The Incidence Of Retrolental Fibroplasia, R. Michael Nisbet

With the increased use of oxygen to treat the respiratory distress syndrome, retrolental fibroplasia was encountered in more infants. One hundred consecutive premature infants were screened for ocular disease and a diagnosis of retrolental fibroplasia was made in six infants. A profile of a "high risk" infant emerged as one with a birth weight less than 1,500 grams who suffers from the respiratory distress syndrome and multiple apneic spells. During the examinations many other ocular abnormalities were found which prove the value of such screening.

X-linked cataracts are a rare group and appear as a complication of other disease. Changes in the lens are difficult to visualize and the family history may alert the examiner to the possibility of cataractous changes. This paper is a review of 13 sex-linked conditions with cataract as a possible complication. Cataracts may be present in an occasional case or in some instances in every case. Clinical Uses Of Biofeedback: A Review Of Recent Research, Richard M. Lee, Scott E. Baldwin, Julia A. Lee

Biofeedback refers to a special type of operant conditioning (ie, a category of learning) in which subjects acquire control over physiological processes through the feedback of information. Its methodology is based upon principles of behavioral science developed in the laboratory through experimentation. It has been conclusively demonstrated that humans (and other animals) can learn to control such actions as EKG parameters, blood pressure, body temperature, EEG, individual motor units, and glandular secretions. Clinically, biofeedback has been applied to a wide variety of psycho-physiologic disorders in which a measurable response requires alteration. The best established applications are tension and migraine headaches, …

Intraoperative transanal fiberoptic colonoscopy is presented as an alternative to transcolonic procedures to remove colonic polyps, or to excise or biopsy other intraluminal lesions. This method should be considered when the standard transanal colonoscopy has failed. The results with this procedure have been excellent. The method also can be applied to patients who have colonic lesions and are to undergo elective procedures such as hysterectomy and cholecystectomy.

Early Vagotomies At Henry Ford Hospital: An Historical Vignette And A Follow-Up, James C. Gruenberg, Conrad R. Lam

Over a one-year period beginning in April, 1946, nine transthoracic vagotomies were done at the Henry Ford Hospital. Three of the patients had had a partial gastrectomy previously and the operation was done for recurrent bleeding from marginal ulcers. Thirty years later, an attempt was made to ascertain the present condition of these patients. Long term follow-ups were possible on six patients and follow-ups of one, six and 14 years on the others. All nine patients had immediate relief of ulcer pain and cessation of bleeding if present. No subsequent drainage operation was required for the six patients with primary …

Hereditary spastic paraplegia was diagnosed in 19 patients belonging to four families. All had spastic gait and upper motor neuron signs in lower limbs. In family A, the disease was expressed in this manner, although one member had muscle wasting in an upper limb. There was additional involvement of the posterior columns in six of the eight affected members of family B and cerebellar ataxia was noted in upper limbs of three of them. Technetium 99m pyrophosphate (99mTCPyP) accumulates in recently infarcted myocardium and can be detected by external imaging techniques. This study was performed to evaluate the ability of this isotope to identify the presence of acute myocardial infarction. In 82 patients admitted to a coronary care unit with chest pain of varying etiology, scan was positive in all 13 patients with acute transmural myocardial infarction and in 23 of 27 patients with nontransmural myocardial infarction. The scan was negative in 37 of 42 patients without evidence of recent infarction. Four of the remaining five patients in this group had unstable angina pectoris. …

In a review of 68 patients with dissecting thoracic aorta aneurysm (DTAA), eight patients (11.7%) were found to have painless dissection. Three of the eight patients presented with gross aortic insufficiency (Al). Angiography on a fourth patient, who had severe hypertension, showed Type III DTAA. Three other patients had either dilatation of the ascending aorta or a possible mediastinal mass, according to chest roentgenograms. One patient received treatment for suspected myocardial infarction with cardiogenic shock. Technedum 99m stannous pyrophosphate (TcPyP) myocardial scans were obtained in 42 selected nonconsecutive patients within 12 days following open heart operations for both valve replacement (19 patients) and coronary bypass (23 patients) procedures. The scans were correlated with elevation of serum lactic dehydrogenase, glutamic oxalacetic transaminase and the electrocardiograms obtained serially in the postoperative period. TcPyP scan was more specific than enzyme studies and it was more sensitive than the electrocardiogram. A logical method of repair of esophageal hiatus hernia based on anatomic principles was proposed nearly 25 years ago by the late Phillip Allison. There was general agreement that his operation had advantages, but because of a disappointing number of recurrences and problems with reflux, more complicated operations were devised. Some of these involved plication of the stomach around the lower esophagus (Nissen, Belsey). In a series of 562 operations for hiatus hernia at the Henry Ford Hospital, there has been no significant deviation from the Allison technic. Susceptibility At High Inoculum Of Penicillin-Resistant Staphylococcus Aureus To Six Cephalosporins, Andres Suarez, Ramon Del Busto, Edward L. Quinn, Donald Pohlod

