This dissertation aims to adapt Meyer’s Minority Stress Model to understand the interplay between minority stress, mental health, and sexual risk behaviors among middle-aged and older adults with and without HIV. The study utilizes data from the MACS WIHS Combined Cohort Study, focusing on adults aged 40 and older. It examines the relationships between perceived stress, psychosocial factors (depression, substance use, social support), and sexual risk behaviors, considering the impact of minority identities such as race, gender, sexual orientation, and HIV status. Background/Purpose

 

Asian populations in the U.S. demonstrate lower adherence to diabetes management guidelines compared to other racial groups. The American Diabetes Association (ADA) recommends a minimum of two annual A1C tests to manage diabetes effectively. This study examines racial disparities in adherence to these guidelines, focusing on the impact of immigration-related factors and patient perceptions of provider communication. The findings aim to identify barriers and inform strategies to enhance diabetes management and improve patient-provider interactions in underserved communities.

Purpose

The purpose of this scholarly project was to improve discharges by 11:00 a.m. within the Monitored Pod Units located in a regional hospital in Bryan, Texas.

Background

The current discharge rate by 1100 for the monitored pod units at the regional health center was below the health system goal of 50%. The impacts of delayed discharges are broad and can be seen throughout the regional health center. Delayed discharges have a direct effect on staff satisfaction, patient satisfaction, and decreased revenue.

This quality improvement (QI) project aimed to reduce hospital-acquired pressure injuries (HAPIs) by 10% in an intensive care unit through the implementation of a skin risk assessment form and a visual reminder system.

Background

Hospital-acquired pressure injuries negatively impact patient outcomes and can increase costs associated with a hospital stay. Studies have shown that visual reminders can improve nursing intervention compliance and correlate with improved patient outcomes.

This thesis presents the design, implementation, and mixed-methods evaluation of Systems Simulation, a reusable mixed reality (MR) application intended to help undergraduate nursing students explore human anatomy and pathophysiology using immersive 3D visualization.

Introduction/Background: Adult hospitalized medical and surgical patients at an academic medical center are being transferred out of the Intensive Care Unit (ICU) to the step-down floor and having rapid response calls in less than 48 hours after the transfer. Nurse intuition is not a consideration in the decision to transfer a patient. Physiological variables such as Modified Early Warning Scores (MEWS) and vital signs are relied on heavily as indicators for escalation or de-escalation of care. There is evidence supporting the use of acknowledging nurse intuition to better predict a patient’s readiness for transitions of care.

Depression affects 21 million American adults (8.4%), yet only 28% receive appropriate screening. In Texas, where 20.8% of adults experience depression, systematic screening remains inconsistent. This quality improvement project addressed a critical gap at an outpatient mental health clinic in the Dallas-Fort Worth metroplex, where Patient Health Questionnaire-9 (PHQ-9) screening occurred in only 40% of psychiatric evaluations, resulting in missed diagnoses and inconsistent treatment. Using the Plan-Do-Study-Act framework, the project implemented a comprehensive PHQ-9 screening bundle consisting of staff training, electronic health record integration, and a treatment algorithm based on severity thresholds. This randomized controlled trial with repeated measures aimed to evaluate the effectiveness of an integrated application supported MBCT program in reducing depression among adolescents with NSSI. The sample consisted of 96 participants who met the inclusion criteria and were randomly assigned to either the experimental group or control group, with 48 participants in each group. The experimental group received the program, was developed based on the MBCT framework proposed by Segal et al. (2013) once a week for 60 minutes over eight weeks. This DNP Project aimed to influence a process change within the nursing faculty advising process for pre-licensure nursing students at the project site. The clinical site mandates faculty to meet with their assigned advisees twice per academic semester for advising. วัตถุประสงค์: ศึกษาความร่วมมือในการรักษาของผู้ติดเชื้อเอชไอวีที่มีโรคร่วมวัณโรค และศึกษาปัจจัยที่มีความสัมพันธ์ กับความร่วมมือในการรักษาของผู้ติดเชื้อเอชไอวีที่มีโรคร่วมวัณโรค

