A Preliminary Analysis Of Beneficiary Discharge Status And Post-Hospital Placement Before And After The Implementation Of Medicare's Prospective Payment System, Anne Marguerite Wilkinson

In recognition of the inherently inflationary nature of retrospective reimbursement, the Reagan Administration enacted legislation that substantially changed Medicare's hospital reimbursement system. The Prospective Payment System (PPS) mandated paying hospitals a fixed payment, set in advance, based on the patient's diagnosis rather than retrospectively paying for all services delivered to a patient. Critics contend that PPS introduces incentives for hospitals to conserve resources during the hospital stay and to shift care to less costly settings, both potentially affecting quality of care to the elderly. A review of the literature identifies the loss of work as a stressful life event that has been linked to a number of psychosocial ills. The paper examines the loss of social relationships, a major noneconomic cost of unemployment, as a major contributor to the development of those ills. Practice implications of this finding are identified.

The end of the cold war brought with it a new period of globalization and an emerging world cultural consensus in which social welfare values of the welfare state figure importantly. An essential element of this development has been indigenization in which universal social welfare values, like those on display at various United Nations agencies and in the 1948 Universal Declaration of Human Rights are adapted and fitted to the unique demands and requirements of individual cultures.

The paper argues for the emergence of a world-wide universal pluralistic culture, in which a common core of humanitarian values will eventually be institutionalized in the major institutions of each society in ways which are consistent with the unique historical, cultural, economic and political context of that society. It is this process of adaptation of universal, or at least trans-cultural, values to the unique circumstances of individual cultures which can be called "indigenization".

Cultural competence is a set of congruent behaviors, attitudes and policies that come together in a system, agency or professional to work effectively in cross-cultural situations. The word culture is used because it implies having the capacity to function effectively. A culturally competent system of care acknowledges and incorporates - at all levels - the importance of culture, the assessment of cross-cultural relations, vigilance towards the dynamics that result from cultural differences, the expansion of cultural knowledge and the adaptation of services to meet culturally unique needs.

Recent Trends In The Economic Status Of Boston's Aged: Determinants And Policy Implications, William H. Crown

New England Journal of Public Policy

The economic status of the older population has improved significantly since the early 1970s. Yet poverty rates among certain groups of elderly, especially older minorities, have declined very little. To understand the reasons for these seemingly contradictory trends, changes in the income composition of the elderly in Boston are compared to changes in income for the elderly in the United States. This analysis suggests that low-income older persons were largely bypassed by one of the major factors in income growth among the older population — growth in pension income.

Twenty-four parents and professionals from around the country recently attended a training session in Portland, Oregon designed to promote and improve parent/professional collabora tion to better serve children with emotional handicaps. The overall training goal was to provide each of the parent/professional teams with materials and skills they could take back to their regions and share through local workshops. The training workshop was held October 5-9, 1987 on the campus of Portland State University, and was hosted by the Research and Training Center to Improve Services for Seriously Emotionally Handicapped Children and The ir Families.

Adolescent shoplifting has provoked limited and somewhat controversial perspectives within the sociological and psychological literature. These controversies center around the empirical variables used for analysis. A companion argument focuses on the subjective and objective measurement of these variables. This research explicated variables from the sociological literature to test their relationship, using multiple linear regression, to adolescent shoplifting behavior. These variables and situational stimuli were operationalized in a simultaneous model to demonstrate a proximate occurrence of the attitude-situation-behavior reciprocal. This reciprocal is a learning theory which suggests that direct and vicarious experiences accompanied by rewards and punishment, in one's environment, lead …

Adolescents with emotional handicaps experience multiple challenges as they leave youth serving systems and attempt to find their place in society. Without adequate preparation or established linkages to the adult system to facilitate this transition, they are bound to become lost until their symptoms dictate intervention. Service delivery efforts must be sensitive to these issues and begin to realistically address the transition needs of this population. A framework which can be used as a guide for developing transition oriented programs and nine integrative principles will be discussed hear as a means to stimulate thinking about the provision of transition services.

This report presents a list of current state legislative initiatives which directly or indirectly, affect older women in Massachusetts.

Despite the progress made by the Commonwealth in the area of elder services, a significant portion of this population continues to live in dire need. It is important that State Legislators, State Administrators, policymakers and community leaders understand the needs of this population and work to increase the financial resources and enhance the dignity of older women in the Commonwealth.

Therapeutic Case Advocacy (TCA) is one approach to helping emotionally handicapped children and their families. It is described here as a multi-level model to improve services for this population for two reasons. First, the approach typically requires planned, coordinated activity on three levels: the child and family, the lead organization providing services, and interagency collaboration. Second, the word "model" conveys the idea of what the real thing might look like.

When the Old Age Assistance (OAA), Aid to the Blind (AB), and Aid to the Permanently and Totally Disabled (APTD) caseloads were converted to SSI in 1974, only cash payments were transferred to the Social Security Administration; social services remained available through public welfare agencies. This paper assesses the extent to which the service utilization of low income aged and disabled persons changed with the social service delivery system changes associated with the implementation of SSI. Since early 1981, there has been a large-scale removal of persons from the SSI and Social Security disability programs as a result of the Social Security Disability Amendments of 1980. This article reports on the findings of a national survey designed to determine whether the removal of persons from the federal disability programs had an impact on state and local General Assistance programs and the extent to which older recipients of General Assistance are in need of long-term income assistance for health and other reasons.

The purpose of this paper is to discuss why I think the Reagan administration's avowed commitment to helping only "actual victims" of racial discrimination retards rather than advances the cause of civil rights. I make reference in my title to "seniority" and "the shadow of Stotts" because the current administration is relying upon Supreme Court decisions having to do with seniority, particularly its 1984 opinion in Memphis Firefighters v. Stotts, to justify a wholesale attack upon race-conscious remedies, not only in employment but in education and public contracting as well.

This preface was prepared, along with several new or rewritten chapters and additional changes for a possible revised edition of the book. The publisher decided not to do a second edition and the original first edition continued to sell in its small market niche for 25 years.

This paper is an effort to propose a moderately optimistic future for the personal care sector of human services. It is proposed that the best available future scenario for personal care services between now and the year 2019 is to concentrate on privatization of service delivery on a small-scale basis. Government, in this model should be limited largely to three roles: 1) source of venture capital; 2) regulation of service delivery; and 3) income maintenance for the poorest populations.
