrome (AWS) develops after abrupt cessation of heavy drinking and is a common occurrence among incarcerated people when newly arrested, resulting in $1.3 billion in healthcare costs. Management of AWS using a fixed-dose therapy of benzodiazepines will improve the utilization of healthcare services in the correctional setting.

Childhood sexual abuse (CSA) is a notable public health concern, affecting a significant proportion of girls in high-income countries. A considerable number of CSA survivors develop Post-Traumatic Stress Disorder (PTSD) by adolescence, which is often considered to be a lifelong disorder with severe emotional and social consequences. Women with CSA-related PTSD display hypervigilance, which is characterized by significantly increased fear network activity and poor top-down control over the amygdala. This meta-analysis examines the potential of intranasal oxytocin (OT) as a treatment for hypervigilance symptoms in women with CSA-related PTSD. Major depressive disorder (MDD) is a mood disorder characterized by persistently depressed mood and/or anhedonia that causes significant functional impairment in daily life. MDD affects nearly 15% of adults in their lifetime, and since 2019, prevalence estimates have increased up to ~30% in young adults (18-25 years old). In addition to its debilitating effects on daily functioning, epidemiological studies link MDD to ~3 times greater risk of cardiovascular disease (CVD), followed by accelerated CVD progression and mortality. Although the mechanisms contributing to MDD-CVD comorbidity are unclear, there is evidence of stress system dysfunction in adults with MDD.

While the consumption of alcohol has decreased, the consumption of marijuana has increased rampantly due to youth's positive perception after being legalized in 24 states with no clear message of the many adverse effects this drug causes. Alcohol use disorder (AUD) is a prevalent condition and is a leading cause of death for adults aged 20 to 64 (Esser et al., 2022). Patients with AUD who present to acute care hospitals seeking medical treatment are at risk for alcohol withdrawal. Undertreated alcohol withdrawal can progress to delirium tremens, which can result in increased transfers to ICU level of care, increased need for sedative medications, and has been shown to prolong hospital length of stay. Previous work in human and animal studies has implicated aberrant insular cortex (IC) activity in the pathophysiology of hallucinations. Of note, the IC overlaps with the olfactory cortex (OC), a region of the brain that receives extensive projections from sensory neurons of the olfactory epithelium. SARS-CoV-2 (COVID-19) is a highly infectious coronavirus with a demonstrated ability to cause anosmia; the virus does this by colocalizing with and damaging support cells in the olfactory epithelium, ultimately leading to alterations in olfactory receptor cell signaling to the OC. More than half of Americans will be diagnosed with a mental illness in their lifetime (CDC, 2021), yet stigma towards mental health affects both patients and providers. Although programs exist to address stigma, improve cultural competency among providers, and educate families on the importance of support, facilities are often limited on programs they provide due to allocation of resources and funds. Without a shift in treatment and programing, stigma will continue to impact patient care and outcome.

The heart–brain interaction is the main mechanism for maintaining normative physiological processes, and its dysregulation underlies the somatic symptoms of various mental disorders. Cortical inhibition, triggered by afferent signals from baroreceptor activation, induces systematic variations in sensorimotor responses within a cardiac cycle, with reaction times (RTs) slower at cardiac systole compared to diastole (known as cardiac cycle time effects). However, recent data suggest that baroreceptor afferents not only inhibit simple responses but also facilitate complex sensorimotor responses during cardiac systole. The Minnesota Longitudinal Study of Risk and Adaptation (MLSRA) is a landmark prospective, longitudinal study of human development focused on a sample of mothers experiencing poverty and their firstborn children. Autism spectrum disorder (ASD) is a neurodevelopmental disorder that influences an individual’s cognitive functions and social interaction. While most studies have focused on children and adolescents diagnosed with ASD, elevated levels of autistic traits in subclinical populations may also influence individuals’ daily functioning. Autistic traits are also linked to the Big Five personality. Queer women face disproportionately high rates of postpartum depression compared to heterosexual and cisgender individuals. Studies showed that queer women face heightened mental health challenges, including higher rates of depression and anxiety due to factors like discrimination, limited support systems, and negative healthcare experiences. Based on the use of drugs to treat hypertension and tuberculosis for depression, the monoamine hypothesis led to the development of antidepressants in 1952. Serotonin was recognized as a deficiency associated with depression in 1967. As a fine-tuning orchestration of neurotransmitters and neuromodulators, the glutamate system became known using Spravato in 2019 and Auvelity in 2022. In this pilot study, mental health experts rated the importance of 24 science-based recommendations to optimize glutamatergic therapy in depression for a proposed guideline on a Likert scale from zero to four.