รูปแบบการวิจัย: การวิจัยเชิงบรรยายแบบหาความสัมพันธ์

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ที่ได้รับการวินิจฉัยว่าติดเชื้อเอชไอวีและเป็นวัณโรค มีอายุตั้งแต่ 20 -59 ปี จำนวน 84 ราย จากคลินิกยาต้านเอชไอวีและคลินิกโรคปอด ณ โรงพยาบาลระดับตติยภูมิ 3 แห่งในจังหวัดกรุงเทพมหานคร เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินความร่วมมือในการรักษา แบบประเมินการสนับสนุนทางสังคม แบบประเมินปัญหาการดื่มเครื่องดื่มแอลกอฮอล์ แบบประเมินความรู้เกี่ยวกับการรักษาด้วยยาต้านเอชไอวีและยารักษาวัณโรค และแบบประเมินการตีตราภายในตนเองในผู้ติดเชื้อเอชไอวี เก็บรวมรวมข้อมูลระหว่างเดือนมกราคม ถึงเดือนพฤษภาคม 2567 โดยกลุ่มตัวอย่างตอบแบบสอบถามขณะมารับการรักษา ณ คลินิกยาต้านเอชไอวี วิเคราะห์ข้อมูลโดยใช้สถิติบรรยาย สถิติสหสัมพันธ์ของเพียร์สัน และสถิติสหสัมพันธ์ของสเปียร์แมน

ผลการศึกษา: กลุ่มตัวอย่างจำนวน 84 ราย มีระดับความร่วมมือในการรักษาโดยรวมในระดับสูง (  =  56.23, SD  =  6.81) การสนับสนุนทางสังคม มีความสัมพันธ์ทางบวกกับความร่วมมือในการรักษา อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r  =  .218) และ การดื่มเครื่องดื่มแอลกอฮอล์ มีความสัมพันธ์ทางลบกับความร่วมมือในการรักษา อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (rs  =  -.311) แต่ความรู้เกี่ยวกับการรักษาด้วยยา ต้านเอชไอวีและยารักษาวัณโรค (r  =  .189) และ การตีตราภายในตนเอง (r  =  -.119) ไม่มีความสัมพันธ์กับความร่วมมือในการรักษาอย่างมีนัยสำคัญทางสถิติ

สรุป: ผลการศึกษานี้สามารถเป็นแนวทางในการพัฒนาโปรแกรมในการพยาบาล การส่งเสริมพฤติกรรมการให้ความร่วมมือในการรักษาของผู้ติดเชื้อเอชไอวีที่มีโรคร่วมวัณโรค

วัตถุประสงค์: เพื่อศึกษาความสัมพันธ์ระหว่างปัจจัยคัดสรรกับความเข้มเข็งทางใจของผู้สูงอายุโรคหลอดเลือดสมอง

แบบแผนงานวิจัย: การวิจัยเชิงบรรยาย

วิธีดำเนินการวิจัย: กลุ่มตัวอย่างคือผู้สูงอายุที่ได้รับการวินิจฉัยว่าเป็นโรคหลอดเลือดสมองครั้งแรก จำนวน 93 คน ที่มาติดตามการรักษาที่แผนกผู้ป่วยนอก โรงพยาบาลระดับตติยภูมิของรัฐ ในเขตกรุงเทพมหานคร สุ่มกลุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือวิจัย ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินกิจวัตรประจำวันดัชนีบาร์เธลเอดีแอล แบบประเมินความเศร้าในผู้สูงอายุไทย แบบสอบถามความรู้สึกไม่แน่นอนในความเจ็บป่วย แบบประเมินความรู้สึกมีคุณค่าในตนเอง และแบบสอบถามความเข้มแข็งทางใจสำหรับผู้สูงอายุไทย ค่าความเที่ยงสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .84, .85, .87, .80 และ .93 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติพรรณนา สถิติสหสัมพันธ์อีต้า สหสัมพันธ์สเปียร์แมนและสหสัมพันธ์เพียร์สัน

ผลการวิจัย: 1) ความเข้มแข็งทางใจของผู้สูงอายุโรคหลอดเลือดสมองโดยรวมอยู่ในระดับปานกลาง ( = 72.83, SD = 0.64) 2) ปัจจัยที่มีความสัมพันธ์ทางบวกกับความเข้มแข็งทางใจของผู้สูงอายุโรคหลอดเลือดสมอง ได้แก่ ระดับการศึกษา ความรู้สึกมีคุณค่าในตนเอง ความสามารถในการปฏิบัติกิจวัตรประจำวัน (r = .659, .620, .265 ตามลำดับ) ส่วนภาวะซึมเศร้า และความรู้สึกไม่แน่นอนในความเจ็บป่วยมีความสัมพันธ์ทางลบกับความเข้มแข็งทางใจของผู้สูงอายุโรคหลอดเลือดสมอง (r = -.310, -.603 ตามลำดับ) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 3) เพศและสถานภาพสมรส ไม่มีความสัมพันธ์กับความเข้มแข็งทางใจของผู้สูงอายุโรคหลอดเลือดสมอง

