A Screening, Referral, And Follow-Up Program For High Blood Pressure At Henry Ford Hospital: Part I. Results Of Screening, Andrea Foote, John C. Erfurt, John R. Caldwell

Data are reported on the screening results from the Henry Ford Hospital screening, referral and follow-up program for high blood pressure. The report covers the period of March 27 through December 11, 1975, during which time 808 people were screened in the hospital lobby. Thirty-nine percent were found upon initial screening to have high blood pressure readings or previously diagnosed hypertension. Of these people, 70% were already under treatment for hypertension, but only 21% showed adequate blood pressure control (with readings below 140/90 mm Hg). Susceptibility Of Clinical Isolates Of Bacteria To Cefamandole, Cefoxitin And Cephalothin, R. Del Busto, A. Suarez, E. Quinn, D. Pohlod

The in vitro susceptibility was determined of 274 isolates to cephalothin and two new antibiotics, cefamandole and cefoxitin. Cefamandole was comparable to cephalothin in preventing growth of cultures of the gram positive organisms except for penicillin-resistant Staphylococcus aureus which was more sensitive to cephalothin. Cefamandole was more active than cephalothin against all the gram negative bacteria including Haemophilus influenzae and in addition it was active against many strains of Enterobacter sp. Cefoxitin was less active than cephalothin against the gram positive organisms but it was more active against most of the gram negative bacteria. In addition. It was active against …

Acute cerebellar encephalopathy is a rare neurological disorder associated with neuroblastoma and is characterized by ataxia, somatic myoclonus and opsoclonus. The pathogenesis which underlies the relationship between the two conditions is obscure. One patient with neuroblastoma and acute cerebellar encephalopathy Is reported. The occurrence of neuroblastoma and acute cerebellar encephalopathy In this case may be coincidental but, on the basis of frequent recent reports of this association, a possible relationship between these two entities is entertained. It is therefore recommended that all patients with acute cerebellar encephalopathy be surveyed for unapparent neuroblastoma. The search should include chest and skeletal X-rays, …

The immunological approach to neoplastic disease is gaining favor because malignant tumors are associated with immunodeficient states. Mesenchymal tumors have been seen in renal allograft recipients. The discovery of carcinoembryonic antigen, alfafetoprotein and other protein antigens associated with malignant diseases are other factors. Spontaneous regression of malignant neoplasm has been attributed to an effective immunologic process in the host. Identification of tumor specific antigens may explain these spontaneous tumor regressions. It may not be too optimistic to expect tumor vaccine of some sort within the next few years. B.C.G. and other nonspecific immunostimulants are under investigation for treatment of malignant …

Productivity Of The Roll-Streak Method To Perform Anaerobic Bacteriology In The Routine Clinical Laboratory, Thomas R. Neblett

Data presented are derived from one year of performing routine clinical anaerobic bacteriology by the roll-streak tube method. 826 specimens from 1508 (54.8%) contained anaerobes. 162 specimens were polymicrobic (19.6%) and contained 528 strains of a total of 1051 isolated (50.2 %). 479 blood specimens contained 525 strains of anaerobes. Propionibacterium, considered a contaminant, occurred 209/210 times in blood as the sole isolate. Bacteroides occurred alone or polymicrobically 308 times and comprised 54.7% of the blood isolates and 32.5% of the total anaerobic genera isolated. Clostridium, Peptococcus and Fusobacterium were the next most frequently occurring anaerobes. The roll-streak method is …

A young woman was found to have persistent Cushing's syndrome one year after surgical removal of both hyperplastic adrenal glands. Improvement followed pituitary irradiation butshe developed skin pigmentation. Remission of the adrenal hyperfunction occurred only after several months of therapy with o,p'-DDD. Within 15 months, the recurrent disorder required a second course of DDD with resulting adrenal insufficiency. Eighteen months later, however, steroid supplements were no longer required and increasing Cortisol levels indicated still another imminent relapse. There was no enlargement of the sella turcica. The patient had received no treatment whatever for nearly nine months when she experienced a …

Retrograde endoscopic pancreatic duct cannulation for evaluation of patients with chronic pancreatitis is reviewed over the last two years. Of 40 patients that were endoscoped, 30 had a successful pancreatogram. The X-ray findings fell into three groups: 1) single and multiple stenosis, 2) diffuse narrowing, and 3) apparently normal duct. This procedure proved of value in ruling out or delaying surgical operation in 10 persons. Twenty-one patients underwent operations. Thirteen had a 95% pancreatectomy, one had drainage of a necrotic collection thought to be a pseudocyst and one had a pseudocyst resected. One had a gastrojejunostomy and vagotomy bypassing a …

Multiple isolations of T. Cutaneum from the blood stream of two patients enabled the authors to characterize the morphology, biochemical reactions and antibiotic susceptibility of T. Cutaneum. This organism appears to be an opportunistic pathogen and responds promptly to intravenous Amphotericin B or oral 5-Fluorocytosine.

