On the 16th March 2010 at [International demining organisation] task CBU 478, an uncontrolled detonation of a US M series M77 sub-munition occurred while [International demining organisation] searcher [the Victim] was conducting sub-surface instrument search.

[The Victim] sustained fragmentation injuries to his right leg in the thigh and lower leg. When he stood on an unstable rock in his clearance lane, this caused a sub-munition underneath the rock to detonate.

According to the investigation report the de-miner was using the scraper direct on the top of the detected signal causing it to go off. It was then found that the exploded item was a MUV fuse. As a result of the explosion, [MUV fuze] sustained injuries on his eyes, right side ear, arm and head; the injuries indicate that the de-miner was not using his visor properly.

The Deminer today was officiated as a team leader due to the absence of the team leader. He took one deminer tools and started to work himself in one of the remaining cluster in his section. The deminer was instructed by his Section Leader to move the UXO and place the item concerned in the demolition pit, which was located 21 metres away from the deminer’s working area / lane. According to the investigation report, the item was a rifle Grenade of 40mm which was exploded as a result of irresponsible action of the victim deminer. According to the investigation report, it seems that the deminer started the excavation drill directly from the centre of the detected signal by his bayonet and caused the mine to be exploded. As per investigation report the deminer was using pickaxe instead of bayonet because the ground surface was hard. On 14 September 2009, a deminer of CBDT-06 was busy in his clearance lane, he detected an unknown device and informed his respective section leader who had 18 year demining experience to check it. On 28 July 2009 at 08:10 hrs when Ahmed de-miner was working on a signal in his clearance lane by using a pickaxe instead of bayonet, he hit the mine on its top and caused it to go off.