สรุป: การส่งเสริมความเข้มแข็งทางใจของผู้สูงอายุโรคหลอดเลือดสมอง อาจพิจารณานำปัจจัยที่มีความสัมพันธ์จากการค้นพบในการศึกษานี้ เป็นแนวทางในการสร้างโปรแกรมส่งเสริมความเข้มแข็งทางใจของผู้สูงอายุโรคหลอดเลือดสมองต่อไป

วิธีการดำเนินการวิจัย: กระบวนการพัฒนาโปรแกรมแบ่งเป็น 3 ระยะ ได้แก่ 1) การศึกษาสภาพปัญหาของผู้ที่มีภาวะหัวใจขาดเลือดเรื้อรังร่วมกับการทบทวนวรรณกรรม 2) การออกแบบและพัฒนาโปรแกรมส่งเสริม ความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันไลน์และ 3) การทดลองใช้โปรแกรมในกลุ่มผู้ที่มีภาวะหัวใจขาดเลือดเรื้อรัง โปรแกรมถูกประเมิน ในด้านความเหมาะสม ความต่อเนื่องของเนื้อหา และความถูกต้องทางภาษา กลุ่มตัวอย่างเป็นผู้ที่มีภาวะหัวใจขาดเลือดเรื้อรังที่เข้ารับการรักษาในโรงพยาบาลเลิดสิน จำนวน 15 คน เครื่องมือที่ใช้ในการศึกษาได้แก่ โปรแกรมส่งเสริมความรอบรู้ด้านสุขภาพ แบบประเมินความพึงพอใจ และแบบสอบถามพฤติกรรมการดูแลตนเองของผู้ป่วยโรคหลอดเลือดหัวใจ การวิเคราะห์ข้อมูลใช้สถิติ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน

ผลการศึกษา: โปรแกรมส่งเสริมความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันไลน์มีคะแนนเฉลี่ยด้านความสะดวกในการใช้งาน 4.6 (±0.48) คะแนนเฉลี่ยด้านการใช้ประโยชน์ได้จริงของโปรแกรม 5 (±0.00) คะแนนเฉลี่ยด้านความปลอดภัยในการใช้งาน 4.53 (±0.51) และคะแนนเฉลี่ยด้านความพึงพอใจในการใช้โปรแกรม 4.8 (±0.35) ชี้ให้เห็นว่าโปรแกรมดังกล่าวมีความเหมาะสมที่จะนำไปใช้ในการศึกษากับกลุ่มตัวอย่างที่มีขนาดใหญ่ขึ้นในอนาคต

วัตถุประสงค์: เพื่อศึกษาคุณภาพชีวิตด้านสุขภาพและความสัมพันธ์ระหว่าง อายุ เพศ โรคร่วม ดัชนีมวลกาย อาการ และสภาวะการทำหน้าที่ กับคุณภาพชีวิตด้านสุขภาพของผู้ติดเชื้อโควิด-19 หลังจำหน่ายออกจากระบบ

รูปแบบการวิจัย: การวิจัยแบบพรรณนาเชิงความสัมพันธ์

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง เป็นผู้ป่วยที่เคยติดเชื้อโควิด-19 และได้รับการจำหน่ายจากโรงพยาบาลรามาธิบดี จำนวน 113 คน เครื่องมือที่ใช้ประกอบด้วย แบบประเมินอาการ แบบประเมินสภาวะการทำหน้าที่ และแบบประเมินคุณภาพชีวิตด้านสุขภาพ ตรวจสอบความตรงตามเนื้อหา ได้ .80 1.0 และ .88 และความเที่ยงได้ค่าสัมประสิทธิ์แอลฟาของครอนบาค .83 .83 และ .81 วิเคราะห์ข้อมูลด้วยสถิติพรรณนา สัมประสิทธิ์อีต้า และสัมประสิทธิ์สหสัมพันธ์เพียร์สัน

