Ultrasound-Guided Percutaneous Needle Barbotage And Ultrasonic Tenotomy For Rotator Cuff Calcific Tendinopathy, Toru Omodani
Rehabilitation Practice and Science
Rotator cuff calcific tendinopathy (RCCT) is a common source of shoulder pain caused by calcium deposits within the rotator cuff tendons. Ultrasound-guided interventions have emerged as effective, minimally invasive alternatives to surgery for symptomatic cases unresponsive to conservative treatments. This review summarizes two primary techniques: percutaneous needle barbotage and percutaneous ultrasonic tenotomy. In needle barbotage, one or two needles are inserted under ultrasound guidance to inject and aspirate saline, thereby fragmenting and removing the calcific deposit. This method yields rapid pain relief and improved shoulder function, with follow-up imaging often demonstrating near-complete deposit resolution. For particularly hard or recalcitrant deposits, …

Dry needling is an intervention that physical therapists use to address various neuromusculoskeletal pain syndromes, primarily through targeting myofascial trigger points (TrPs) in muscle tissue. This technique involves inserting thin filiform needles into specific areas to relieve pain and improve movement. Despite its therapeutic potential, concerns regarding adverse events (AEs) necessitate stringent safety protocols and guidelines. In response, the International Dry Needling Education & Training Advisory Group (IDNETAG) was established in 2023 with members from diverse professions across eight countries. Rotator cuff (RC) tears are a common cause of shoulder pain in the elderly, though tears at the musculotendinous junction (MTJ) are uncommonly observed. We present a rare case of bilateral supraspinatus tendon tears in a 68-year-old male, with unilateral involvement of the MTJ. Symptoms developed three months after resuming resistance training following left acromioclavicular dislocation surgery. Ultrasound revealed a full-thickness insertional tear on the left and a partial MTJ tear on the right. Conservative treatment led to symptom resolution without surgery. Students with disabilities face myriad barriers and hurdles to success in higher education settings. Institutions in developing nations often lack the necessary resources to provide accessible instruction, and the absence of clearly defined policies further impedes upholding the educational rights of such a special population. The purpose of this study was to examine how undergraduate students with visual impairments in Bangladesh felt about their experiences related to social justice and challenges in learning during the COVID-19 pandemic. Functional power-based exercise training can improve physical performance in older adults and cognitive training can improve measures of cognition, but their combined effects on cognition and related risk factors (neurological and inflammatory markers) remains uncertain. This 6-month cluster randomised controlled trial evaluated the effectiveness of dual-task functional power training (DT-FPT) on cognition and circulating neurological and inflammatory markers in older adults at increased falls risk, and whether intervention responses varied by apolipoprotein-E (ApoE) and brain derived neurotrophic factor (BDNF) polymorphisms. Senior-level executive leaders’ work in a dynamic, competitive, 24/7 business environment can be physically rigorous, mentally demanding, and emotionally isolating. The same holds true for women pursuing senior-level leadership roles in corporate America. Women who achieve and succeed in such roles remain the minority: Outsiders in elite spaces who are held to higher standards and overly scrutinized. Among senior-level executive women leaders, those committed to a fitness and training regimen including weightlifting constitute an even smaller minority. Moral injury is increasingly recognized as a distinct and debilitating consequence of military service, arising when individuals engage in, witness, or are unable to prevent actions that transgress their deeply held moral beliefs. In this dissertation, I argue that this form of trauma can be experienced as an attachment rupture, particularly when the military—functioning as an institutional attachment figure—betrays the servicemember’s trust. Such experiences compromise internal working models (IWMs) of self, others, and the world, and may result in complex trauma symptoms, including dissociation, depersonalization, and profound relational disconnection. Purpose: The purpose of this study was to examine proximal tibial IMU accelerations between the involved limb and uninvolved limbs of individuals who have undergone ACL reconstruction performing the single-leg hop test battery and to correlate these results to deficits in quadriceps strength and distance hopped. Methods: Thirty-four subjects (19 male, 15 female; age = 20.5 ± 6.7 years; mass = 76.4 ± 15.8 kg; height = 1.74 ± .08 cm) who have undergone unilateral ACL reconstruction were recruited for this study. Importance: Community mobility (CM) enables access to locations outside the home, allowing participation in work, leisure, and social environments. Autistic adults have reported less access to public transportation and driving than the general population (Wilson et al., 2021), impacting their community participation, life satisfaction, and likelihood of employment and further education (Egilson et al., 2016, Song et al., 2022, Wilson et al., 2021). Purpose: Stroke is a leading cause of long-term disability with upper limb hemiplegia, significantly limiting participation in daily life (Lee & Howe, 2024; Long et al., 2020). Little research has been conducted investigating the consultative role of occupational therapy (OT) in childcare, especially their role in supporting the social and emotional development of children aged 1 – 5 years. Social-emotional competence is a prerequisite to school performance and participation in daily tasks. Children in West Virginia face social and emotional barriers including the effects of the opioid epidemic leading to children placed in foster care, born with neonatal abstinence syndrome, and being exposed to toxic stress in the home environment. Additionally, the COVID-19 pandemic has impacted children's social-emotional functioning due to isolation periods. Extended hospitalization in the postpartum period is occasionally medically indicated due to a variety of diagnoses, but can pose challenges to the psychosocial well-being of pregnant women, such as social isolation, anxiety, alteration of normal life routines, and less preparation for postpartum life. Advancements in medicine and technologies have led to an increase in the population of children living with complex and chronic health conditions. Children with medical complexities (CMCs) and their caregivers frequently have challenges navigating home life after transitioning from hospital to home-based care. A 14-week doctoral capstone project was conducted at Dayton Children’s Hospital to improve caregiver readiness for hospital discharge. Project participants included caregivers of CMCs (n = 6) admitted to the hospital and pediatric acute care occupational therapists (n = 4). Purpose: To study the influence of ultra-widefield fluorescein angiography (FA) on the diagnosis and management of diabetic retinopathy (DR).
Methods: Ten experts in DR completed an online survey in which they were asked to diagnose and manage DR cases using different imaging modalities. Experts independently reviewed 20 cases of DR and provided a diagnosis and management plan for each case, first based on ultra-widefield color-free and red-free images alone and again with the corresponding ultra-widefield FA images. Background: Rehabilitation methods that include anodal transcranial direct current stimulation (atDCS) and extended reality (XR) exercises have been used to enhance neural networks and improve functional performance in stroke patients, but the neuromuscular and neurophysiological mechanisms underlying these improvements are not fully understood. The purpose of this study was to examine the effects of atDCS during XR rehabilitation exercises on cortical, neuromuscular, and clinical outcomes of stroke survivors. Methods: Nineteen chronic stroke survivors were placed into either a transcranial direct current stimulation (tDCS) or a Sham group, without significant (p > 0.73) differences in the baseline levels of disability between groups. …

