Trauma-Focused Interventions: A Clinical Practice Analysis, Jose Carbajal

This paper presents a systematic review of trauma-treatment interventions, which have different theoretical perspectives on trauma etiology and its application. The empirical findings of these trauma treatment therapies are presented. The extant literature identifies five therapies as the most-effective treatment modalities for PTSD: cognitive therapy (CT), cognitive-behavioral therapy (CBT), cognitive processing therapy (CPT), prolonged exposure (PE) therapy, and eye movement desensitization and reprocessing (EMDR) therapy. They are all validated with some nuances on treatment approach and effectiveness. A Single Case Research Design (SCRD) with a multiple-baseline across participants was used to investigate the effects Spiritually Oriented Cognitive Processing Therapy-Cognitive (SOCPT-C) had on spiritual struggle, posttraumatic stress disorder (PTSD) scores and depression in four Christian sexual assault survivors with PTSD reporting spiritual struggle. A complex reciprocal relationship between spiritual struggle and PTSD is suggested in the literature as influencing posttraumatic adjustment and treatment for Christian trauma survivors. Many empirically supported treatments (EST) for PTSD lack spiritual interventions to directly target effects from this relationship. Despite a significant amount of literature on genograms in the ERIC, PsycINFO, and SocINDEX with full-text databases, only a handful of articles and book chapters address using genograms with African Americans. Of the articles that were found on the clinical application of the genogram as a counseling technique with African American couples, three provided a specific set of interview questions for the construction of a genogram, but did not explore the effects of transgenerational trauma and racism on these couples. The following is an exploratory model to understand the factors that promote growth after traumatic events, also known as post-traumatic growth (PTG). Additionally, the hope is to inspire further research and create quantitative data regarding the effectiveness of these factors, add additional interventions and integrate into crisis/trauma paradigms during recovery. Military Mental Health Stigma (MMHS) is attributed as a significant factor in the 20 suicides that occur each day by United States Military Veterans (USMVs) (Kemp & Bossarte, 2012). Although experiences of trauma are common, reactions vary due to a host of biopsychosocial and cultural factors that influence the individual reaction to the trauma (Nakai et al., 2015). One such factor is resiliency, the capability to adapt in adverse environmental circumstances (Basim & Cetin, 2011). This study used hierarchical multiple regression to examine the relationships between childhood trauma, recent experiences of depression, and resilience in adult university students. This study also examined the possible moderating effects on depression by resilience. Spiritual struggle has been described as a disruption in religious practice and spirituality resulting in questioning beliefs, experiencing discord within religious communities, decreasing spiritual practices, and experiencing painful cognitions such as the belief that one is being punished by God. This study used constructivist grounded theory to explore how women identifying as Protestant Christians at the time of the traumatic event resolve their spiritual struggles. The past 20 years have been turbulent regarding Reactive Attachment Disorder (RAD), with conflicting research about its causes, effects, treatment, and prognosis. The current diagnostic criteria in the DSM-5 fails to adequately address this disorder. A number of deviant and maladaptive behaviors common amongst children with RAD are not even mentioned in the diagnostic criteria. As such, the diagnostic definition is almost unidentifiable or incompatible with real-life conduct manifestations of the disorder. Walking On Eggshells: The Lived Experience Of Partners Of Veterans With Ptsd, Tiffany A. Beks

This phenomenological study examined the descriptions of lived experience among female partners of veteran men with combat-related posttraumatic stress disorder (PTSD) via internet discussion forums. Personal, self-initiated written accounts of 30 partners were analyzed with respect to meaning, challenges, coping responses, and role in veterans’ healing and rehabilitation. Following data analysis, five descriptive themes emerged: all-consuming effect of the illness, walking on eggshells, ambiguous loss, alone, and facing PTSD as a unit. The central meaning of these themes describes the widespread priority of the veterans’ illness, and the resulting isolation, grief, and apprehension experienced by intimate partners as they assume …

How Therapists Use And Choose Mindfulness To Treat Trauma, Jessica M. King

This qualitative study used the phenomenological method to examine how therapists use mindfulness therapies and interventions to address trauma-salient issues with their clients. Specifically, it explored therapists’ use of and choices about mindfulness-based treatments when addressing post-traumatic stress symptoms, and trauma-relevant emotion dysregulation and attachment injury. Informants were associate and fully-licensed local therapists, recruited using convenience sampling and snowball sampling by word-of-mouth referrals. Data was collected by semi-structured interviewing. Interview data was analyzed with Moustakas’ (1994) recommended procedures for analysis of phenomenological data. Results, Discussion, Limitations and Suggestions for Future Research are described at the end.

This article briefly reflects on the HIV/AIDS epidemic in the African American community. The article suggests an alternative paradigm that views the high-risk behaviors that contribute to the spread of the virus as a reaction to ‘trauma of the heart’ as opposed to viewing the epidemic solely in terms of a lack of HIV and sexually transmitted disease awareness. It further suggests a pastoral intervention of expanding the paradigm of family beyond the classical nuclear construct to family as a system of individuals committed to embracing and ‘adopting’ individuals who have experienced trauma of the heart. Using narrative theory, the chapter examines how ideas and attitudes about mental disorder are shaped by cultural values and stereotypes, and how the experience of trauma can shatter the narrative of self and world. Placing the question of illness, differentness, and personhood within a social justice perspective, it challenges clinicians to consider how the vocabulary of illness is used to frame experience and, in many cases, to minimize, marginalize, or discount the individual's own lived experience.

Immediately following the drop of the Little Boy atomic bomb, the city of Hiroshima was decimated, leaving the surviving citizens to deal with poverty, starvation, loss of loved ones, and utter destruction of their lives. After the bombing, survivors were left with burns, radiation poisoning, and physical scars. The purpose of the study was to investigate the effectiveness of a personalized school-wide crisis and trauma management training program for master's-level school counselors-in-training. The study began with Pilot Study 1: Crisis Training Need Assessment which sought to best identify the crisis training needs for a specific geographic region. Using grounded theory analysis, the current study identifies the perspectives of therapists and counselors regarding the nature of informal and formal resources supporting the treatment of deaf individuals with trauma. Nineteen counselors and therapists were interviewed, and accessibility, formal support, informal networks, and gaps in resources were identified as salient themes. Subsequent analysis identified concerns with confidentiality as a strong theme. This study emphasizes the need for counselors and therapists to become mindful of utilizing resources to support the treatment of trauma while being concerned with confidentiality.
