Many studies have investigated the effects of energy drinks on cardiovascular parameters. These studies were typically conducted with high volume (32 ounces) energy drinks and have demonstrated association with QT prolongation and blood pressure elevation after consumption. Currently, there is inconclusive evidence with lower volume energy drinks. This study intends to evaluate the effects of the largest commercially available energy drink can (24 ounces) in the United States on cardiovascular parameters compared with placebo arm. The hydrophobicity of many chemotherapeutic agents usually results in their nonselective passive distribution into healthy cells and organs causing collateral toxicity. Ligand-targeted drugs (LTDs) are a promising class of targeted anticancer agents. The hydrophilicity of the targeting ligands in LTDs limits its nonselective passive tissue distribution and toxicity to healthy cells. In addition, the small size of LTDs allows for better tumor penetration, especially in the case of solid tumors. Benzo[a]pyrene (BaP), is one of the most potent mutagens and carcinogens known. It requires metabolic activation through cytochrome P450 (CYP)1A1 to yield the ultimate carcinogenic metabolite, benzo[a]pyrene-7,8-dihydrodiol-9,10-epoxide (BPDE). BPDE can bind to DNA and form predominantly covalent (+) trans adducts at the N2 position of guanine causing DNA damage. Rutaecarpine (RTC) is an herbal medicine that has been used to treat several diseases such as headache, hypertension, gastrointestinal disorders, amenorrhea, and anti-inflammation. Pharmaceutical expenditures are an important part of the entire hospital operating budget, and inpatient pharmaceuticals denote one of the highest costs in hospital care. Predictions for medication budgets based on the types of patients have been largely undertaken in medical hospitals and not psychiatric facilities. According to several previous studies, gender, age, diagnosis, comorbidity and length of stay (LOS) affect the general inpatient treatment expenditures. State legislation focused on promoting and supporting breastfeeding is relatively new, beginning in the 1990s. The purpose of this study is to determine if there is an association between state breastfeeding rates and state breastfeeding legislation and other institutional policies. The Centers for Disease Control and Prevention releases breastfeeding report cards with the most up-to-date state breastfeeding rates. These reports are used to examine the relationship between state breastfeeding rates and number of breastfeeding policies passed by a state. Coral reefs are important and diverse marine ecosystems, and sponges represent the majority of species in Caribbean reefs. Xestospongia muta is a giant barrel sponge that is one of the most abundant and visible sponges on Caribbean reefs, and has been the subject of many studies. Xestospongia muta produces a diversity of secondary metabolites that have various ecological functions including allelopathy, anti-fouling, predation deterrence, and antibacterial activity. Little is known about the interaction between diabetes and sex in vasculature. This study was designed to investigate the effects of