Lars Eighner became homeless in 1988 after leaving a job he had held for ten years as an attendant at a state hospital in Austin, Texas. He lives in a small apartment in Austin and continues to scavenge. This article was originally published in the Fall 1990 issue of The Threepenny Review. Reprinted with permission.

Provisions in the Medicaid statute permit states to apply for waivers from traditional program requirements. On January 16, 2009, the federal government approved Rhode Island's Global Consumer Choice Compact Waiver. In exchange for a cap on combined federal and state spending of $12.075 billion through 2013, Rhode Island received greater flexibility to adopt certain Medicaid program changes. On January 16, 2009, the Federal government approved Rhode Island’s application for a Global Consumer Choice Compact Medicaid Waiver whereby the state became the first granted permission to operate its entire Medicaid program under the state plan and a single 1115 “research and demonstration” waiver. The Global Waiver has been implemented in the context of Republican proposals to turn Medicaid into a block grant which would give states substantially more flexibility administering the program in exchange for receiving an upfront allotment from the Federal government. Federal approval of Rhode Island’s Global Consumer Choice Compact Global Waiver in 2009 provided Rhode Island with greater flexibility to modify its Medicaid program. Because 96% of long-term care expenditures in Rhode Island were directed toward institutional settings, a primary goal was to facilitate the state’s efforts to shift the locus of long-term care to non-institutional settings. This study draws lessons from Rhode Island’s experience with the Global Waiver for the long-term care rebalancing provisions of the Patient Protection and Affordable Care Act of 2010. The paper explores the question, “What global core competencies can public administration glean from the field of borderlands studies?” Borderlands studies have traditionally focused on the geographic area on or near the frontier demarcation between nation states. Borderlands also function as buffering or mediating zones where often markedly different countries develop complex networked administrative systems to permit the passage of peoples and products across territorial boundaries. Policy shifts over the past 20 years have created an agenda that calls for a sustained commitment to integrated employment for individuals with disabilities. But despite these clear intentions, unemployment of individuals with disabilities continues to be a major public policy issue.

For people with intellectual and developmental disabilities (IDD), the disparity in labor market participation grows. Data suggest only 14.7% of individuals who receive supports from state IDD agencies work in either individual or group integrated employment, and 19% of individuals who receive day services from a state IDD agency participate in a service designed to support integrated employment …

In FY2011, an estimated 570,406 individuals received day or employment supports from state IDD program agencies. This number grew from 458,650 in FY1999. The estimated number of individuals in integrated employment services increased from 108,296 in FY1999 to 110,295 in FY2011. State investment continues to emphasize facility-based and non-work services, rather than integrated employment services. Figure 1 shows the trends in the percentage of people served in integrated employment and facilitybased and non-work settings between FY2004 and FY2011.

To assist them with this goal, state vocational rehabilitation (VR) agencies offer employment services based upon Individualized Plans for Employment (IPEs). A commonly used measure of outcomes is the rehabilitation rate, defined as the percentage of individuals exiting the program who have achieved an employment outcome after receiving services with an IPE. Since 2009, researchers have analyzed the economic and employment impact of ARRA spending and tax policies. The current report is possible due to the efforts of Massachusetts policymakers who went beyond federal reporting requirements to require additional reporting on the number of workers who received ARRA-funded pay. ARRA workers’ demographic characteristics were also collected as part of the Massachusetts’ ARRA reporting requirements to allow for transparency and determine the fiscal policy’s impact on the state and local economy. Currently, just less than one quarter (24.5%) of Massachusetts legislators are female, putting Massachusetts behind all other New England states when it comes to the election of women to state legislative office. Vermont has the highest percentage (38.9%) of women in its legislature in the New England region.

This project develops decision tools and analytical methods to help non‐profit community development corporations (CDCs) acquire and redevelop foreclosed housing for neighborhood stabilization and revitalization. Interviews and direct observations at partner CDCs have helped us identify current practices, data and requirements for our decision models. Problem‐structuring methods through CDC focus groups have generated core operational and organizational objectives. Spreadsheet‐ and optimization based decision models generate policy and operational alternatives that address multiple resident and community outcomes. Our research will support efficient responses to the recent foreclosure crisis, especially in distressed neighborhoods, and suggest policy implications at local and national levels.
