This study has the following purposes: (1) to inquire the incidence of radiculopathy on the asymptomatic side in the cervical spondylotic patients with unilateral symptomatic radiculopathy. (2) to inquire the relationship between asymptomatic side and symptomatic side in the unilateral symptomatic radiculopathic patients due to cervical spondylosis. All subjects has been studied by motor and sensory nerve conduction test, and electromyography (EMG) for muscles supplied by roots from C-5 to T-l levels bilaterally. The results showed that the incidence of radiculopathy and the EMG abnormalities on the asymptomatic side correlated well with the EMG abnormalities on the symptomatic side, but …

H-reflexes of the flexor carpi radialis muscle were studied in 60 controls and 42 patients with C7 radiculopathy. Reflex parameters were compared to normal standards and correlated with clinical evaluations, radiologic and other electrophysiologic findings. The surface recording technique used was similar to that described in the previous report for the study of normal subjects.The present data reveled: 1) Patients with C7 radiculopathy showed an abnormally prolonged latency of the FCR H-reflex. It increased from an average of 16.8±1.9 msec in normal subjects to 22.4±1.7 msec, p<0.05. 2) The peak-to-peak amplitude of the H-reflex was significantly decreased from a normal …

The purpose of this study was to investigate the treatment effect of Russian's electrical stimulative training program versus conventional exercise in muscle strengthening of quadriceps. Seventeen patients who had undergone surgery for fracture of lower leg or ACL, PCL reconstruction or repairment, were randomly divided into two groups; 7 patients served as a control group underwent isometric contraction of quadriceps, another 10 patients served as a study group underwent with maximal tolerable isometric contraction induced by Russian's E.S.. A good exercise habit should include warm-up, aerobic exercise, and cool down. The latter can prevent post-exercise venous pooling and reduce the risk of postural hypotension and myocardial ischemia. Besides, the frequency, duration, and mode of exercise also related to the benefits of exercise. This study investigated the exercise habit of 273 randomly selected (145 men, 128 women) ordinary people by questionnaire. Iatrogenic femoral nerve damage has already been described after retroperitoneal tumor resection, hysterectomy, abdominal rectoplexy and total hip arthroplasty etc., but never after ascending colon carcinoma resection. We report the occurrence of femoral nerve injury in a 76 years old male patients after the procedure of right hemicolectomy and debulking of the tumor. Neurological symptoms appeared immediately after surgery. Sensory deficit was evident with reduced sensation of the anterior surface of right thigh and knee. Pigmented villonodular synovitis is an uncommon disorder characterized by synovial proliferation and hemosiderin deposition into the synovial lining of the affected joints, tendon sheaths and bursae. Its etiology is not fully known, however, inflammation, neoplasm and trauma have been postulated. It commonly affects the knee, hip, ankle and elbow joints, but rarely, the shoulder and its bursae. Shoulder pain is a common complication present in the stroke patients and its incidence varies from 5% to 84%. We report a 76year-old female of T11and T12 vertebral compression fracture, presenting with abdominal pain and acute ileus but minimal back symptom within 24hours of onset. The treatment was focused on the abdominal condition, exploratory lapartotmy was done with normal intra-abdominal organ, except diffuse moderate distention of intestines. Gallium-67scan was arranged to differentiate recent fromold fracture. The mechanism of thoracolumbar compression fractures presenting with an acute ileus is possible due to the retroperitoneal hemorrhage or structure change irritating the sympathetic chains which innervate the intestines. Treatment of acute ileus were reviewed and discussed.

The facial expression of functional recovery in patients with peripheral facial paisy/paraiysis on rehabili-tation therapy was investigated. Thirty-four cases of fa-cial palsy were collected from January 1986 to Decem-ber 1991 in the Department of Physicai Medicine and Rehabilitation of Tri-Service General Hospital. Twenty-five cases (74%) were classified as idiopathic and the remaining 9 cases (26%) were as organic facial palsy.The rehabilitation program included daily superficial heat, oii massage, electrical stimulation and active exer-cise of the affected muscles was prescribed and instituted completely. A 60 year-old male patient who presented with lumbago, fever, local tenderness over lumbar spine and associated with paravertebral muscle spasm, was prescribed antibiotic therapy 12 days after the onset of initial symptom under the impression of septic lumbar discitis, which was then verified by MRI and microbiology within one week after admission. Early diagnosis and adequate antibiotic therapy gained a favorable result, although the patient needed anterior inlerbody fusion for symptomatic retrolisthesis of L3 on L4 at the sixth week after admission.

This study was taken to determine the cardiovascular response after isokinetic endurance exercise among different extremities (Upper vs Lower), speeds (High-180°sec vs Low-60°sec), Types (Concentric vs Eccentric). The purpose of this study was to evaluate the effect of quadriceps isokinetic eccentric trainingprogram at the strength of both concentric and eccentric strengths, as well as for the different eccentric training positions. One hundred and fifty-six stroke patients were investigated to analyze the changes of time or frequency of performing social activities between pre-stroke and post-stroke stages. Modified (Frenchay activities index) questionnaire was used and subjects were devided into three groups by the Barthel scores. Within- subjects Wilcoxon signed-rank test was used to analyze the collected data. Renal failure was once the leading cause of death in patients with chronic spinal cord lesions. In western countries, dramatic improvements in urologic care enhancing patient survival have occurred in the last 25 years. Nonetheless, urological complications continue to be the major morbidity in spinal cord injured patients. A case of 4 years 8 months old girl who had the brain insult was followed till 8 years 1 month old. Precocious puberty with hyperphagia was noted. Results of endocrine testing revealed normal pituitary function, but increased to pubertal level of gonadotropins. Radiological study confirmed posttramatic encephalomalacia at right hemisphere including right ventromedial hypothalamus. This report discussed the medical course of the case including rehabilitation management.

Myofascial pain syndrome is a common diagnosis in the outpatient department. Ciinicians often emphasize pain management and neglect the under-lying causes. This study was based on 5 cases of microtumor manifested with myofascial pain syndrome. The data were collected from April 1984 to December 1991 in Veterans General Hospital Taichung. The clinical symptoms which lasted from 1 month to 17 years, physical examination which included active, passive & resisted active ROM, painpattern, image study, operative findings and outcome will be discussed. Inclusion Body Myositis: Report Of A Case, Chein-Wei Chang, Tien-Jye Chang, Ming-Shiang Wu, Che-Yen Chen

A female of age 30 years developed a muscular weakness in proximal extremities during 2 years and responded poorly to steroid treatment. Creatine kinase in isoenzymic MM form was markedly elevated according to a serum muscle enzyme test. Electromyography examination showed a myopathic feature. Histopathological tests in muscle biopsy specimens revealed prominent cytoplasmic vacuoles rimmed by basophilic granules under space optical microscopy, and abundant intranuclear filamentous inclusions in electron microscopy. These clinical features and athological especially electron microscopic findings are considered to be of paramount importance for the diagnosis of inclusion body myositis.
