Associated Study Between Dopamine Drd2 Gene Polymorphism And Finger Tapping Test, Wen-Hsuan Hou, Tai-Jui Chen
Rehabilitation Practice and Science
Neurotransmitter dopamine has relation with neuro-psychiatric disease, it also has effect on motor function for healthy individuals and also determines the outcome of rehabilitation for neuro-psychiatric disease patients. Finger Tapping Test (FTT), a neuro-psychological test, is thought to be associated with dopamine. Therefore, the goal of this study is to investigate the association relationship between dopamine genes polymorphism (DRD2 TaqI) and FTT. Method: We recruit healthy Han Chinese, age between 20 and 60 years old, having no neuro-psychiatric illness and family history for dopamine genes polymorphism (DRD2 TaqI) and FTT assessment. Result: Total 106 cases were enrolled in the study. …

Clinical Experience Of Ventriculoperitoneal Shunt Dysfunction In A Patient With Hydrocephalus After Brain Injury: A Casereport, Shean-Jing Lin, Yun-Yee Chen, Mu-Jung Kao, Ching-Yang Lin, Sheng-Huang Hsiao
Rehabilitation Practice and Science
We report a case of an adult patient with post-traumatic hydrocephalus who was treated using a ventriculoperitoneal (VP) shunt with a fixed pressure setting valve. Slit ventricle syndrome was found two months later and an antisiphon device (ASD) was added. A fixed pressure valve with ASD was found not to be optimal in terms of improving his neurological symptoms and a programmable valve system was therefore implanted with a pressure setting of 140 mmH2O in the beginning. The valve then was gradually programmed to reduce down to 70 mmH2O over two months. This case demonstrates that selection of a shunting …

Combining Therapeutic Ultrasound And Breast Massage For Plugged Ducts: Three Casereports, Yu-Chun Lee, Xiu-Yong Hong, Chin-Teng Chung, Wei-Chih Chen, Hui-Mi Liu, Si-Wa Chan
Rehabilitation Practice and Science
Plugged ducts are a common problem during lactation. They often occur when the breast is not emptied properly, or when there is local pressure on the breast. Plugged ducts usually occur three to four weeks postpartum. Symptoms include a sensitive lump with well-defined margins in the breast. There may be pain and tenderness in the affected area, although this is not necessarily accompanied by systemic symptoms such as high-grade fever. Milk flow can be blocked and sometimes a tiny white spot can be seen at the opening of the mammary duct on the nipple. Without treatment, plugged ducts can lead …

Ultrasonographic Diagnosis Of Bicipitoradial Bursitis: A Casereport, Kuan-Wen Chen, Yi-Pin Chiang
Rehabilitation Practice and Science
The bicipitoradial bursa is located between the distal biceps tendon and the radial tuberosity. The bursa functions as a buffer to reduce the friction between tendon and bone during pronation of the forearm. Bicipitoradial bursitis most frequently results from repetitive mechanical microtrauma by supination and pronation movements of the forearm, and it may cause painful swelling at the antecubital fossa. However, only a few cases of bicipitoradial bursitis have been reported to be diagnosed by ultrasound. We report a case of a 32-year-old woman with a tender soft tissue mass in her right antecubital area. Musculoskeletal ultrasound depicted a distended …

Clinical Diagnosis Of Mild Misalignment Of Symphysis Pubis Using Ct Scan With Three-Dimensional Reconstruction Of Pelvis: A Casereport, Cheng-Hung Lin, Ching-Hung Hung, Nam-Seng Goh, Chen-Chang Hsiung
Rehabilitation Practice and Science
The patients who suffered from symphysis pubis injury would normally be treated in two clinical departments. Symphysis pubis diastasis is an uncommon intrapartum complication which may occur in women with some associative factors such as macrosomic fetus, small pelvis, and rapid descent of presenting part during spontaneous vaginal delivery. The automotive side impacts are a leading cause of injuries to the pubic symphysis which might be combined with major pelvic trauma. Other factors that might contribute to this type of injury is general fall events, although very rare, which would induce minor symphysis pubis injury. Recently, we presented with a …

