Toxoplasma gondii is a parasitic infection that can be transmitted in utero, resulting in fetal chorioretinitis and other long-term neurological outcomes. If diagnosed early, pregnancysafe chemotherapeutics can prevent vertical transmission. Unfortunately, diagnosis of acute, primary infection among pregnant women remains neglected, particularly in low-andmiddle-income countries. Clinically actionable diagnosis is complex due to the commonality of infection during childhood and early adulthood which spawn long-last antibody titers and historically unreliable direct molecular diagnostics. Background

The COVID-19 pandemic has significantly affected maternal care services especially for minoritized individuals, creating challenges for both service users (i.e., African American and Hispanic pregnant/postpartum women) and maternal care providers (MCPs). Guided by a socioecological framework, this study aims to investigate the experiences of African American and Hispanic pregnant and postpartum women, as well as MCPs, in accessing and providing maternal care services during the COVID-19 pandemic in the Deep South.

Methods

This study compared marginal and conditional modeling approaches for identifying individual, park and neighborhood park use predictors. Data were derived from the ParkIndex study, which occurred in 128 block groups in Brooklyn (New York), Seattle (Washington), Raleigh (North Carolina), and Greenville (South Carolina). Background

Cognitive impairment has multiple risk factors spanning several domains, but few studies have evaluated risk factor clusters. We aimed to identify naturally occurring clusters of risk factors of poor cognition among middle-aged and older adults and evaluate associations between measures of cognition and these risk factor clusters.

Methods

We used data from the National Health and Nutrition Examination Survey (NHANES) III (training dataset, n = 4074) and the NHANES 2011–2014 (validation dataset, n = 2510). Background

Sleep problems are associated with abnormal cardiovascular biomarkers and an increased risk of cardiovascular diseases (CVDs). However, studies investigating associations between sleep problems and CVD biomarkers have reported conflicting findings. This study examined the associations between sleep problems and CVD biomarkers in the United States.

Methods

Data were from the National Health and Nutrition Examination Survey (NHANES) (2007–2018) and analyses were restricted to adults ≥ 20 years (n = 23,749). The purpose of this study was to evaluate the reliability and validity of the raw accelerometry output from research-grade and consumer wearable devices compared to accelerations produced by a mechanical shaker table. Raw accelerometry data from a total of 40 devices (i.e., n = 10 ActiGraph wGT3X-BT, n = 10 Apple Watch Series 7, n = 10 Garmin Vivoactive 4S, and n = 10 Fitbit Sense) were compared to reference accelerations produced by an orbital shaker table at speeds ranging from 0.6 Hz (4.4 milligravity-mg) to 3.2 Hz (124.7mg). The advancements in high-throughput technologies provide exciting opportunities to obtain multi-omics data from the same individuals in a biomedical study, and joint analyses of data from multiple sources offer many benefits. However, the occurrence of missing values is an inevitable issue in multi-omics data because measurements such as mRNA gene expression levels often require invasive tissue sampling from patients. A wealth of gene expression data generated by high-throughput techniques provides exciting opportunities for studying gene-gene interactions systematically. Gene-gene interactions in a biological system are tightly regulated and are often highly dynamic. The interactions can change flexibly under various internal cellular signals or external stimuli. Cancer may be associated with objective cognitive decline. However, studies examining the link between cancer and subjective cognitive decline (SCD), which occurs earlier during cognitive trajectories, are lacking. In addition, research assessing risk factors for SCD among cancer survivors is scant. Policy Points The White House Blueprint for Addressing the Maternal Health Crisis report released in June 2022 highlighted the need to enhance equitable access to maternity care. Nationwide hospital maternity unit closures have worsened the maternal health crisis in underserved communities, leaving many birthing people with few options and with long travel times to reach essential care. Ensuring equitable access to maternity care requires addressing travel burdens to care and inadequate digital access. Background

Obesity disparities in the United States are well documented, but the limited body of research suggests that geographic factors may alter the magnitude of these disparities. A growing body of evidence has identified a “rural mortality penalty” where morbidity and mortality rates are higher in rural than urban areas, even after controlling for other factors. Black-White differences in health and mortality are more pronounced in rural areas than in urban areas.