Bupivacaine As A Local Anesthetic For Hernia Repair, Joseph L. Ponka, James A. Sapala

Bupivacaine hydrochloride is a safe, useful local anesthetic agent for repair of groin hernias. Prolonged anesthesia, lasting six hours or longer, was observed In 94 of WO patients. No toxic effects involving the central nervous system were observed using a 0.25% solution with epinephrine. Hypotension and cardiac arrhythmia occurred in one patient. Wound complications, such as hematomas and ecchymosis, were more common with Bupivacaine than when Chlorprocaine was used. However, painstaking hemostasis, which required ligation of even the tiniest blood vessels, reduced the wound complications. As experience is gained in the use of Bupivacaine, complications will be reduced to a …

Patterns Of Fatty Acid Release From Endogenous Substrates By Human Platelet Homogenates And Membranes, A Derksen, P Cohen

We describe a method for measuring the release of fatty acids from endogenous substrates of human platelet homogenates and membranes. The method depends on the availability of lipids whose fatty acids are odd-chained and therefore suitable as internal reference compounds that, at the time of lipid extraction, can be added to an incubation to permit subsequent quantification of the content of free fatty acids or fatty acids esterified to specific lipids. We found four types of lipolytic activities in human platelets. In homogenates at pH 4.0 a triglyceride lipase operated as shown by the synchrony of triglyceride degradation and release …

Lymphangiography In Staging Carcinoma Of The Prostate. A Comparison With Operative Findings, Joseph C. Cerny, Malcolm Weckstein, Riad Farah

Pedal lymphangiography was performed In 30 patients who subsequently underwent radical retropubic prostatectomy, and radical pelvic lymphadenectomy. In 24 of 30 patients, lymphangiograms correctly predicted the presence or absence of nodal Involvement. In spite of certain limitations of the technique. It is considered an essential diagnostic study in patients considered for possible radical prostatic surgery, or in patients who may be candidates for external megavoltage radiotherapy to the prostate gland and its lymphatic drainage.

Ten patients with inoperable prostatic carcinoma, producing ureteral obstruction and azotemia, underwent palliative urinary diversion. Good quality survivals of two months to three years were obtained. These results compare favorably with the good results seer) In gynecologic neoplasm of the bladder, colon and breast. In selected cases of carcinoma of the prostate, palliative urinary diversion is indicated when the prospects exist for further benefit from chemotherapy or radiotherapy.

An operative technique Is described which combines regional pelvic lymphadenectomy with radical prostatectomy. The technique has been used on 30 patients with no operative mortality. Morbidity has been within acceptable limits. Thus far, only one patient has shown recurrence of the neoplasm.

Basic principles of radiation biology and radiation tolerance are reviewed. The Implications for radiation therapy of newer staging techniques in carcinoma of the prostate such as exploratory laparotomy, lymphangiography, and bone marrow acid prosphatase are discussed. A technique for treating the prostate and para-aortic lymph nodes in continuity is presented. Complications and results of therapy are discussed.

Radionuclide bone imaging is a rapid, easily performed, noninvasive method for evaluation of skeletal disease in carcinoma of the prostate, or any other malignancy. In a significant number of cases, it may provide the only evidence of metastases and frequently demonstrates more widespread Involvement than previously suspected. Thus, it may redirect the chosen course of therapy or localize sources of bone pain that are not radiographically apparent. Sixty-two percent of 42 patients studied were abnormal, of which 12% had no radiographic evidence of disease.

Based on experience with 30 men having clinical stage A and B adenocarcinoma of the prostate, the authors conclude that elevation of bone marrow acid phosphatase (BMAP) is a sensitive indicator of the disease state. BMAP revealed periprostatic or nodal Involvement when osseous metastases seemed absent. BMAP is considered essential in the evaluation of all patients with prostatic carcinoma. Patients who have false negative BMAP (and are not candidates for radical prostatectomy), should undergo lymphangiography and bone scan before radiation therapy or hormonal treatment is planned.
