Spinal arteriovenous malformations (Spinal AVM) are a rare disorder with an estimated incidence of about 1 per million people per year. Because of the diversity of the presenting symptoms, this disorder is difficult to diagnose, despite new advances in neuronimaging. We report the case of a girl who was diagnosed at 12 years old to have spinal AVM at a high cervical level and presented with four limbs weakness and dyspnea. The work-related musculoskeletal disorders among the child care workers could be high given the fact that the workers frequently lift the children and conduct physical actions under inappropriate postures. The objective of this study is to survey the musculoskeletal disorders and potential ergonomic risk factors among the workers in a child care center through an on-site visit and a questionnaire survey. We also used the Chinese Health Questionnaire (CHQ) as a screening questionnaire to detect the emotional distress of the workers.There were 85 workers recruited in our study. Objective: This reviewarticle article describes the etiology, pathogenesis, clinical characters and management of low back pain (LBP) caused by myofascial trigger points (MTrPs), i.e. myofascial low back pain.Findings: Based on the currently available knowledge and our clinical experience, we analyzed the basic and clinical aspects of myofascial LBP. Most cases of myofascial LBP are related to injuries, either current or previous. Active MTrPs that cause LBP are usually activated as a consequence of other etiological lesions. The purpose of this casereport is to demonstrate the remote effect of myofascial trigger point (MTrP) injection for pain control. It describes a patient suffering from chronic pain due to MTrPs in bilateral upper trapezius muscle with poor results to direct MTrP injection to upper trapezius. After injection to the MTrPs in bilateral extensor carpi radialis longus, extensor indicis, and first dorsal interosseous muscles, the effectiveness of MTrP injection to bilateral upper trapezius muscles remarkably enhanced. Flatfoot disease is caused by disappearance or collapse of the medial longitudinal arch of the foot. Most of patients with flatfoot disease have what is known as ”flexible flatfoot”. Flatfoot in developmentally delayed children, however, is due to muscle weakness of lower limbs, which leads to disproportionate muscle growth or joint contracture. A study conducted by Chii-Jeng Lin in 2001 indicated that flat-footedness may influence gait, single-foot standing, and jumping ability. A 31-year-old lady complained of multiple painless masses on her left arm. Two masses were clearly visible in the left palm and distal forearm, and a smaller lump was also palpable in the left antecubital area. A neurogenic tumor in the left palm was diagnosed by magnetic resonance image (MRI). Sonography demonstrated highly vascular neurilemmomas along the median nerve in the palm, distal forearm, and antecubital area. A nerve conduction velocity study of the median nerve showed prolonged distal latency of the motor nerve portion but normal sensory function. Children with spastic cerebral palsy (CP) usually exhibit motor deficits and limited daily activities. Studies in recent years indicated that constraint-induced movement therapy (CIMT) might improve motor performance in CP patients. Combining kinematic analysis with clinical assessments, this study investigated the effect of modified CIMT (mCIMT) in four children with spastic CP immediately and one month after treatment. This casereport describes a less common but not rare condition: iatrogenic accessory nerve injury after neck lymph node biopsy. The Buddist Tzu Chi Dalin Hospital was responsible for providing medical services to special education students of Chiayi County in 2004. We evaluated the cognitive skills, activities of daily living, language skills, and gross and fine motor functions of 160 disabled students, 113 male and 47 female. Students with mental retardation were the most frequent, followed by students with dyslalia, then autism, and attention deficit and hyperactivity disorder. A needs assessment revealed that 155, 132, and 107 students fed, dressed, and bathed themselves independently, respectively. This study investigated and analyzed the prevalence of contraindications that can co-exist with a prescribed physical agent. To prevent likely complications, this study implemented a computer warning system, which prompts physiatrists to possible contraindications. Once a physical agent is prescribed, the system automatically reviewarticles the ICD-9 codes in the patients’ medical chart. Two pre-established databases are used in the warning system: one contains contraindications for each physical agent and the other has possible contraindications for each selected ICD-9 code. Between February and September 2004, 73 patients with spinal cord lesions of greater than 2 years duration were enrolled in this study. A chart reviewarticle was conducted to record