Since the era of colonialism to date, Tanzania has been practicing representation—a key democratization pillar. Throughout this period, the question of who is representing who and at what cost continues to feature in political discussions in the country. MOTIVATION: Thirty states are expanding Medicaid eligibility under the Affordable Care Act (ACA) as of 2015, and while federal funds cover most costs for newly eligible recipients, states must share the additional costs of covering previously-eligible state residents who enroll in Medicaid. States, together with their local government counterparts, also provide the vast majority (87%) of public sector funds for public health programs designed to promote health and prevent disease and injury on a population-wide basis. Fiscal constraints and generous federal matching funds create strong budgetary incentives for states to channel their health-related spending to Medicaid rather than to other …

Abstract Laws that prohibit, regulate, or tax cigarettes can generate illicit markets for tobacco products. Illicit markets both reduce the efficacy of policies intended to improve public health and create harms of their own. Enforcement can reduce evasion but creates additional harms, including incarceration and violence. There is strong evidence that more enforcement in illicit drug markets can spur violence. The presence of licit substitutes, such as electronic cigarettes, has the potential to greatly reduce the size of illicit markets. We present a model demonstrating why enforcement can increase violence, show that states with higher tobacco taxes have larger illicit …

Objective: Dietary guidelines are important education and policy tools to address local nutrition concerns. The current paper presents a comparative analysis of nutrition messages from three Spanish-speaking Caribbean countries (Cuba, Puerto Rico and Dominican Republic) to explore how these dietary guidelines address common public health nutrition concerns, contextualized in different changing food environments and food culture similarities.

Design: Qualitative, comparative analysis of current dietary guideline documents and key recommendations.

Results: Key recommendations were categorized into sixteen themes (two dietbased, ten food-based and four ‘other’). Only the Cuban dietary guidelines included diet-based key recommendations. Of the ten food-based …

The foundation of preserving and enhancing global nuclear security rests on three fundamental pillars: nuclear disarmament; preventing further proliferation of nuclear weapons; and international cooperation aimed at safeguarding nuclear materials. Today, experts argue that the recent decision of Russian president Vladimir Putin to cut cooperative efforts to secure nuclear materials are placing in peril the future of international efforts to promote global nuclear security. We argue that in addition to the clear erosion of the third pillar of nuclear security, there are more threatening ramifications resulting from the recent actions of Russia in Ukraine.

Foreign civilian nuclear start-ups have an increasing number of international partners capable of supplying fuel cycle technologies. The desire to prevent the spread of dual-use enrichment and reprocessing technology by asking partner states to rely on international fuel markets is a major obstacle for US negotiating civilian nuclear trade agreements, leading to delays. US participation in emerging nuclear markets is being undercut by foreign competition, leading to decreasing economic competition and influence in international nonproliferation issues. In King v. Burwell, the U.S. Supreme Court once again saved the Affordable Care Act (ACA) by upholding subsidies (tax credits) offered to low- and middle-income individuals for insurance bought on federal exchanges. A contrary opinion would have put at risk health insurance for 6.4 million Americans and threatened to destabilize insurance markets for millions more.

Liberal democracies encourage citizen participation and protect our freedoms, yet these regimes elect politicians and decide important issues with electoral and legislative systems that are less inclusive than other arrangements. Some citizens inevitably have more influence than others. Is this a problem? Yes, because similarly just but more inclusive systems are possible. Political theorists and philosophers should be arguing for particular institutional forms, with particular geographies, consistent with justice.
