Patient Safety Culture (PSC) is recognized as a cornerstone of healthcare quality and a key determinant of patient outcomes. Despite the Institute of Medicine’s early calls to establish safety-oriented systems, evidence on the long-term stability of PSC in U.S. hospitals has remained limited. This dissertation addresses this gap through a multi-year evaluation of PSC using the Hospital Survey on Patient Safety Culture (HSOPSC v1.0) and advanced statistical methods. Drawing on one of the largest national datasets—comprising over 993,000 healthcare providers from 1,601 U.S. hospitals across three survey cycles (2013–2020)—the study employed a longitudinal repeated cross-sectional design. Background

Home parenteral nutrition (HPN) patients often have multiple comorbidities. A validated multimorbidity index (MMI) could help determine resource needs and risks with HPN. We evaluated MMI scoring to determine if it predicted HPN resource use and outcomes.

Methods

We performed a secondary analysis of 60 HPN patients from a previous study based on Cumulative Illness Rating Scale (CIRS) scoring. We examined three variables: PN formula changes, hospitalizations, and hospital length of stay (LOS). Density plots were produced to select a CIRS score cutoff value. Accurate prediction of ICU Length of Stay (LoS) is essential for clinical decision-making and healthcare resource management. Graph Neural Networks (GNNs), such as GraphSAGE, offer a natural fit by capturing patient data from Electronic Health Records (EHRs) through graph structures. However, the distributed and sensitive nature of this data raises both privacy and legal concerns regarding the aggregation and training of GNN models. Hospital Length of Stay (LOS) analysis is essential for managing hospital efficiency and financial performance, yet the analytical depth required remains inaccessible to many organizations due to technical and resource constraints. This thesis introduces the AI Agent Modular Capabilities (AIAM) framework, which decomposes LOS analysis into discrete, codifiable capabilities integrated into a single skill activatable with one prompt. Seven AI Agent Modular Capabilities were developed using a Work Breakdown Structure and tested against a simulated hospital scenario across three successive skill versions. BACKGROUND: Hospital acquired pressure injuries (HAPIs) continue to plague hospitals across the country. HAPIs are an indication of the quality of care delivered by the hospital’s nursing workforce. These injuries are costly to organizations and can lead to increased length of stay (LOS). The development of a HAPI, depending on severity, is a reportable injury and is classified as patient harm. Title: Improving Confidence in Certified Registered Nurse Anesthetists in Providing Care for Patients Undergoing Transcatheter Aortic Valve Replacements (TAVR)

Background: Anesthesia staffing for complex, high-risk procedures like Transcatheter Aortic Valve Replacements (TAVR) is challenging due to a national shortage of anesthesia providers and the company's heavy reliance on non-permanent (locum tenens and 1099) Certified Registered Nurse Anesthetists (CRNAs). At this TAVR procedure location, 70% of the team is comprised of non-permanent staff. A lack of specialized training and experience in TAVR procedures led to a decrease in provider confidence and staffing shortages for these critical cases.

With new regulations placed upon abortion care forcing the closing of low-cost health clinics such as Planned Parenthood, legislation that has made health insurance more expensive, and rising pharmaceutical prices, women’s OB/GYN care in America has struggled to meet patient needs. Dental insurance claims sit at the intersection of professional ethics, contract administration, and regulatory oversight. This article argues that claim submission and payer adjudication should be treated as ethical practices grounded in truthfulness, fair use of shared resources, and respect for patient autonomy.