the manner in which the patients' bladder management, previous urologic complications, and any corresponding treatments and effects. Among the patients, 55 were male and 18 were female, with an average age of 29.3±12.1 and 32.0±13.0 years, respectively. Hereditary neuropathy with liability to pressure palsies (HNPP) is an autosomal-dominant disorder characterized by self-limited, recurrent compressive mononeuropathies at common entrapment sites precipitated by trivial injuries. This disorder typically develops in early adulthood. Electrodiagnostic studies revealed diffuse mild demyelinating neuropathies with entrapment over common entrapment sites. Nerve biopsies frequently show segmental demyelination and thickening of the myelin sheath which is mostly caused by 1.5 Mb deletion of the 17p 11.2 site containing the peripheral myelin protein 22 (PMP22) gene on the 17(superscript th) chromosome. Bickerstaff's brainstem encephalitis is a post-infectious brainstem inflammatory disorder of unknown etiology. The characteristic symptoms include: acute ophthalmoplegia with diplopia, ataxia, disturbed consciousness, and extensor plantar response. In this report we describe a 33-year-old woman with Bickerstaff's brainstem encephalitis combined with Guillain-Barré syndrome that presented 25 days after a Cesarean section. After five treatments with plasmapheresis and intravenous immunoglobin for 5 days, the patient's muscle strength recovered to MRC 4/5. The ophthalmoplegia and bilateral extensor plantar responses also disappeared. The patient then was transferred to the PM&R department for persistent slurred speech, ataxia, and diplopia. Infective discitis accompanying low back pain is uncommon. In general, risk factors for infective discitis include an age greater than 50 years, a history of cancer, unexplained weight loss, pain that lasts more than 1 month, a history of intravenous drug abuse, presence of a urinary tract infection or other infections, diabetes mellitus, and recent invasive procedures. The presence of an elevated erythrocyte sedimentation rate (ESR) and a C-reactive protein (CRP) level have proven to be reliable laboratory criteria consistent with infective discitis. Viscosupplementation by intraarticular administration of exogenous hyaluronic acid has been a favorable treatment option to alleviate the symptoms of knee osteoarthritis. The potential but uncommon adverse reactions associated with intraarticular hyaluronic acid injection are local pain, swelling, and allergic response. These reactions are typically mild, transient, and self-limiting. The focus of this report is an 80-year-old man who presented with acute painful swelling of his knee joint after an injection of naturally derived hyaluronic acid. Aspiration of knee joint effusion yielded 40 mL of turbid straw-colored fluid. Synovial fluid cell counts revealed numerous leukocytes with a marked predominance of neutrophils. …

Patients who suffer antiphosphlipid antibody syndrome may have two kinds of antibodies, Anticardiolipin and Lupus anticoagulant. These antibodies may increase the risk of ischemic stroke and cerebral ischemia. However, the incidence of cerebral bleeding is relative rare. As a result, patients with antiphosphlipid syndrome often take oral anticoagulants for a long-term period to prevent ischemic stroke attack.We report a female patient who was diagnosed antiphosphlipid antibody syndrome in 1998, and took oral anticoagulants for a long period to restrain the symptom of thrombosis. Cerebellar mutism syndrome is a rare but devastating consequence following posterior fossa surgery or lesion. It tends to occure predominantly in children, and is characterized by transient mutism, oromotor apraxia, apathy, hypokinesis, irritability and mood lability. The vermis, paravermian region and deep cerebellar nuclei are important structures which are highly correlated with the occurrence of cerebellar mutism syndrome. We report two cases, aged 11 and 15 years, who developed cerebellar mutism after rupture of posterior fossa arteriovenous malformation. Objective: Prematurely born infants have been believed to be neurologically less mature than full-term neonates. The purpose of this study was to investigate the regulation and maturation of autonomic nervous systems, in both full-term and preterm neonates of different conceptional ages.Methods: One hundred and thirteen infants, both clinically and neurologically healthy and with conceptional ages of between 33 and 42 weeks, were enrolled in the study. High resolution musculoskeletal ultrasonography has been a comprehensive tool for differentiation of soft tissue mass. However, it is not commonly used as a standard examination in the diagnosis of an aneurysm. We present a case of a growing mass in the medial aspect of the left thigh status post internal fixation of periprosthetic fracture. Initial examination with a 10 MHz linear array probe revealed a blurred hypoechoic lesion at a depth of 5 cm.
