The purpose of this study was to investigate prosthesis use among patients who underwent lower extremity amputation owing to peripheral vascular disease and evaluate their functional outcome after returning to the community. We retrospectively reviewarticleed medical charts of patients who underwent lower extremity amputation from 1993 to 1999. The causes of amputation, the level of amputation and patient's health condition were recorded. We then surveyed the patients by telephone, and recorded the stump condition, difficulties encountered in daily activities, and functional status. From January 1993 to December 1999, 347 patients underwent lower-limb amputations in our hospital. Of them, 174 died, …

For patients with symptomatic end-stage coronary artery disease, who have had a poor response to percutaneous transluminal coronary angioplasty or coronary artery bypass grafting, transmyocardial revascularization (TMR) seems to be an alternative to relieve disabling ischemic symptoms. The purpose of this study was to evaluate, by cardiopulmonary exercise testing, the cardiopulmonary capacity after TMR.Ten patients were enrolled in the study. None of them showed significant exacerbation in pulmonary function after TMR. Their average aerobic capacity was about 5MET5; a lower anaerobic threshold and high functional aerobic impairment revealed an insufficient cardiopulmonary capacity. An exact identification among the subsets of pulmonary lupus is important in planning efficacious therapy. Asymptomatic pulmonary lupus needs no specific treatment, while prompt and aggressive treatment is mandatory for the potential life-threatening pulmonary version. We present a case of systemic lupus erythematosus(SLE)with lobar atelectasis and severely restricted lung impairment and illustrate how a pulmonary rehabilitation program aided therapy during the acute infectious episode. An individualized pulmonary rehabilitation program for SLE patients compromised with chest distress may thus be a prudent course of action.

According to the definition proposed by Grosjean, more than half of the world population is multilingual. Bilingual aphasia is more complex and variable; thus it may need a more individualized rehabilitation program than monolingual aphasia. Supratentorial primitive neuroectodermal tumors are a very rare brain tumor. This reports the case of a 14 year old girl who visited our emergency department owing to severe headache and vomiting. One 72mm×45mm mass located at the left corpus callosum and extending to the left ventricle was identified by Brain magnetic imaging. The tumor was removed and the pathology demonstrated highly cellular tumor with poorly differential cells and brisk miotic activity. Based on the tumor location and pathology, a supratentorial primitive neuroectodermal tumor was diagnosed. Osteoarthritis (OA) is a slowly evolving articular disease that appears to originate in the cartilage and affects the underlying bone, soft tissues, and synovial fluid. This condition usually occurs late in life, principally affecting the large weight-bearing joints such as the knee. Carpal tunnel syndrome is one of the most common peripheral entrapment neuropathies. Patients with carpal tunnel syndrome experience numbness, pain, and/or weakness in the hands over the median nerve distribution area. In current practice, diagnosis of carpal tunnel syndrome is based on subjective symptoms and objective findings from physical examinations and electrodiagnosis. We examined 40 hands with carpal tunnel syndrome in 24 patients and evaluated the relation between symptom severity and electrodiagnosis. Churg-Strauss syndrome (CSS) is a rare systemic vasculitis of unknown cause, occurring in patients with asthma and accompanied by hypereosinophilia. Pathologically, CSS is characterized by necrotising eosinophilic vasculitis which may involve multiple organ systems, commonly including the lungs, skin, and peripheral nervous system. Corticosteroids are the first-line therapy, with cyclophosphamide as adjuvant therapy. The overall prognosis is good.We report a case of CSS with the clinical pattern of asthma, hypereosinophlilia (>10%), pulmonary infiltration, and mononeuritis multiplex. Aortic valve replacement (AVR) is a common surgical procedure to treat severe valvular heart disease. Major complications of AVR include perioperative myocardial infarction, thromboemboli, hemorrhage, prosthetic endocarditis, aortic dissection, prosthetic dehiscence, and prosthetic dysfunction. Ischemic myeloradiculopathy due to aortic dissection after AVR is another complication documented in the medical literature, but the combination of lower-extremity compartment syndrome and ischemic myeloradiculopathy is rarely reported. We describe the clinical course and recovery of a 57-year-old man with this unusual combination and discuss the proper management to reduce long-term disability.

