Community pharmacists are among the most accessible healthcare providers in the United States and play an increasingly important role in improving patient outcomes. This study observes whether geographical distance and demographic differences influence patient healthcare needs within community pharmacy settings. Communities in Wisconsin and Mississippi, approximately 800 miles apart, were compared due to differences in population demographics, socioeconomic factors, and disease burden.

A survey was distributed to community pharmacy employees, including pharmacists, pharmacy interns, and pharmacy technicians, to assess common disease states, medication patterns, pharmacy workload, and patients' barriers to care. Twelve responses were collected from pharmacy employees.

Community pharmacies in the United States (US) face an increasingly unsustainable future due to declining third-party reimbursement (remuneration) and ongoing cash flow challenges following the elimination of retroactive direct and indirect remuneration (DIR) fees. These pressures have contributed to widespread pharmacy closures, the emergence of pharmacy deserts, and reduced access to care for millions of patients. Despite these challenges, community pharmacy remains the most common employment setting for pharmacy school graduates in the US. Long lines at the pharmacy can frustrate patients. Consequently, patients becoming frustrated can add stress to an already overworked pharmacy. Without knowing what patients understand about the often complicated prescription filling process at community pharmacies, it can be difficult for pharmacists and pharmacy technicians to communicate the reason behind any delays in patients receiving their medication. To learn what patients understand about the prescription filling process, fifteen interviews were conducted over the phone after mailing recruitment flyers to members of a diabetes research collaborative in North Mississippi. This paper proposes that co-located retail clinics (RCs) and community pharmacies can increase opportunities to provide more accessible, affordable, and patient-friendly primary care services in the United States. RCs are small businesses of about 150-250 square feet with a clientele of about 10-30 patients each day and most frequently staffed by nurse practitioners (NPs). Community pharmacies in the U.S. at ~67,000 far outnumber RCs at ~2800, thereby opening substantial opportunity for growth.