estrogen as well as type 2 diabetes (T2D) on aortic function in rats with respect to sex. To test the effects of T2D and sex, UC Davis Type 2 Diabetes Mellitus (UCD-T2DM) rat model was used. Angiogenesis plays important role in tissue regeneration. Biomaterials that allow and promote angiogenesis are thus beneficial. Silk fibroin (SF) scaffolds are good candidates for angiogenesis activation because of the biodegradability, biocompatibility, and interaction of extracellular matrices. Human adipose-derived stem cells (ADSCs) are also promising for tissue regeneration. The objective of this study was to evaluate and compare the antioxidant activity and tyrosinase inhibition activity of Murraya paniculata (Linn.) Jack leaves extract (MPE) obtained from different extraction solvents for the use of cosmetic purposes. The result showed that 75 % ethanolic extract presented the highest antioxidant activity (IC50 = 1.43 mg/mL), the total phenolic content of 66.12 mg GAE/g and the total flavonoids content of 7.83 mg QE/g. The purpose of this study was to develop a stable niosomal eye drop containing angiotensin converting enzyme (ACE) inhibitor for lowering intraocular pressure. Three ACE inhibitors i.e., captopril, quinapril and fosinopril (FOS) were used in this study. Their inclusion complexes formation with parent CDs i.e., alphaCD, betaCD and gammaCD were determined. All drugs represented 1:1 stoichiometry inclusion complex with each CD. Of these, FOS/gammaCD complex showed the highest stability constant. Kinetic degradation study confirmed that FOS showed the lowest degradation in the presence of gammaCD. The purpose of this research was to develop and optimize a self-microemulsifying delivery system (SMEDS) to improve dissolution rate of poor soluble compound AST by mixture experimental design. The solubility of AST was analyzed with various excipients so that the appropriate oil, surfactant, and cosurfactant were figured out. Through the findings about the microemulsion existence area, the pseudoternary phase diagrams were constructed for selecting the optimum combination of excipients in a formulation of SMEDS. วัตถุประสงค์: เพื่อเปรียบเทียบผลของการให้บริบาลทางเภสัชกรรมโดยเภสัชกรในผู้ป่วยสูงอายุที่เข้ารับการรักษาตัวในโรงพยาบาลในด้านค่าเฉลี่ยจำนวนปัญหาจากการใช้ยา คะแนนคุณภาพชีวิตด้านการใช้ยา และจำนวนผู้ป่วยที่กลับเข้ารับการรักษาตัวซ้ำในโรงพยาบาลด้วยสาเหตุที่สัมพันธ์กับการใช้ยาภายในระยะเวลา 30 วันหลังจำหน่าย วิธีการวิจัย: การวิจัยเชิงทดลองแบบสุ่มชนิดที่มีกลุ่มควบคุมแบบปกปิดทางเดียว ศึกษาในผู้ป่วยสูงอายุที่เข้ารับการรักษาตัวในหอผู้ป่วยอายุรกรรม จำนวนทั้งสิ้น 192 คน แบ่งผู้ป่วยออกเป็น 2 กลุ่ม คือ กลุ่มศึกษาและกลุ่มควบคุม ผู้ป่วยถูกติดตามตั้งแต่เข้ารับการรักษา ณ หอผู้ป่วยอายุรกรรมจนถึงระยะเวลา 30 วันหลังจำหน่ายออกจากโรงพยาบาล ผลการวิจัย: กลุ่มศึกษาพบค่าเฉลี่ยของปัญหาจากการใช้ยาต่ำกว่ากลุ่มควบคุม (p<0.001) สำหรับคะแนนคุณภาพชีวิตด้านการใช้ยาโดยรวมดีขึ้นกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p<0.05) และอัตราการกลับเข้ารับการรักษาตัวซ้ำในโรงพยาบาลด้วยสาเหตุที่สัมพันธ์กับการใช้ยาภายในระยะเวลา 30 วันหลังจำหน่ายในกลุ่มศึกษาต่ำกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p=0.003) สรุปผลการวิจัย: การให้บริบาลทางเภสัชกรรมโดยเภสัชกรในกลุ่มผู้สูงอายุที่เข้ารับการรักษาในโรงพยาบาลสามารถลดปัญหาจากการใช้ยา เพิ่มคุณภาพชีวิตด้านการใช้ยา รวมถึงลดการกลับเข้ารับการรักษาตัวซ้ำในโรงพยาบาลเนื่องจากสาเหตุที่สัมพันธ์กับการใช้ยาภายในระยะเวลา 30 วันหลังจำหน่ายได้

