In the low-income countries of sub-Saharan Africa, the performance of pyramidal reproductive health and family planning services with public outreach initiatives has not met the expectations or the needs of the communities they serve. Insufficient case management, limited management capacity and referral and communication failures are challenges faced on the delivery level, while on the policy level these health clinics face insufficient coordination among organizations and weak links between programs (Schneider, 2006). The Mexico City Policy, first introduced by President Reagan in 1984, only exacerbated these challenges for organizations that offer comprehensive contraception and family planning programs by denying any …

A recent report by the Muskie School and the Maine Department of Health and Human Services provides a new way of looking at the state’s long term support system. With an emphasis on developing a common approach for description and analysis across programs, the profile reveals that users of long term services and supports in Maine span all age groups and types of service users. In fact, 28% are ages 17 or younger and 29% are between the age of 35 and 64. On February 4th, 2009, President Obama signed into law the reauthorization of the Children Health Insurance Program. The federal government will now spend 30 billion dollars towards the goal of ensuring that more children in the United States are insured. I investigate this particular health care policy and look at the different dynamics that affect the ultimate goal of universal health insurance for children. For decades, health researchers have documented the links between individual and household income/wealth and clinical health outcomes. The research and literature consistently point to both income inequality (i.e. individual household income/wealth as compared to other households in the same state) and individual household income as predictors of diminished health and morbidity. And yet the current framework for providing financial education, disseminating asset building information, and overall responding to the financial health of individuals and households takes no cues from public health and community health best practices. In the United States, drug-free substance abuse treatment programs generally operate under an abstinence-only, disease model based service orientation. Citing several reasons, critics suggest disease model approaches hinder client utilization and retention within such services. The framework of harm reduction offers an alternative approach to substance abuse treatment services and may have potential for improving utilization and retention outcomes. Yet little is known about how harm reduction is perceived by practitioners in drug-free substance abuse treatment settings, and the circumstances in which harm reduction may or may not be accepted. The Lived Experience Of Hiv-Positive Tanzanian Orphans, Carol A. Parker

The purpose of this investigation was to generate a context- specific model of the lived experience of HIV positive Tanzanian orphans. With the advent of anti-retroviral drugs, Sub-Saharan African populations with high burdens of human immunodeficiency virus (HIV) face annually accumulating increases in the number of HIV positive children, and ultimately adults. Perinatally infected, these children often become orphans while still young. Extended interviews were performed with a purposive sample of 12 HIV infected orphaned children between 9 and 12 years of age. The argument that people with psychiatric disabilities cannot work is an empty one, as anecdotal and research data have shown (Bond, 2004). Recently, there has been a plethora of information on evidence based employment strategies, prominently connected with the research on the Individual Placement and Support (IPS) model associated with Drake and colleagues at Dartmouth College (Drake, 1998).

It has been known for decades that individuals with autism spectrum disorders (ASD), including those with significant impairment or who have behaviors that others find challenging, can work when they are given appropriate supports. It is also clear that individuals with ASD can benefit from employment. Benefits include improved emotional state, greater financial gain, decreased anxiety, greater self-esteem, and greater independence. Nonetheless, employment outcomes for individuals with ASD have traditionally been poor. Even those who do find work are often underemployed or do not hold onto jobs for a long period of time.

State Mental Health Agencies provide a wide range of supports to consumers including rehabilitation services, vocational and pre vocational training, and supported and competitive employment supports. This Data Note explores how states vary in number of individuals served in Community Mental Health Programs, i.e., all services not provided in an inpatient setting, who are employed as well as the percentage of individuals served in Community Mental Health Programs who are employed.

This report presents findings from a telephone survey of children currently enrolled in or recently disenrolled from MaineCare, the State‘s Medicaid and State Children‘s Health Insurance Program (SCHIP). The sample was randomly selected, and stratified to include children enrolled in MaineCare through the Medicaid eligibility category, and through two SCHIP eligibility categories, Medicaid Expansion and the Separate Child Health Program (CHP). 1 These three eligibility categories include children ages 18 or under living in households with income up to 200% of the Federal Poverty Level. Income eligibility limits are lowest for the Medicaid eligibility category, followed by the Medicaid Expansion …

Prior to January 1, 1997, individuals with drug- or alcohol-related disabilities could qualify for federal public assistance through the Supplemental Security Income (SSI) program. During the welfare reforms of the Clinton administration, this policy was changed, resulting in lost income and health care benefits for many lowincome substance abusers. This paper examines the historical underpinnings to the elimination of drug addiction and alcoholism (DA&A) as qualifjing impairments for SSI disability payments. Following this, empirical evidence is presented on the effect this policy change had on the subsequent economic security of former SSI DA&A beneficiaries. This report, prepared for the Maine Hospice Council and funded by the Carpenter Foundation, presents findings of a qualitative study of barriers to hospice utilization. This study includes the perspectives of all 26 Medicare certified hospice providers in Maine. Significant findings of the qualitative study include: A continuing need to educate the general public about hospice and the Medicare benefit. Maine health care providers have low referral rates to the hospice programs and often misunderstand the regulations and guidelines of the hospice benefit. The referral process to hospice programs is based on fragile systems of communication, fraught with potential miscues, …

Beginning in 1953, radiation exposure and its effects became a hotly contested issue between the government, members of communities surrounding the Nevada Test Site (NTS), and researchers within the national scientific community. The Atomic Energy Commission (AEC), concerned about maintaining a continental testing facility and aware that atomic testing would impact communities surrounding the NTS, sent representatives to areas potentially affected by fallout to instruct and reassure the affected public. How government officials perceived neighboring communities and how these communities perceived these representatives of the government often determined public responses to the atomic testing program. The Executive Order regarding Maine State Government as a Model Employer of people with disabilities was signed on February 24, 2006. The order included several action steps, including a survey of State employees to ascertain the prevalence of workers with disabilities in State employment.
Maine’s Bureau of Human Resources and the Commission on Disability and Employment were named to oversee this survey. The University of Southern Maine’s Muskie School of Public Service administered the survey and analyzed results. Each of these three organizations are members of an Executive Order Working Group designed to overall all action steps under the Executive …

Under Brazilian law, abortions are prohibited in all cases except for pregnancies resulting from rape or sexual violence and pregnancies which present possible death or harm to the mother. Upon a closer look, however, it is obvious that regardless of the illegality, Brazilian women are getting abortions. In light of this, I conducted a research project to uncover more about the reality that Brazilian women, specifically located in Praia Grande (a small community on Ilha de Mare off the coast of Salvador), face given the simultaneous existence of the two situations listed above (which I refer to as the “abortion …
