This purpose of this study is to quantify potential cost savings and member care improvements as a result of engagement through BlueCross BlueShield of Tennessee’s (BCBST) Identification and Stratification (IDStrat) process. Commercial members engaged in clinical management that were identified through IDStrat were compared to commercial members identified through other means across several metrics including per-member, per-month (PMPM) cost and physician visits. Members identified by IDStrat experienced a statistically significant 7% greater reduction in costs after being engaged when compared with those identified by other methods. A patient's medical problem list describes his or her current health status and aids in the coordination and transfer of care between providers, among other things. Because a problem list is generated once and then subsequently modified or updated, what is not usually observable is the provider-effect. That is, to what extent does a patient's problem in the electronic medical record actually reflect a consensus communication of that patient's current health status? Problem/Impact Statement: The pharmacists in Scarborough & Westbrook Primary Care conduct Medicare Annual Wellness Visits (AWVs) with a physician. The pharmacist and physician see the patient individually. The practices achieved and sustained FY18 Joint AWV volume goals through a new process to reschedule physician-only AWVs to Joints AWVs with a pharmacist. This leads to reschedule rework and reduced time practice staff have for patient care.

Increasing Patient Satisfaction Survey Response Rate At An Ambulatory Surgery Center, Carlee Nicholson

In an effort to increase distribution rate and response rate of the patient satisfaction survey at this ASC, adjustments were made to current processes. These interventions included accentuating the option of a paper or emailed survey, providing pre-operative and post-operative response prompts, as well as adding a cover letter to the existing survey. The purpose of these interventions was to ensure a 100% distribution rate of the survey as well as to increase the response rate by 20%.

Results were obtained by collecting forms to monitor distribution of the survey. The response rate was calculated by the number of responses …

Background: Intravenous immune globulin (IVIG) is a plasma-derived product from pooled donors. It has six FDA approved indications. It is also considered standard of care for a few other conditions in specific populations. Additionally, there is growing non-evidence-based use. Due to a global shortage, IVIG has become increasing difficult to obtain.

Conclusions: Development, implementation, and promotion of adherence to an evidence-based criterion allowed our scarce IVIG supply to be sustained. Goal: Standardize all aspects of discharge in a large, busy 84-bed Level IV NICU with >1000 admissions/year and >300 nurses, >60 NNPs, and >25 neonatologists.

Background: Carbon dioxide levels can be monitored throughout the respiratory cycle via capnography. In this way, capnography allows healthcare professionals to follow several respiratory factors in real-time, including respiratory depression, apnea, and hypercapnia. Postoperative patients are subjected to significant harm while receiving sedating medications without appropriate monitoring and intervention. Earlier detection of alterations to ventilation status will better enable providers to more accurately dose medications during procedures, especially in high-risk patient populations.