Levamisole-Induced Multifocal Inflammatory Leukoencephalopathy: A Casereport, Chueh-Hung Wu, Lu Lu
Rehabilitation Practice and Science
Levamisole, which is the L-isomer of tetramisole, has been used as an antihelminthic agent. Due to its immunomodulating property, it has been used to treat recurrent aphthous ulcers, pemphigus vulgaris, and malignant melanoma. It also has been used along with 5-fluorouracil to treat colon cancer. The side effects are mild and include nausea, vomiting, diarrhea and metal taste. However, existing evidence suggests that levamisole can induce multifocal inflammatory leukoencephalopathy. The course of levamisole-induced multifocal inflammatory leukoencephalopathy is monophasic and progressive. Clinical symptoms include gait ataxia, dysarthria, dysphasia, conscious disturbance and weakness of the extremities (from hemiparesis to tetraplegia). The exact …

Nasogastric Tube Syndrome: A Casereport, Meng-Chang Wu, Hung-Chou Chen, Jiunn-Horng Kang, Shin-Ching Chen
Rehabilitation Practice and Science
The nasogastric tube syndrome (NG tube syndrome) is a rare but a potentially life-threatening complication of a patient with indwelling nasogastric tube feeding. The cause of the syndrome is due to the NG tube irritation, resulting in ulceration and infection of the posterior cricoid region with subsequent dysfunction of vocal cord abduction. This dysfunction may result in complete loss of vocal cord abduction and cause serious airway obstruction. Reports of this syndrome are rare in the literature. A 64-year-old female patient with long-term NG tube feeding, suffered from serious upper airway obstruction. The patient had obvious desaturation while the tracheostomy …

Initial Blood Glucose Level In The Emergency Department As A Prognostic Factor On Functional Outcome Of Ischemic Stroke, Yi-Min Chen, Gwo-Chi Hu, Jiann-Perng Chen, Chien-Shen Chen, Cheng-Tsong Liao, Kuan-Wen Chen
Rehabilitation Practice and Science
Although many studies are available in focus on prognostic factors for acute cerebral vascular disease, there are few reports to discuss the impact of the initial glucose level on the functional outcome after stroke. The initial glucose level in the emergency department is more representative of acute glucose response after stroke and is not influenced by the treatment in the emergency department. The aim of our study is to evaluate the prognostic role of the initial glucose level on the functional outcome after stroke. We recruited individuals who sustained first-ever stroke and were admitted to an inpatient stroke rehabilitation program. Patients …

Combining Botulinum Toxin Type A Injection And Modified Constraint-Induced Movement Therapy In A Taiwanese Patient With Spastic Hemiplegic Cerebral Palsy: A Casereport, Yi-Chen Tseng, Jui-Yun Su, Yu-Ching Lin
Rehabilitation Practice and Science
Hypertonia interferes with selective motor control and performance of activities in children with spastic cerebral palsy. At present, most therapists use traditional therapeutic techniques such as neurodevelopmental treatment or other approaches to handle the problems posed by cerebral palsy but the results are not very good. Many studies had shown that botulinum toxin type A (BTX-A) injection can control abnormal muscle tone effectively without major side effects. Constraint-induced movement therapy (CIMT) includes constraint of the sound side upper limb and intensive therapy of the affected limb. In order to reduce spasticity and enhance the use of the affected limb simultaneously, …

Ultrasound Examination Of Shoulders In Wheelchair Users With Paraplegia, Jiann-Perng Chen, Yi-Pin Chiang, Gwo-Chi Hu, Wan-Yi Shih, Kuan-Wen Chen
Rehabilitation Practice and Science
Due to the prolonged use of upper extremities for weight-bearing, the shoulder joints of paraplegic wheelchair-users are prone to injury. Once shoulder pain occurs, performance of activities required for daily living would thus be disturbed. In past studies, the shoulders of the spinal cord injured patients were evaluated with arthrography or magnetic resonance images and the rotator cuff injuries were found. In the current study, we used ultrasonography to detect rotator cuff injuries of paraplegic wheelchair-users. The factors associated with ultrasonographic findings were also investigated.Thirty paraplegic wheelchair-users, 26 males and 4 females, were recruited for this study. Both shoulder joints …

