Medicare Advantage's Private Fee-For-Service Plans: Paying For Coordinated Care Without The Coordination, Brian Biles, Emily Adrion, Stuart Guterman

Like the private managed care plans offered under Medicare Advantage, private fee-for-service (PFFS) plans are paid more per beneficiary than those individuals would be expected to cost if they were enrolled in traditional fee-for-service Medicare. However, PFFS plans are not required to provide the same type of coordinated care required of Medicare Advantage plans. Payments to PFFS plans in 2008 average 16.6 percent more than costs in traditional Medicare, or $1,248 for each of the 2 million enrollees in PFFS plans—a total of nearly $2.5 billion in extra payments. Accurate measurement of digital divides is important for policy purposes. Empirical studies on broadband subscription gaps have largely used cross-sectional data, which cannot speak to the timing of technological adoption. Yet, the dynamics of a digital divide are important and deserve study. With the goal of improving our understanding of appropriate techniques for analyzing digital divides, we review competing econometric methodology and propose the use of duration analysis. We compare the performance of alternative estimation methods using a large dataset on DSL subscription in the U.S., paying particular attention to whether women, blacks, and Hispanics catch up to others in …

As the U.S. economy falters and recession looms, 79 percent of jobs in New Hampshire do not pay a wage sufficient for single-parent families with two children to provide basic needs such as housing, food, transportation, child care, and health care. Carroll County has the lowest percentage of livable wage jobs, with only 13 percent of jobs paying a livable wage for single-parent families with two children.

The impact of school factors on academic achievement has become an important focus for school social work and revealed the need for a comprehensive school social work model that allows for the identification of critical areas to apply social work services. This study was designed to develop and test a more comprehensive school social work model. Specifically, the relationship between cumulative grade point average (GPA) and the cumulative risk index (CRI) and an additive risk index (ARI) were tested and a comparison of the two models was presented. Improvements in the welfare of animals arise from two primary sources: Changes in public attitudes and beliefs regarding what constitutes acceptable treatment of animals, and mandatory changes in behavior toward animals imposed by governments and regulatory authorities. During the last 30-40 years, public and political support for improvements in animal welfare has increased dramatically in many countries. Funding from both governmental and non-governmental sources continues to grow, and the relatively new field of animal welfare science has emerged and expanded to support and inform these efforts. This paper describes ongoing research and initial results regarding collection of airborne imagery for detection of submerged features in optically shallow water types. The approach is to utilize land surface, water surface and submerged or in-situ line targets of various types during the airborne flights or fixed platforms. The imagery collected by the airborne sensing systems is processed in concept to correct for the presence of the water surface wave effects and water column effects. The correction approach is based upon calculation of a residual image between either (a) the observed submerged target image and the non-submerged image or (2) …

Conflicting and complex forces are shaping the diffusion patterns of the Internet and e-commerce in South Asia. Drawing upon the literature on institutional theory, we explore the drivers and inhibitors of the Internet in South Asian countries. We examine the influence of the three pillars of institutions (Scott, 1995) on the digital world of South Asia. The paper discusses how regulatory, normative, and cognitive institution–such as laws, relationships, culture, and habit–have shaped the diffusion patterns of the Internet and e-commerce in South Asia.
