Standardizing Markers To Evaluate And Compare Their Performances, Margaret S. Pepe, Gary M. Longton

Introduction: Markers that purport to distinguish subjects with a condition from those without a condition must be evaluated rigorously for their classification accuracy. A single approach to statistically evaluating and comparing markers is not yet established.

Methods: We suggest a standardization that uses the marker distribution in unaffected subjects as a reference. For an affected subject with marker value Y, the standardized placement value is the proportion of unaffected subjects with marker values that exceed Y.

Results: We apply the standardization to two illustrative datasets. In patients with pancreatic cancer placement values calculated for the CA 19-9 marker are smaller …

No single biomarker for cancer is considered adequately sensitive and specific for cancer screening. It is expected that the results of multiple markers will need to be combined in order to yield adequately accurate classification. Typically the objective function that is optimized for combining markers is the likelihood function. In this paper we consider an alternative objective function -- the area under the empirical receiver operating characteristic curve (AUC). We note that it yields consistent estimates of parameters in a generalized linear model for the risk score but does not require specifying the link function. Like logistic regression it yields …

The purpose of this research was to determine a prevalence of degenerative joint disease in our pre-orthodontic adolescent patients age ten through seventeen. The prevalence was assessed according to gender. Severity of DJD was established for each patient and a "red flag" for diagnosis was identified. The purpose was also to establish norms for joint space for patients with and without degenerative joint disease. Correlations to distal condyle position and angle classification were described. 123 consecutive patients, 77 female and 46 male T1 NewTom records were reviewed. The present pilot study is a histological evaluation of mandibular condyles taken from minipigs that underwent either traditional mandibular advancement (bilateral sagittal split osteotomy), or a distraction osteogenesis procedure to advance the mandible. One pig received a bilateral sagittal split osteotomy with an immediate mandibular advancement of 14 mm, while another pig underwent distraction osteogenesis consisting of a gradual advancement of 2 mm per day to advance the mandible the same distance. The control animal received no surgery.

We compare simple logistic regression with an alternative robust procedure for constructing linear predictors to be used for the two state classification task. Theoritical advantages of the robust procedure over logistic regression are: (i) although it assumes a generalized linear model for the dichotomous outcome variable, it does not require specification of the link function; (ii) it accommodates case-control designs even when the model is not logistic; and (iii) it yields sensible results even when the generalized linear model assumption fails to hold. Selecting the best treatment for a patient's disease may be facilitated by evaluating clinical characteristics or biomarker measurements at diagnosis. We consider how to evaluate the potential of such measurements to impact on treatment selection algorithms. For example, magnetic resonance neurographic imaging is potentially useful for deciding whether a patient should be treated surgically for carpal tunnel syndrome or if he/she should receive less invasive conservative therapy. Laser etching of enamel surfaces alters the physical and chemical characteristics of the enamel. These changes in characteristics enhance the bonding to enamel. The purpose of this study was to compare the shear and tensile bond strength of Er,Cr:YSGG hydrokinetic laser system (Biolase) with 37% phosphoric acid and self etching primer 30 minutes and 72 hours after bonding. Four different laser power output setting was observed: 1.5W, 2.0W, 2.5W, and 3.0W. The purpose of this research was to compare the tensile bond strength of orthodontic flat based buttons bonded to bovine teeth with four commercial light-emitting diode (LED) curing lights and one conventional quartz tungsten halogen (QTH) curing light.

The radiodensity of a composite resin can greatly influence a clinician's ability to analyze a restoration for failure. The purpose of this study was to determine the relative radiopacities of 29 resin restorative materials compared to enamel, and to verify the comparable radiopacity between shades of 23 of the restorative composite resin materials. Composite resin discs measuring >5mm in diameter and 2 mm in thickness were made and polymerized. The purpose of this study was to quantitatively evaluate; 1) mandibular posterior mono-cortical block onlay bone graft success rates 2) the amount of grafted bone retained after initial resorption in radiographic projections. Seven consecutive patients with 14 mandibular posterior implant sites requiring bone augmentation for implants of a standard 3.8mm x 10mm dimension were sought. Mono-cortical block bone grafts from the ipsilateral ascending ramus were placed then observed with two standardized CT scans, dividing them into two groups: Group A had the first CT at 4 weeks after graft placement and a second taken before Prosthodontic reconstruction, 36.7 weeks later. …