Rehabilitation And Prognosis Of Childhood Arterial Ischemic Stroke: A Casereport, Chia-Huang Dai, Wei-Jang Yen, Ta-Shen Kuan, Chang-Zern Hong
Rehabilitation Practice and Science
The incidence of ischemic stroke in childhood is two to 13 per 100,000 children per year. Although the incidence is much lower than adults, strokes in children not only cause a large number of deaths, but also induce significant disabilities. Because childhood ischemic strokes are rare, multi center, controlled trials should be performed to provide evidence based data related to effectiveness of medical treatment and rehabilitation. Current studies on the management of stroke in children are mainly based on casereports, uncontrolled trials, or are adapted from adult literature. An eleven-year-old, obese boy presented to the Emergency Department with an initial presentation …

Rehabilitation In A Patient With Delayed Post-Hypoxic Encephalopathy: A Casereport, Yao-Jen Chen, Su-Ju Tsai, Yueh-Lin Tsai
Rehabilitation Practice and Science
Carbon monoxide poisoning from accidents or suicide attempts often result in delayed post-hypoxic encephalopathy. Patients with delayed post-hypoxic encephalopathy typically present with recurrent apathy, confusion, agitation, and/or progressive neurological deficits, despite apparent transient recovery. Autopsy reports often reveal diffuse demyelination of subcortical white matter, but the exact pathogenesis is not clear. While comprehensive physical examination and history taking is necessary in diagnosing delayed post-hypoxic encephalopathy, magnetic resonance imaging can be very helpful in the diagnostic work-up. Rehabilitation for severe post-hypoxic encephalopathy can prevent complications such as immobilization and can improve patients' functional status. We report the history, diagnosis, and rehabilitative outcome …

Rehabilitation Practice and Science
Paroxysmal nocturnal hemoglobinuria (PNH) is a rare acquired disorder caused by a somatic mutation of the hematopoietic stem cells, which results in a loss of the complement regulatory proteins usually present on the membrane of blood cells. The outcome is an increased tendency for complement to damage the blood cells and, after a series of reactions, hemolytic and thrombotic episodes may occur. On reviewarticleing past reports, PNH-related thrombosis usually occurs in the venous system and only rarely happens in the arterial system. In our case, a male patient was found to have suffered from dark urine and anemia after each of …

Comparison Of Motor Performance Between Elderly Patients With Mild Alzheimer And Vascular Dementia In Motion Analysis, Jing-Jung Chen, Hen-Yu Lien, Wei-Hsien Hong, Chin-Chang Huang, Wen-Chun Hsu, Alice Mk Wong
Rehabilitation Practice and Science
In the past research, most factors caused motor and gait impairment in patients with dementia were related to the lesions of different areas and lobes. However, patients with different types of dementia may have distinct motor impairment features because of different neuropsychological dysfunction that may result in different motor features. At present time, clinical diagnosis of dementia patients were depended on subjective rating scales, but lack of objective measurement. The purpose of this study was to compare the patients with mild Alzheimer's dementia (AD) and patients with mild vascular dementia (VaD) in motion analysis of gait and upper limbs function …

The Cost-Effectiveness Analysis Between Outreach And Hospital Rehabilitation Models, Wei-Ni Chou, Shu-Yen Huang, Shu-Chuan Jennifer Yeh, Hing-Man Wu, Ming-Yang Koe, Yi-Ru Chen
Rehabilitation Practice and Science
Hospitals and clinics are usually the places where patients receive medicare. However, it is extremely inconvenient for the elderly to frequently go between the homes and hospitals/clinics for long-term medicare. With doctor-served home care, the accessibility of medical treatment would increase; the risk of trauma or infection from traveling in-between would be avoided; more prompt and timely treatment-offered effectiveness would rise. However, there has been no study on the difference between the cost effectiveness of outreach versus hospital rehabilitation. Thence, the peculiarity of the outreach rehabilitation regarding the physiatrists providing medical services and the physical therapists and occupational therapists offering …