The occupational demands of policing are challenging and potentially hazardous, with substantial rates of musculoskeletal injuries occurring during recruit training. Academy injury prevalence can be as high as one in five recruits. Injuries impact performance, result in lost training, and impose financial burdens on police agencies. However, limited research has explored pre-injury body composition or musculoskeletal characteristics in relation to police recruit performance. This thesis investigates injuries, body composition and musculoskeletal characteristics in physical performance and injury risk among law enforcement recruits.

CONTEXT: Cardiometabolic disease (CMD) frequently occurs in individuals with spinal cord injury (SCI), with growing awareness surrounding the expansive scope of this problem. As CMD has significant morbidity and mortality, early guidelines-based screening and management have been established. However, the extent to which these guidelines have been adopted are unclear.
OBJECTIVE: Describe physicians' screening and management pattern for CMD in patients with SCI, as compared to SCI-specific CMD screening guidelines, and elucidate variables linked to screening and management patterns.

This mixed-method cohort study focused on supporting direct support professionals through burnout education modules. Burnout is a psychological phenomenon characterized by mental, physical, and emotional exhaustion in response to chronic stress in the workplace. Direct support professionals (DSP) provide services that include independent living and vocational skill education to adults with intellectual disabilities. Burnout has negative impacts on work quality, efficiency, and can led to high turnover rates. This project aimed to provide DSP’s with resources to mitigate the risk of burnout. Those admitted to the intensive care unit (ICU) commonly experience deficits in areas linked with cognition, mental health, and physical functioning (Rapolthy-Beck et al., 2023). Delirium is the most common complication among individuals who are mechanically ventilated, which can be associated with adverse outcomes, including increased mortality, cognitive decline, and decreased functional outcomes (Martinez et al., 2017). Life skills competency in adolescence can be an important predictor of success in adulthood. Educational and training programs focused on life skills can help youth develop the skills that they need to live independently. Movement is the primary means by which the nervous system converts intention into interaction with the environment. Voluntary limb motion enables standing, walking, reaching, and grasping—actions that underpin self‑care, communication, and economic participation. When diseases or injuries disrupt the pathways that couple neural commands to muscle forces—whether through cortical stroke, spinal cord injury, or peripheral nerve damage—mechanical outcomes such as weakness, loss of coordination, and ultimately reduced mobility emerge.