ผลการศึกษา: พบว่า คุณภาพชีวิตด้านสุขภาพของผู้ติดเชื้อโควิด-19 หลังจำหน่ายออกจากระบบโดยรวมอยู่ในระดับปานกลาง (M = 23.38, SD = 3.55) โดยสภาวะการทำหน้าที่มีความสัมพันธ์ทางบวกกับคุณภาพชีวิตด้านสุขภาพของผู้ติดเชื้อโควิด-19 หลังจำหน่ายออกจากระบบอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = .257) และอาการมีความสัมพันธ์ทางลบกับคุณภาพชีวิตด้านสุขภาพของกลุ่มตัวอย่าง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 โดยอาการที่มีความสัมพันธ์ ได้แก่เจ็บหน้าอก/ใจสั่น อาการอ่อนเพลีย/เหนื่อยล้า
อาการหายใจลำบาก/หอบเหนื่อย อาการปวดศีรษะ/เวียนศีรษะ อาการนอนไม่หลับ อาการมึนงง/คิดไม่ออก อาการปวดเจ็บในข้อ และปัญหาด้านอารมณ์ (r = -.363, -.292, -.247, -.250, -.281, -.295, -.229 และ -.198 ตามลำดับ) ส่วนอายุ เพศ โรคร่วม และดัชนีมวลกาย ไม่มีความสัมพันธ์กับคุณภาพชีวิตด้านสุขภาพของผู้ติดเชื้อโควิด-19 หลังจำหน่ายออกจากระบบ

สรุป: ผลของการศึกษาปัจจัยที่มีความสัมพันธ์กับคุณภาพชีวิตด้านสุขภาพของผู้ติดเชื้อโควิด-19 หลังจำหน่ายออกจากระบบจะเป็นข้อมูลในการเตรียมการดูแลผู้ติดเชื้อกลุ่มนี้ ต่อไป

มะเร็งจอประสาทตา เป็นมะเร็งที่พบบ่อยในเด็ก เกิดจากความผิดปกติของเซลล์ต้นกำเนิดในชั้นจอประสาทตา เนื่องจากโครโมโซมคู่ที่ 13 เกิดการผ่าเหล่าที่ผิดปกติ เซลล์จึงมีลักษณะการเจริญไปเป็นเซลล์มะเร็ง ส่งผลกระทบรุนแรงต่อการมองเห็นและอาจเป็นอันตรายถึงชีวิตหากวินิจฉัยล่าช้า การดูแลรักษาผู้ป่วยกลุ่มนี้ต้องอาศัยทีมสหสาขาวิชาชีพ พยาบาลทางคลินิกจึงมีบทบาทสำคัญในการดูแลผู้ป่วยมะเร็งจอประสาทตา โดยมีบทบาทการประเมินปัญหาและการวางแผนการดูแล ตั้งแต่การประเมินปัญหาภาวะสุขภาพ การดูแลและให้การช่วยเหลือเพื่อให้ผู้ป่วยและครอบครัวสามารถปรับตัวได้ การดูแลผู้ป่วยขณะรักษาตัวในโรงพยาบาล ทั้งก่อนและหลังผ่าตัด การดูแลให้การพยาบาลตามแผนการรักษาของแพทย์ นอกจากนี้พยาบาลทางคลินิกยังมีบทบาทในการให้แนวทางการป้องกันภาวะแทรกซ้อน การเตรียมผู้ป่วยเพื่อการฟื้นฟูและแนวทางการสร้างเสริมสุขภาพ ทั้งนี้เพื่อให้ผู้ป่วยเด็กมะเร็งจอประสาทตาและครอบครัวสามารถดูแลตนเองทั้งด้านร่างกายและจิตใจเพื่อคุณภาพชีวิตที่ดีขึ้น

บทความวิชาการนี้มีวัตถุประสงค์เพื่อทบทวนและนำเสนอวรรณกรรมที่เกี่ยวข้องกับโรคมะเร็งจอประสาทตา รวมทั้งอธิบายบทบาทเชิงวิชาชีพของพยาบาลทางคลินิกในการดูแลผู้ป่วยโรคมะเร็งจอประสาทตา เพื่อเป็นแนวทางในการปฏิบัติการพยาบาลสำหรับพยาบาลทางคลินิกในการดูแลผู้ป่วยกลุ่มนี้อย่างมีประสิทธิภาพ
