When it was introduced in the Affordable Care Act of 2010 as the new 2012 payment model for Medicare, an accountable care organization (ACO) was a new and untested concept in healthcare delivery and payment. The authors estimated the likelihood of engagement in ACOs by small group and solo healthcare practitioners. An evaluation of five case studies showed that significant organizational, financial, and technological challenges had to be met in order to launch an ACO. Sufficient resources to meet those challenges were best supplied by large organizations. Small or solo practices participated only through varying levels of integration as salaried …

In the United States, rhetoric in support of the Iraq War often focuses on discourses of patriotism and supporting the troops. These discourses hold enormous sway over the American public because of the discursive legacies of the Vietnam War and the September 11, 2001 terrorist attacks. In response, members of the peace movement who are veterans, soldiers, and military families stress their military identities during activism. These individuals have organized as an important branch of the U.S. antiwar movement that challenges the pro-war framing of patriotism and troop support by strategically deploying 'oppositional identities.' Recent events in Tunisia, Egypt, and Yemen have caught senior U.S. policymakers off guard like a herd of deer frozen in the headlights of a big rig barreling down the highway. The State Department contingency plan now appears to be to pretend to play the middle in the media—between the democratic yearnings of the mob and the longing looks of friendly despots— while privately clinging to principals of realpolitik. Calls for democratic freedoms and reforms to be implemented in Egypt, the true center of gravity for the Arab region, are being made but they are no more than hallow exaltations.

Observations of the various limitations of freeway capacity expansion have led to a provocative planning and policy question – What if we completely stop building additional freeway capacity. From a theoretical perspective, as a freeway transportation network matures, there exists a saturation point beyond which any additional freeway capacity would only be counterproductive from a welfare point of view, and worsen the existing urban transportation problems. Traditional benefit/cost analysis of individual freeway capacity expansion projects often ignores long-term induced demand and land use changes and does not represent a systems approach to this important theoretical issue. By many different measures, rural Nebraskans are positive about their community. Many rural Nebraskans rate their community as friendly, trusting and supportive. Most rural Nebraskans also say it would be difficult to leave their community. In addition, most rural Nebraskans disagree that their community is powerless to control its future.

Differences of opinion exist by the size of their community. Residents of smaller communities are more likely than residents of larger communities to rate their community favorably on its social dimensions and to have positive sentiments about their community. Microeconomic analysis of counternarcotics strategies in Afghanistan suggests that current policies lend material support to the enemy. Vigorous enforcement can increase the flow of funds to insurgents and other parties that profit from trafficking. Rural-development programs, promoted as elements of a counternarcotics strategy, are open to some of the same objections. The benefits of drug-fighting in Afghanistan for consumer countries in Europe and North America are likely to be modest. Anti-corruption efforts in Afghanistan and demand-reduction programs both in Afghanistan and in consumer countries, insofar as they are feasible, could serve both counternarcotics and counterinsurgency objectives.

Explaining The Rise Of The Left In Latin America, Luisa Blanco, Robin Grier

Latin American politics has taken a left-hand turn in the last decade, with an increasing number of chief executives hailing from left-of-center parties. We investigate the political and socio-economic factors explaining political ideology of the chief executive in a sample of 100 elections taking place between 1975 and 2007 in eighteen Latin American countries. We find that the commodity booms in agricultural, mining and oil are positively and significantly related to the probability that a country will have a chief executive from a left-of-center political party. However, for oil exports, we observe that this effect only holds for Venezuela. We …

The (Non) Effect Of Natural Resource Dependence On Capital Accumulation In Latin America, Luisa Blanco, Robin Grier

In a simultaneous model of human and physical capital accumulation for 17 Latin American countries from 1975 to 2004, we show that overall resource dependence is not significantly related to physical and human capital. Disaggregating the natural resource variable into subcategories, we find that petroleum export dependence is associated with higher physical capital and lower human capital, while agricultural export dependence is often associated with lower levels of physical capital. All of these effects are quantitatively small, however, casting doubt on the idea that natural resource dependence has stifled the accumulation of capital in the region.

The Impact Of Spatial Interdependence On Fdi In Latin America, Luisa Blanco

This analysis considers whether spatial interdependence is an important determinant of foreign direct investment (FDI) in Latin America. Two types of spatial interdependence are explored: 1) surrounding market potential and 2) spatial autocorrelation of FDI. Using a sample of 17 Latin American countries, with observations from 1986 to 2006, we find that spatial interdependence matters for world net FDI in the region. Surrounding market potential has a positive effect on FDI of significant magnitude, but there is no evidence that FDI is spatially autocorrelated.