High throughput technologies, such as gene expression arrays and protein mass spectrometry, allow one to simultaneously evaluate thousands of potential biomarkers that distinguish different tissue types. Of particular interest here is cancer versus normal organ tissues. We consider statistical methods to rank genes (or proteins) in regards to differential expression between tissues. Various statistical measures are considered and we argue that two measures related to the Receiver Operating Characteristic Curve are particularly suitable for this purpose. The idea of using measurements such as biomarkers, clinical data, or molecular biology assays for classification and prediction is popular in modern medicine. The scientific evaluation of such measures includes assessing the accuracy with which they predict the outcome of interest. Interleukin-10 (IL-10) is a cytokine with anti-inflammatory activities. IL-10 knockout (KO) mice exhibit severe intestinal inflammation in the presence of normal gut flora. We hypothesized that IL-10 KO mice would also be susceptible to inflammatory periodontal disease resulting in alveolar bone loss (ABL). The purpose of this study was to compare naturally occurring ABL in IL-10 KO and wild type (WT) mice. Twelve IL-10 KO and 12 WT mice, age-(7 months) and sex-(male) matched, were used. Through microarray technology it is now possible to measure the number of copies of thousands of genes and gene segments in samples of chromosomal DNA. Background. Typically, infant mortality decreases as maternal age increases in teenagers. In contrast to these national data, San Bernardino County (SBC) experienced a different phenomenon in 1989-1994 among very low birthweight (VLBW) infants. The infant mortality rate (IMR) of VLBW infants born to mothers under 17 years old (283 per 1000 live births) was significantly lower than the IMR of VLBW infants born to mothers between the ages of 17-19 years old (430 per 1000 live births) (p = .02).

This article summarizes current issues in drug abuse prevention research through integration of other articles in this journal and by heeding historical trends in prevention science. Recommendations are made for future research directions. The purpose of this study is to evaluate skeletal and dental asymmetry in unilateral class II malocclusion. A study group of 25 unilateral subdivision patients was compared to 25 class I malocclusion patients serving as the control group. Submento-vertical and frontal radiographs were used for the evaluation. Symmetry was evaluated by comparing measurements between right and left sides for both groups and matching the middle points with the midsagittal axis. Statistical paired and simple t-test were used to evaluate the difference between the two sides for both groups. This study evaluated candidate carriers for recombinant human bone morphogenic protein-2 (rhBMP-2) in periodontal reconstructive surgery.

Canine demineralized bone matrix (DBM), bovine inorganic crystallene bone matrix (Bio-Oss), bovine derived microfibrillar collagen matrix (Helistat), poly (D,L-lactide-coglycolide) micropaiticles (PLGA), and polylactic acid granules (Drilac) were used with rhBMP-2 (20 (µg/100 µl implant volume) in routine critical size canine supraalveolar periodontal defects. Contralateral defects in six beagle dogs were randomly assigned to receive: DBM/rhBMP-2, DBM-control, Bio-Oss/rhBMP-2, Helistat/rhBMP-2, PLGA/rhBMP-2, or Drilac/rhBMP-2, all with autologous blood except for Helistat and PLGA. A few studies have identified etiologic factors in pulpal disease, but little is known about the odds ratios of factors associated with pulpal disease. Odds ratios are well established measures of the effects of exposure to a particular outcome, and are comparisons of exposure to non-exposure in cases versus controls. The purpose of this study was to compare exposure of individual teeth to hypothesized risk factors in endodontic cases and in controls. Pre-operative (T-1) and post-operative (T-3) radiographs were obtained and traced for fifty-one non-growing patients who had undergone isolated Le Fort I osteotomies. These tracings were then digitized with the results arranged in a data file and subjected to statistical analysis. The purpose of this study was to establish ratios for certain hard to soft tissue movements that could be used to aid the clinician in post-surgical soft tissue profile prediction. Radiation due to x-ray exposure is a concern, and the development of the Dolphin™ DigiGraph represents an attempt to limit this exposure. The purpose of this paper was to compare measurements (using Ricketts analysis) obtained from conventional radiographic cephalometric tracings with those obtained by computerized digitizing with the DigiGraph. Forty-six patients from the Loma Linda University School of Dentistry graduate orthodontic clinic were digitized, and the results were compared to the x-ray tracings of these patients. The author undertook to develop an extraoral model for predicting mandibular incisor relapse similar to the model described by Harvey and Sheldon Peck. Whereas the Peck and Peck method involves intraoral measurements, the method described in this paper relied on the taking of measurements from the patient's pre-treatment plaster models. Two hundred and sixty teeth were measured both orally and on plaster casts, and several statistical tests were performed to isolate any differences between the intraoral and extraoral methods. The present investigation utilized the Bolton Analysis to determine the incidence and magnitude of tooth-size discrepancy in a sample of 227 orthodontic patients. Sliding calipers were used to measure the mesiodistal width of the permanent dentition on pretreatment study models of 120 Caucasian, 51 Black, and 56 Hispanic patients. The overall and anterior ratios were analyzed and the frequency and magnitude of excess maxillary and mandibular tooth structure was determined. Each racial group demonstrated a significant incidence of tooth-size discrepancy in the maxillary and mandibular arches of a magnitude (> 0.5mm) which would require some form of correction. Endodontic (root canal) infections are generally considered to be caused by the invasion of microorganisms which destroy pulp tissue and extend to the periapical region. No definitive study has identified a specific bacterium or group of bacteria as etiological agents. We quantitively measured antibodies present in the serum of 30 normal individuals and patients with either chronic (n = 30) or acute (n = 30) periapical lesions with the indirect fluorescent antibody (IFA) and enzyme-linked immunosorbent assay (ELISA) for IgG and IgM against 11 oral microorganisms routinely isolated from such lesions.
