According to a recent report on the status of women in Massachusetts, the Commonwealth has an “extraordinarily high” incidence of women of color with HIV/AIDS. Over 4,200 women are infected and women of color account for a disproportionately high number of these cases.

Recent changes in administration of the Medicaid program make it imperative that local agencies improve their ability to forecast demand for Medicaid services. In October of 1998 the State of Michigan redesigned the Medicaid specialty care in Michigan from a fee-for-service system to a capitated system. In a capitated healthcare system, financial risk is a result of unanticipated changes in the population size and mix. Numerous demographic factors, such as crime, population, unemployment, median age, income, and ethnicity may be used to improve the accuracy of predicted changes in enrollment for Medicaid. The success of state efforts to control rising health care costs depends on the incentives contained in the legislative design of regulatory policies and in the administrative capacity and autonomy of state agencies. States have regulated the construction and expansion of health care facilities and services for more than two decades through "certificate of need" (CON) programs designed to limit the diffusion of expensive new medical technologies and to avoid the duplication of health care facilities. It examines the age, race, and geographical distribution of women and girls across the state; family structure, income and poverty; and women's labor force participation, occupational and industrial distribution in jobs, and earnings. When relevant 1990s Massachusetts data are compared to national data and to Massachusetts data from the 1970s.

This study addresses three policy issues found in the literature: How can hospitals best manage the problem of shortages of the nursing home resource, how have federal policy makers responded to the mounting shortages, and how has the unique character of nursing influenced policy.

An analysis of existing hospital policy making is presented. An analysis of the class conflict within nursing is also presented. Data have been generated from a case sample of six urban hospitals in the Pacific Northwest to assess their management of the nursing resource during the critical shortages of the late 1970s.