Six cephalosporins (currently available for clinical use or undergoing clinical trials) were tested by tube dilution method against high inoculum (107 colony forming units) of 37 strains of penicillin resistant Staphylococcus aureus. 3.1 μg/ml of cephalothin inhibited 100% of strains while higher concentrations of cefoxitin, cephapirin, cefamandole, cephalexin and cephradine were required. The percent of strains with minimal inhibitory concentration higher than 3.1 μg/ml ranged between 10.8% (cefoxitin) and 100% (cephradine). These findings support the observation that cephalothin is less susceptible to inactivation than the other cephalosporins. A case of miliary tuberculosis complicated by respiratory insufficiency and disseminated intravascular coagulation was treated successfully with antituberculosis drugs, ventilatory support, and heparin, packed red cells, platelet transfusions and plasma. Tuberculosis is a rare cause of disseminated intravascular coagulation.

A technique for treating the para-aortic nodes to a high dose of irradiation in continuity with the pelvis has been developed at Henry Ford Hospital and recently described by one of the authors. The "nine-field technique" was initially used in late 1973 to treat two patients with advanced prostatic carcinoma who had grossly involved para-aortic nodes. Since that time, 16 additional patients with carcinoma of the prostate, one patient with carcinoma of the urinary bladder, and one patient with carcinoma of the cervix have been treated by this technique. This study presents the clinical features, roentgenographic manifestations, pathological features, abnormal laboratory studies, treatment and prognosis of 157 cases of osteogenic sarcoma. The series is the fifth largest ever reported. The authors favor prompt surgical ablation followed by controlled intensive chemotherapy as a prophylactic or combined chemotherapy and radiation therapy for metastatic osteogenic sarcoma. All case reports but one present rare or unusual situations.

Hemodynamic and angiographic findings are described in a 27-year-old patient, who had undergone surgical operation for subvalvular membranous aortic stenosis. Left ventriculography revealed persistence or regrowth of the subvalvular fibrous ring, and hemodynamic data revealed associated dynamic obstruction of the left ventricular outflow tract. The authors emphasize the importance of uncovering associated dynamic obstruction by provocative maneuvers and use of Beta blockers.

Prolonging Oxygen Consumption During Preservation Of Canine Kidneys By The Addition Of Continuous Dialysis, Daniel Dall'olmo, Stanley G. Dienst

Kidneys were preserved by continuous pulsatile perfusion and their metabolic rate monitored by O2 consumption. Experiments were carried out to determine if dialyzing the perfusate to supply metabolic substrates would aid in kidney preservation. A control group was compared against two experimental groups at 25°C. Average preservation time before fall off of oxygen consumption differed for each group: control - 8 hours, dialysis with amino acids - 6 hours, dialysis with amino acids and cofactors - 11 hours. Results indicated that, if suitable concentrations of metabolic substrates and cofactors were used, the preserved kidneys maintained higher levels of O2 consumption …

Oral Contraceptives And Myocardial Infarction: Report Of Three Cases And Review Of The Literature, Mohsin Alam, Remigio Garcia, Ellet H. Drake

Three cases are presented of young women who had myocardial infarctions possibly due to coronary thrombosis related to oral contraceptive therapy. These patients had none or very few risk factors for coronary artery disease. In two of the patients, selective coronary arteriograms showed normal results on the third and tenth week after the diagnosis was established. The pathogenesis of coronary artery thrombosis in patients receiving oral contraceptives containing estrogen is still controversial, but it appears to be the result of its multifactorial effect on the platelets, coagulation factors, vessel wall, blood pressure, postprandial blood sugar and abnormalities of lipid metabolism.

This is the second of two articles on the Henry Ford Hospital screening, referral, and follow-up program for high blood pressure. The first reported on the screening results of the program, along with associated epidemiological findings. This article presents the results of referral and follow-up. During the period of March 27 through December 11, 1975, 808 people were screened; 196 (24%) were found to have uncontrolled high blood pressure and were referred to physicians for diagnosis and possible treatment for hypertension. The management of penetrating abdominal wounds has become a necessity in most larger metropolitan hospitals. Most of these wounds are produced by low velocity gunshots, but some are stab wounds. Several recent advances have contributed to the more successful management of penetrating abdominal wounds. These include: 1) improvement in on-site care by various emergency medical services, 2) more rapid transportation of the injured to a facility equipped to care for such patients, 3) vigorous resuscitative and diagnostic measures, 4) prompt and adequate surgical intervention, 5) improvements in anesthesia techniques, and 6) improvements in blood banks. This paper introduces the use of the Weibull distribution function for the analysis of small samples of clinical data. It is compared with the use of the conventional analysis of variance in determining the results of a study comparing the use of gold salts and a placebo in the treatment of rheumatoid arthritis. Although the samples were small, 14 control patients and 13 who were treated, analysis of variance determined several significant differences between the two groups.