This dissertation focuses on theory and application of discrete data methods, particularly approaches to over- and underdispersion relative to the Poisson distribution and an application of random forest and logistic regression modeling. The first chapter derives a score test for over- and underdispersion in the heaped generalized Poisson distribution. Equi-, over-, and underdispersed heaped generalized Poisson and heaped negative binomial data are simulated to evaluate the performance of the score test by comparing the power it achieves to that of Wald and likelihood ratio tests. The recent emergence of single cell sequencing (SCS) technology has provided us with single-cell DNA or RNA sequencing (scDNA/RNA-seq) information to investigate cellular evolutionary relationships. Despite many analysis methods have been developed to infer intra-tumor genetic heterogeneity, cluster cellular subclones, detect genetic mutations, and investigate spatially variable (SV) genes, exploring SCS data remains statistically challenging due to its noisy nature.

Scan statistics are useful methods for detecting spatial clustering. While they were initially developed to detect regions with an excess of binomial or Poisson events, spatial scan statistics have been extended to detect hotspots in other types of data including continuous data. They have many applications in different fields such as epidemiology (e.g. detecting disease outbreaks), sociology (e.g. detecting crime hotspots), and environmental health (e.g. detecting high-pollution areas). BACKGROUND: The Framingham 10-year cardiovascular disease risk score, which is based on age, sex, smoking, total cholesterol, high-density lipoprotein-cholesterol, blood pressure, and diabetes, has been found to be associated with cognitive health, but these findings have not been validated in a representative sample in the United States. We aimed to examine the associations of Framingham risk score with cognitive function among older adults in a nationally representative sample, as well as by race or ethnicity, education, and family income.

Addressing the psychosocial concerns of Black men who have sex with men (MSM), such as HIV disclosure, is critical for effective HIV treatment efforts. Black MSM living with HIV experience multiple psychosocial challenges, such as “triple stigma” due to their sexual orientation, racial minority status, and HIV status, which hinder their HIV disclosure and subsequent HIV care-seeking behavior. Importance: In an ideal regionalized system, all infants born very preterm would be delivered at a large tertiary hospital capable of providing all necessary care. Objective: To examine whether the distribution of extremely preterm births changed between 2009 and 2020 based on neonatal intensive care resources at the delivery hospital. Design, Setting, and Participants: This retrospective cohort study was conducted at 822 Vermont Oxford Network (VON) centers in the US between 2009 and 2020. This dissertation explores different methods to study the dependence structure among many ordinal variables under the Bayesian framework.

Chapter 1 introduces ordinal data analysis methods, and the related literature works are briefly reviewed. An outline of the dissertation is put forward.

In Chapter 2, Gaussian copula graphical models with different priors of graphical Lasso, adaptive graphical Lasso, and spike-and-slab Lasso on the precision matrix are assessed and compared. The proposed models are well illustrated via simulations and a real ordinal survey data analysis.

In this dissertation, we focus on studying three mixture cure models with background mortality. With the development of treatment, patients may be cured and suffer from other cause of death. The cure model with background mortality can measure the population cure which refers to the patients with comparable mortality with their counterpart in general population. Generalized linear models which include spatially varying coefficient terms allow researchers to determine if the association between predictor and outcome variables vary across geographic space. Such models are particularly applicable to research with public health data where interventions and limited health care resources must be allocated carefully. Variable selection methods in both the frequentist and Bayesian frameworks are powerful techniques that provide prediction and inference in high-dimensional linear regression models. These methods often assume independence between observations and normally distributed errors with the same variance. In practice, these two assumptions are often violated. To mitigate this, we develop efficient and powerful Bayesian approaches for linear mixed modeling and heteroscedastic linear regression. These method offers increased flexibility through the development of empirical Bayes estimators for hyperparameters, with computationally efficient estimation through the Expectation Conditional-Minimization (ECM) algorithm. Wernicke’s Encephalopathy is a consistently underrecognized neurodegenerative brain disorder resulting from prolonged thiamine deficiency. Clinical presentation of the disease results from brain lesions attributable to thiamine deficiency. Because these lesions occur in various locations in the cerebral cortex, symptoms can vary significantly. Varied presentation of symptoms, in addition to the lack of a widely accepted biomarker for the disorder cause challenges to clinicians when identifying and diagnosing the disorder. Importance Chlorhexidine mouthwash enhances treatment effects of conventional periodontal treatment, but data on chlorhexidine as a source of heterogeneity in meta-analyses assessing the treatment of maternal periodontitis in association with birth outcomes are lacking.

The COVID-19 pandemic has strained healthcare systems in many parts of the United States. During the early months of the pandemic, there was substantial uncertainty about whether the large number of COVID-19 patients requiring hospitalization would exceed healthcare system capacity.
