The Readiness for Hospital Discharge Scale, Quality of Discharge Teaching Scale, and Post-Discharge Coping Difficulty Scale were developed by Dr. Marianne Weiss to measure nurse-sensitive discharge care processes and patient outcomes. Background: Childhood immunization rates for children aged 2 years and under at a community health center were assessed and determined to be below optimal levels. Immunizations protect against preventable diseases (Hill, 2023). The project assessed the microsystem to understand low immunization rates and to guide strategies for improvement.

Methods: The Plan-Do-Study-Act (PDSA) model was used. In the planning phase, key stakeholders were consulted to assess current rates and barriers contributing to immunization rates. The chosen intervention was placed simplified immunization schedules in pediatric exam rooms to educate parents and promote adherence.

Background: The benefits of breastfeeding for both mother and baby are known. It is recommended that all infants be exclusively breastfed until six months (Meek & Noble, 2022; North et al., 2021). Despite this, breastfeeding rates remain low, especially in medically fragile infants requiring care in the neonatal intensive care unit (NICU) (Rollins et al., 2016). Receipt of evidence-based breastfeeding support by healthcare providers has been shown to improve breastfeeding success; however, many healthcare providers report not being comfortable or knowledgeable enough to offer support consistently (Mulcahy et al., 2022; Rollins et al., 2016).

ผลของโปรแกรมการเตรียมความพร้อมการดูดกลืน ต่อประสิทธิภาพการดูดนมมารดาของทารกเกิดก่อนกำหนด, นวรัตน์ อธินุวัฒน์

การวิจัยกึ่งทดลองนี้มีวัตถุประสงค์เพื่อศึกษาโปรแกรมการเตรียมความพร้อมการดูดกลืนต่อประสิทธิภาพการดูดนมมารดาของทารกเกิดก่อนกำหนดที่มีอายุหลังมีประจำเดือนระหว่าง 32-36+6 สัปดาห์และมารดา จำนวน 34 คู่ แบ่งเป็นกลุ่มที่ได้รับโปรแกรมการเตรียมความพร้อมการดูดกลืน 17 คู่ และกลุ่มที่ได้รับการพยาบาลตามปกติ 17 คู่ เครื่องมือที่ใช้ในการวิจัยประกอบด้วย แบบประเมินความพร้อมในการดูดนมมารดาของทารกเกิดก่อนกำหนด และแบบประเมินประสิทธิภาพการดูดนมมารดาของทารก โดยเครื่องมือทั้งหมดผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิจำนวน 5 คน มีค่าความตรงเชิงเนื้อหาเท่ากับ 0.87 และ 0.85 และมีค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ 0.88 และค่าสัมประสิทธิ์ Cohen’s Kappa เท่ากับ 0.84 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติการวิเคราะห์ Repeated Measure ANOVA ผลการวิจัยพบว่า ประสิทธิภาพการดูดนมมารดาของทารกเกิดก่อนกำหนดหลังได้รับโปรแกรมการเตรียมความพร้อมการดูดกลืนวันที่ 1, 4 และ 7 ดีกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และประสิทธิภาพการดูดนมมารดาของทารกเกิดก่อนกำหนดหลังได้รับโปรแกรมการเตรียมความพร้อมการดูดกลืนวันที่ 1, 4 และ 7 ดีกว่าทารกเกิดก่อนกำหนดที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ผลการศึกษานี้สามารถนำไปใช้เป็นแนวทางดูแลทารกเกิดก่อนกำหนดในการเตรียมความพร้อมการดูดกลืน เพื่อให้ทารกมีประสิทธิภาพการดูดนมมารดา

การวิจัยนี้เป็นแบบกึ่งทดลองวัดเฉพาะหลังการทดลองแบบมีกลุ่มเปรียบเทียบ เพื่อเปรียบเทียบพฤติกรรมป้องกันภาวะตัวเหลืองซ้ำและระดับบิลิรูบินในเลือดของทารกแรกเกิดที่ได้รับการส่องไฟ ระหว่างกลุ่มทดลองที่ได้รับโปรแกรมส่งเสริมการรับรู้ความสามารถของมารดา และกลุ่มควบคุมที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างเป็นมารดาและทารกแรกเกิดครบกำหนดที่มีภาวะตัวเหลืองและได้รับการรักษาด้วยการส่องไฟ ในโรงพยาบาลราชพิพัฒน์ สำนักการแพทย์ กรุงเทพมหานคร จำนวน 30 คู่ คัดเลือกกลุ่มตัวอย่างตามสะดวก และคัดเข้าด้วยวิธีจับฉลาก โดยจับคู่ (Matched pairs) ให้มีความคล้ายคลึงกันด้านระดับการศึกษา แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 15 คู่ เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย 1) โปรแกรมส่งเสริมการรับรู้ความสามารถของมารดา 2) แบบสอบถามการรับรู้ความสามารถของมารดา 3) แบบสอบถามพฤติกรรมป้องกันภาวะตัวเหลืองซ้ำในทารกแรกเกิดที่ได้รับการส่องไฟ และ 4) แบบติดตามระดับ บิลิรูบินในเลือดทารก ซึ่งได้ผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน แบบสอบถามทั้ง 2 ฉบับมีค่า CVI เท่ากับ 0.88 และ 0.97 ตามลำดับ และมีค่าความเที่ยง (Cronbach's alpha coefficient) เท่ากับ 0.92 และ 0.95 ตามลำดับ วิเคราะห์ข้อมูลคะแนนเฉลี่ยพฤติกรรมป้องกันภาวะตัวเหลืองซ้ำในทารกแรกเกิดที่ได้รับการส่องไฟ และค่าเฉลี่ยระดับบิลิรูบินในเลือดของทารก โดยใช้สถิติ Independent t-test ผลการวิจัย พบว่ามารดากลุ่มทดลอง มีคะแนนเฉลี่ยพฤติกรรมป้องกันภาวะตัวเหลืองซ้ำในทารกแรกเกิดที่ได้รับการส่องไฟสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .01 และทารกในกลุ่มทดลองมีระดับบิลิรูบินในเลือด ณ วันที่มาตรวจติดตาม ต่ำกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติที่ระดับ .01

Antimicrobial resistance is rising while new drug development declines, making antimicrobial stewardship crucial for optimizing use, preventing resistance, and improving patient outcomes. Antibiotics are vital for bacterial infections, but overuse and inappropriate prescribing have contributed to resistance. Although 5-35% of the global population reports β-lactam allergies, over 90% of reports are inaccurate. Adults have the highest prevalence; however, these labels are often acquired in childhood, with a notable increase among children recently. Unverified allergy labels hinder antimicrobial stewardship, leading to longer hospital stays, alternative prescribing, and a higher risk of adverse events.
