In chis chapter, the need for long-term care services is explained from the perspective of the explosive growth of the older population as well as the increase in disability and chronic illness in the general population. A definition of long-term care within the contexts of inpatient and community-based services is also explained. In the United States the focus of long-term care is on nursing homes. The potential for doctor of nursing practice students to inform and influence the national health care reform movement inspired faculty teaching an online Health and Social Policy course to create an assignment combining comparative effectiveness research with a trip to Capitol Hill to meet with congressional representatives. Preparing for and participating in such an assignment can present challenges for online doctoral students already busy with family and professional responsibilities. Background: Pancreatic cancer (PC) is the fourth leading cause of cancer death in the United States. Reported risk factors for PC include: advancing age, a family history of PC, high-risk inherited syndromes, cigarette smoking, exposure to occupational and environmental carcinogens, African-American race, high fat/high cholesterol diet, obesity, chronic pancreatitis, and diabetes mellitus.

Purpose: The purpose of this study was to characterize one of the largest national registries of familial PC (FPC) and sporadic PC (SPC), focusing on demographics, clinical factors, self-reported environmental and occupational lifetime exposures and survival status.

Conclusions:

Direct observation and the use of the observational instruments are not clinically useful in predicting milk intake in preterm infants.

PHA, PNA, and BFE should not be used arbitrarily as predictor variables

Test weighing is the most reliable method to measure milk intake.

The PIBBS is a reliable instrument when used by trained lactation specialists but not predictive of milk intake in preterm infants for the volume of milk intake in preterm infants.

The Joint Commission in a joint effort with the Centers of Medicare and Medicaid Services (CMS) has established certain "core measures" by which hospital performance is measured. One of these is the measure for patients with ST-elevation myocardial infarction (STEMI) recommending percutaneous coronary intervention within 90 minutes of presentation to the Emergency Department in institutions that are able to provide this service. This recommendation does not take into account the long-term use of clopidogrel that is recommended by the American College of Cardiology and American Heart Association for patients that are treated with coronary stents. The lived experiences of dementia in older persons have been well studied, but the unique experiences of persons between ages 35 and 65 years who are living with young-onset dementia have not been closely examined. The purpose of the research was to explore the experiences of middle-aged individuals living with young-onset dementia. Van Manen‘s (1990) approach to interpretive phenomenological inquiry was used to answer the research question. Nursing schools struggle to prepare enough nurses to meet the health care needs of a diverse and aging population in the United States. Many students do not complete their degrees, contributing to the problem. The purpose of this qualitative study was to explore and describe how successful Latina students experience and respond to the academic challenges of prerequisite nursing courses. The nursing shortage; healthcare needs of Hispanic patients; student outcomes; self-efficacy, achievement goal orientation, and academic help-seeking; and Hispanic cultural values framed the study. Psychosocial care for patients with cancer is aimed at detection, diagnosis, treatment, and prevention of psychological distress (PD). PD is a universal clinical phenomenon experienced by at least 38% of patients with cancer, yet only10% are identified as having PD. Nurses are presumed providers of psychosocial care, yet no research examined what nurses perceive as their role in caring for patients with cancer, and whether nurses believe that providing psychosocial care to patients with cancer is within their role. Self-management behaviors are the cornerstone for control of type 2 diabetes mellitus (T2DM). Patients living with T2DM manage their own care on a daily basis using a variety of self-care strategies. Rural people living with diabetes are diagnosed later and receive less than optimal care compared to people living with T2DM in urban areas. Research evidence supports a relationship between self-efficacy and self-management behaviors of people living with T2DM. Determining mediators of the relationship between self-efficacy and self-management can provide direction for development of interventions to improve participation in self-management behaviors, which can in turn improve diabetes outcomes.