Ultrasonographic Features Of Schwannoma Of The Lower Limb: Two Casereports, Hsiao-Wei Lin, Jeng-Yi Shieh, Tyng-Guey Wang, Wen-Shiang Chen, Chung-Li Wang, Yen-Ho Wang
Rehabilitation Practice and Science
Schwannoma is a benign encapsulated nerve sheath tumor. It typically develops in middle age with unknown cause and usually involves spinal root and peripheral nerves. We report two cases of schwannoma presenting as palpable mass over lower limbs. Both masses had heterogeneous hypoechogenicity, faint posterior enhancement, and were located eccentric to a nerve fascicle. Preoperative sonography revealed both patients had nerve tumors derived from nerve fascicles. These cases illustrate how sonographic information about the relationship between mass and surrounding structures can facilitate surgical planning.

Acute Spontaneous Cervical Spinal Epidural Hematoma With Hemiparesis: A Casereport, Shu-Wei Chang, Chi-Lun Rau, Shin-Liang Pan, Yen-Ho Wang
Rehabilitation Practice and Science
Spontaneous spinal epidural hematoma is a rare disease. The cardinal presentations include acute unilateral radicular pain, corresponding dermatomal hypesthesia, hemiparesis, hemiplegia, paraparesis, paraplegia, and sphincter dysfunction. If the initial symptoms include acute hemiplegia, it is important to differentiate spontaneous spinal epidural hematoma from stroke, tumor, infection, arteriovenous malformation, or autoimmune diseases. We report the case of a 42-year-old man, who visited the emergency department in September, 2006, with the chief complaints of right limbs weakness and neck pain. After the initial work-up, the impression was ischemic stroke. He was admitted for conservative treatment, but his neurological status progressively deteriorated. A cervical …

Femoral Nerve Injury Following Transfemoral Angiography: A Casereport, Chien-Ching Yu, Ying-Ju Shih, Su-Ju Tsai
Rehabilitation Practice and Science
Femoral nerve injury is a rare complication after femoral angiography. Symptoms including quadriceps weakness and atrophy, absent or reduced knee jerk and sensory impairment along the anterior thigh and medial lower leg have been found. This leads to both muscular weakness and sensory deficit, which impairs walking and interferes with quality of life. Early identification and management of femoral nerve injury could result in better prognosis. We report on the history, diagnosis and rehabilitative outcome of a 54-year-old woman with a medical history of peripheral arterial occlusion disease. She developed significant femoral neuropathy one month after angiography. Severe neurological deficits were …

Clinical Features And Rehabilitation Of Systemic Lupus Erythematosus Complicated With Moyamoya Syndrome: A Casereport, Wei-Chih Lien, Jo-Tong Chen, Shu-Cheng Lin, Nai-Wen Guo, Da-Shen Kuan
Rehabilitation Practice and Science
Moyamoya disease is an uncommon clinical entity characterized by progressive narrowing and occlusion of the bilateral distal internal carotid arteries, as well as the proximal parts of their major branches, namely, the middle and anterior cerebral arteries. Concomitant development of prominent collateral vascularity in the basal regions of the brain results in the typical angiographic appearance of moyamoya, which literally means a wavering puff of smoke. Moyamoya syndrome refers to findings of moyamoya vasculopathy in the context of known systemic disease. We present a 26-year-old woman with systemic lupus erythematosus who had several episodes of ischemic stroke. Her cerebral angiograms …

Spontaneous Spinal Epidural Hemorrhage With Shoulder And Neck Pain As Initial Presentation: A Casereport, Yu-Min Kuo, Wai-Keung Lee, Cheng-Chih Liao, Li-Rong Chen
Rehabilitation Practice and Science
Spontaneous spinal epidural hemorrhage is a very rare disease, with an annual incidence estimated at one per million. Emergency surgery is indicated for a good prognosis, without which the death rate would be higher. Although the initial symptoms of cervical, thoracic, and back pain make it difficult to differentiate spontaneous spinal epidural hemorrhage from other common disease with similar symptoms, magnetic resonance imaging (MRI) is a highly sensitive diagnostic method. We report the case of a 22-year-old male patient who initially complained of pain over the left shoulder and left cervical area. After nine days, numbness was noted in his left …