Botulinum toxin type A (BTA) has treated spasticity in cerebral palsy (CP) in recent years. This study investigated both the clinical effects and gait improvements of BTA on spasticity management in children with CP.Twenty-nine children with spastic CP (2-10 years old) were recruited into this study. Spasticity of the upper limb muscles is a common complication following stroke. Injection of botulinum toxin can decrease upper limb spasticity. The effects of botulinum toxin treatment are usually evaluated by activities of daily living, range of motion (ROM), or modified Ashworth scale. We describe a patient with Salmonella vertebral osteomyelitis who presented with severe low back pain. An initial diagnosis of compression fracture was based on clinical symptoms and signs, as well as imaging studies. During hospitalization, the patient developed a fever. A repeat MRI, positive blood cultures, and abscess culture results obtained during an operative procedure confirmed the diagnosis of Salmonella vertebral osteomyelitis. Salmonella vertebral osteomyelitis is rare, and its clinical presentation may mimic other common diseases such as compression fracture. Central pontine myelinolysis (CPM) characterizes demyelinolysis of the central basal pons without involving the nerve axon. The etiology and incidence are unclear. According to previous reports, CPM occurs most often in chronic alcoholism, followed by hyponatremia due to rapid correction of electrolyte imbalance. Clinical manifestations of CPM include consciousness disturbance, lethargy, impaired brain function, dysphasia, dysarthria, four limbs weakness, ataxia, decreased deep tendon reflex, hypotension and seizure. The objective of this stud is to compare the efficacy between mobilization with movement (MWM) and end-range mobilization techniques (EMTs) in the management of adhesive capsulitis. Eighteen and twenty-two patients from MWM and EMT groups, respectively, who returned after two-week follow-up were evaluated. This was a randomized, evaluator-blind, parallel comparison study. In the United States, the hospital-based fitness centers were rapidly developed in recent years and, in fact, they have already had over twenty years history. However, the concept is just at beginning stage in Taiwan. This investigation was directed against the managers of the hospital in Taiwan. Postural hypotension is often observed in patients with cervical spinal cord injury. It may cause syncope, and interfere with sitting and standing trainings. Neuromuscular electrical stimulation (NMES) was reported to be able to alleviate and even prevent postural hypotension. Little is known about the relation of planned exercise to insulin sensitivity in type 2 diabetes in Taiwan. Thus, the purpose of this study was to explore the effects of a 12-week aerobic exercise program on insulin sensitivity, serum lipids, cardiorespiratory fitness, and blood pressure in patients with type 2 diabetes. This work describes motor, cognition and emotional problems in a child with Moyamoya disease before and after rehabilitation. Following comprehensive rehabilitation, the girl achieved improved gross motor, fine motor, cognition and emotional control. Moyamoya disease is an important cause of childhood ischemic stroke. Moyamoya disease has a bimodal age distribution, with one group of pediatric age (around first decade of life), and a second group of adults in the 30 to 40-year-old age range. Moyamoya disease involves progressive idiopathic stenosis and eventual occlusion of the large cerebral arteries at the circle of Willis. The pathogenesis of Moyamoya disease remains unclear. …

A 55-year-old male patient suffered from left intracranial hemorrhage received craniotomy and removal of intracerebral hematoma. Ventriculoperitoneal shunt was implanted for the complication of hydrocephalus. The skull was put back later because of stable status. He developed scalp necrosis and, therefore several times of debridement was done. The skull was taken away under the suspicion of osteomyelitis. However, he had vomiting, somnolence, urinary incontinence and unable to walk two months after this procedure. Computed tomography showed slit ventricles in right hemisphere. Acute inflammatory demyelinating polyradiculoneuropathy (AIDP), also known as Guillain-Barre syndrome (GBS),the most common illness that causes acute and generalized paralyzing condition in the post-polio era, is diagnosed dependent on the base of clinical manifestations with supportive evidence of electrophysiologic study and cerebrospinal fluid examination. GBS often begins with the distal part of four limbs followed by a progressive course as ascending weakness and may even involve respiratory , facial or oropharyngeal muscles. Spinal fluid examination often reveals elevated protein concentration without associated increase in cells. The subject patient was a case of ankylosing spondylitis (AS). Because of recurrent fine vertebral fracture combined with pseudoarthrosis and neurological deficiency, the patient had received several conventional surgical interventions over ten years. A year before the present admission, the patient suffered lower limbs weakness and received surgical intervention with thoracic corpectomy and fixation with vertebral prosthesis, anterior spinal instrumentation and posterior fusion with pedicle screw at Stanford hospital in the U.S.A. Hemineglect is a common symptom and sign in patients with right hemisphere lesion. These patients usually have problems of reduced awareness of stimuli from left side. This study aims to evaluate the correlation between presence of hemineglect and functional disability in patients with right hemispheric stroke or traumatic brain injury. This study is to investigate the fitness profile in the middle and older persons, and to observe the effect of age on fitness.This study recruited 108 subjects, with independent ambulation skills in a Taipei community. The purposes of this study were to investigate the circumstances of occurrences in 53 patients with moderate to severe burns under 12 years of age, who were admitted to the burn center of the Armed Forces Tsoying Hospital in Kaohsiung from June 1998 to May 2000. A patient with traumatic rotator cuff tear of right shoulder developed severe myofascial pain syndrome with reflex sympathetic dystrophy (RSD) involving the right upper extremity. She was unable to take any type of manual therapy or needle treatment due to severe allodynia in the whole right upper limb. She visited our pain clinic about 2 months after the onset of trauma. She received myofascial trigger point (MTrP) injection beginning with injection into the MTrP of the right first dorsal interosseous muscle, followed by injecting MTrPs of the right wrist-finger extensors and right anterior deltoid muscles. The development of pelvic lymphocele is usually a complication following renal transplantation as well as pelvic lymphadenectomy for gynecologic or urologic cancers. Pelvic lymphocele after blunt trauma is rare described before. Asymptomatic patients are managed with observation and follow-up imaging studies. Symptomatic lymphoceles need to be treated by means of percutaneous needle aspiration or drainage. We reported a case of low back pain after a fall with delayed onset of ispilateral thigh swelling. Computed tomography revealed a fluid-filled mass in the pelvis. After treatment by CT-guided percutaneous aspiration, all symptoms rapidly resolved without recurrent symptoms in 6 months follow-up.
