There are 200 thousand stutterers in Taiwan. In addition to disfluency, they may have social or work adaptation problem. The purpose of this research is to study the efficacy of integrated approach combining stuttering modification therapy and fluency shaping therapy in stuttering adults. Seventeen stuttering adults were included in this study. The experimental group consisted of 7 males and 4 females with average age of 26.73 (SD=7.37). The average age of the control group was 26.67 (SD=7.92), which consisted of 5 males and 1 female. In order to evaluate the prevalence and risk factors of post-stroke depression, this study collected the data of 102 stroke patients from the outpatient clinic of the neurology and rehabilitation departments in a medical center in Taipei. The patients' objective data, such as age, infarction area of the brain, duration from the stroke attack to the time of our study, number of stroke attacks, antidepressant use, and rehabilitation program, were obtained from the history chart. Despite the great number of computer input devices available commercially, many disabled persons still rely on special equipment to operate computers. Providing those persons with an adequate human-computer interface should improve their productivity and efficiency. However, the specialized equipment needed is usually expensive, difficult to obtain and difficult to maintain. This study developed an economical apparatus that, flexibly integrated commercial computer mice and trackballs to tailor them for individual needs. A case of femoral neuropathy as a complication of flap excision for reconstruction of a defect due to buccal cancer excision is presented. A 51-year-old man developed numbness over his left anteromedial thigh and medial lower leg, left quadriceps weakness, and absence of left knee jerk after a flap was obtained from his anterolateral thigh. Electromyographic examination revealed evidence of denervation in the left quadriceps. A nerve conduction study showed prolonged latency, reduced amplitude of the compound muscle action potential, decreased velocity in the left femoral nerve and prolonged latency, reduced amplitude of the sensory nerve action potential, and decreased …

Winging scapula is uncommon clinically and easily ignored. It not only leads to a limited range of motion of the upper extremities, but impairs functional activity and body appearance as well. Many etiologies can induce winging scapula, including trauma, infection, iatrogenic injury, congenital abnormality, and the impairment of periscapular nerves, muscles, bones and joints. Urinary incontinence is a very common but also very debilitating problem for older women. It has been estimated that the incidence of urinary incontinence among women older than 65 of age is about 30-60%. The management includes surgical therapy and conservative treatment such as drug therapy and rehabilitation. The etiology and natural history of adhesive capsulitis remain obscure, which limited subsequent management. Traditional treatments include conservative medical therapy and physical therapy, but severe shoulder pain may constitute an obstacle to joint mobilization. Organophosphates (OP) are inhibitors of acetylcholinesterase. OP intoxication results in an accumulation of acetylcholine. Clinical signs and symptoms of acute OP poisoning include salivation, incontinence, vomiting, muscle weakness, hypertension, palpitation, restlessness, headache, drowsiness, confusion and seizure. After the acute stage of OP intoxication some patients recover fully, but others have neurologic sequelae, including central and peripheral neuropathy. Individuals with milder cases may recover fully within several months; however, more severe cases may result in persistent sequelae.A 20-year old woman attempted suicide by drinking organophosphate pesticide in 1995. Central pontine myelinolysis(CPM) and extrapontine myelinolysis(EPM), sharing the same pathology, are the same disease, although different symptoms and signs developed according to different lesion sites. The etiology and pathogenesis of CPM/EPM remain unclear, meanwhile rapid correction of the serum sodium level for hyponatremic patient has proven to be one of the major causes of CPM/EPM. Therefore an appropriate correction of the sodium level could prevent the development of CPM/EPM. The incidence of hyponatremia after surgical removal of pituitary adenoma reported varies from 8.4% to 38.8%. The incidence of fracture is higher in patients with chronic spinal cord injury (SCI), particularly female and elderly patients, than in healthy people. Functional impairment due to SCI may lead to accidental falls and fractures, most of which are due to trivial injury or without known trauma. In addition, the inevitable osteoporosis in the paralyzed limbs of individuals with chronic SCI increases their risk of fracture. The lower limbs are more often involved than the upper limbs. Limbus vertebral fracture results from the herniation of disc material into a vertebral body, and in the formation of one or more isolated bony fragments. It is mostly found in adolescents and young adults. Clinically it resembles an acute disc prolapse with low back pain and radiculopathy. Trauma plays a significant etiologic role in up to one half of cases reported in the literature. The diagnosis of limbus vertebral fracture can rarely be based on conventional radiographs. The aim of this study was to investigate the short-term effect of temperature variation on type of cerebrovascular accident in first-time stroke patients.There were 1008 in-patients enrolled in this study, including 718 ischemic (infarct) stroke and 290 hemorrhagic stroke patients. Cerebellar stroke is uncommon in young patients. We report the case of a 22-year-old man with a delayed cerebellar infarction secondary to vertebral artery dissection caused by trauma. While riding a bicycle as part of an extreme sport, his left clavicle struck a bicycle handle. Evaluation in the emergency room led to a diagnosis of left shoulder contusion without fracture of clavicle or cervical spine. Thickening of finger extensor tendons in hemiplegic hands of stroke patients has never been reported. We reported 11 patients with stroke who experienced gradual onset of a painless swelling at the dorsal aspect of the hemiplegic wrist. Soft tissue sonography revealed that the swellings were mainly composed of thickened finger extensor tendons. This thickening typically involved the tendons of the 4th dorsal extensor compartment between the proximal carpal and mid-metacarpal levels. There have been isolated reports in the literature of the natural history and less than complete knowledge of the epidemiology and the pathophysiology of endocrine alteration occurring after traumatic brain injury (TBI). This article describes one primarily adrenal-dysfunction patient with severe TBI who had no evidence of present or prior adrenal autoimmune disease, infection, hemorrhage, or malignancy. It is suggested that this distinct condition can be labeled as ”primary adrenal insufficiency (PAI) developing after TBI”.

Patients with high-level cervical cord injuries are dependent in most activities of daily living (ADL) because they possess very little residual motor function. The utilization of appropriate assistive devices could enhance their independence and improve their quality of life.A prospective study was conducted with 11 male tetraplegic patients between 22 to 51 years of age (mean, 32.0±10.0 years), living in central Taiwan, with C4 or C5 cord injuries; the duration of their injuries ranged between 16 to 125 months (mean, 56.8±28.6 months). Polyneuropathy is common in the field of rehabilitation medicine. Its symptoms include sensory impairment in the distal limbs, ataxia, tremor, and pinch weakness. Electrophysiology will show multiple neuropathies with distal involvement first. Sensory nerve impairment usually appears earlier than motor nerve impairment. In severe cases, muscle atrophy can also be seen.Due to the multiple etiologies of polyneuropathy, the treatments vary. We report a rare case of anti-MAG neuropathy. The objective of the present study was to investigate the growth and body composition of younger children with cerebral palsy, using anthropometric measures and bioelectrical impedance analysis (BIA). We also looked into the relative strength and consistency between different measurements in analyzing body composition. Thirty-nine patients, aged 3 to 13, with an established diagnosis of cerebral palsy, and age- and sex-matched control subjects, were recruited for this study. Distribution of height-by-age and weight-by-age percentiles differed significantly between the study and the control groups, whereby the subjects of cerebral palsy grew shorter and lighter than normal subjects.