วัตถุประสงค์ : การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อประเมินเภสัชจลนศาสตร์ประชากรของยาวอริโคนาโซล หาปัจจัยที่มีผลต่อพารามิเตอร์ของเภสัชจลนศาสตร์ และหาขนาดยาที่เหมาะสมในผู้ป่วยเด็กที่มีการติดเชื้อราแอสเปอร์จิลลัสชนิดลุกลาม วิธีการศึกษา : การศึกษานี้เป็นการเก็บข้อมูลย้อนหลังในผู้ป่วย 117 ราย (476 ตัวอย่าง) เภสัชจลนศาสตร์ประชากรวิเคราะห์โดยใช้วิธี non-linear mixed-effect model การหาแบบแผนขนาดยาวอริโคนาโซลถูกประเมินจากผลการรักษาโดยใช้เป้าหมาย fAUC24/MIC ด้วยวิธีการจำลองแบบมอนติคาร์โล ผลการศึกษา : แบบจำลอง two compartment model with first-order absorption, linear elimination, and allometric scaling มีความเหมาะสมกับข้อมูลของการศึกษานี้ การวิเคราะห์ปัจจัยที่มีผลต่อพารามิเตอร์เภสัชจลนศาสตร์พบว่า น้ำหนัก และ alkaline phosphatase (ALP) มีผลต่อค่าการขจัดยา ผลการจำลองแบบแผนการให้ยาวอริโคนาโซลด้วยวิธีมอนติคาร์โล พบว่า ขนาดยาที่แนะนำในปัจจุบันมีประสิทธิภาพทำให้ผลการรักษาถึงเป้าหมาย ยกเว้นในกลุ่มผู้ป่วยที่ ALP ปกติ และมีน้ำหนักน้อยกว่า 10 กิโลกรัมหรือน้ำหนัก 50 กิโลกรัมขึ้นไป สรุปผลการศึกษา : การศึกษาครั้งนี้แสดงให้เห็นว่าการวิเคราะห์ทางเภสัชจลนศาสตร์ประชากรและการจำลองแบบมอนติคาร์โล สามารถนำมาใช้กำหนดแบบแผนการให้ยาวอริโคนาโซลได้

วัตถุประสงค์: เพื่อเปรียบเทียบประสิทธิผลของขนาดยากลุ่มยาปิดกั้นตัวรับชนิดเบต้า ได้แก่ ไบโซโปรลอล และคาร์วีไดลอล ในขนาดยาสูง (ร้อยละ 50 ถึง 100 ของขนาดยาเป้าหมาย) กลาง (ร้อยละ 25 ถึง 50 ของขนาดยาเป้าหมาย) และต่ำ (น้อยกว่าร้อยละ 25 ของขนาดยาเป้าหมาย) ต่อผลลัพธ์การเสียชีวิตจากทุกสาเหตุ ผลลัพธ์รวมของการเสียชีวิตจากทุกสาเหตุและการเข้ารับการรักษาในโรงพยาบาลจากทุกสาเหตุและผลลัพธ์ในการเข้ารับการรักษาในโรงพยาบาลจากโรคหัวใจและหลอดเลือดในผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรังในระยะเวลา 1 ปี วิธีการดำเนินการ: การศึกษาวิจัยเชิงวิเคราะห์จากเหตุไปหาผลแบบย้อนหลัง โดยเก็บข้อมูลย้อนหลังจากเวชระเบียน ในผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรัง (ICD10: I500) ที่เข้ารับการรักษาในแผนกผู้ป่วยนอก โรงพยาบาลมหาราชนครศรีธรรมราช ระหว่างวันที่ 1 มกราคม พ.ศ. 2556 ถึง 31 ธันวาคม พ.ศ. 2561 และได้รับยาปิดกั้นตัวรับชนิดเบต้า ในขนาดคงที่ต่อเนื่องอย่างน้อย 6 สัปดาห์ติดตามผลผู้ป่วยเป็นระยะเวลา 1 ปี ผลการวิจัย: จากผู้ป่วยที่ผ่านเกณฑ์ 542 ราย มีเพียงร้อยละ 3.3 ที่ได้รับยาปิดกั้นเบต้าในขนาดยาเป้าหมาย ส่วนใหญ่ได้รับยาปิดกั้นตัวรับชนิดเบต้าในขนาดต่ำ (286 รายหรือร้อยละ 52.7) พบว่ากลุ่มผู้ป่วยที่ได้รับยากลุ่มปิดกั้นตัวรับชนิดเบต้าในขนาดสูง (87 รายหรือร้อยละ 16.1) เกิดผลลัพธ์รวมการเสียชีวิตและเข้ารับการรักษาในโรงพยาบาลจากทุกสาเหตุในระยะเวลา 1 ปี น้อยกว่ากลุ่มขนาดยาต่ำอย่างมีนัยสำคัญทางสถิติ (HR=0.63, 95%CI 0.41–0.95, p=0.03) กลุ่มขนาดยาสูงมีแนวโน้มเกิดผลลัพธ์การเสียชีวิตจากทุกสาเหตุและผลลัพธ์การเข้ารับการรักษาจากโรคหัวใจและหลอดเลือดน้อยกว่ากลุ่มขนาดยาต่ำเช่นเดียวกัน แต่ไม่มีนัยสำคัญทางสถิติ สรุป: ผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรังที่ได้รับยากลุ่มยาปิดกั้นเบต้าในขนาดยาสูงเกิดผลลัพธ์รวมการเสียชีวิตและเข้ารับการรักษาในโรงพยาบาลจากทุกสาเหตุต่ำกว่าที่ได้รับยาในขนาดยาต่ำ ดังนั้นยากลุ่มปิดกั้นเบต้าควรได้รับการปรับขนาดยาเพิ่มขึ้นจนถึงขนาดยาเป้าหมายหรือขนาดยาสูงสุดที่สามารถทนได้หากไม่มีข้อห้ามใช้ เพื่อประสิทธิผลสูงสุดจากยา

ผลลัพธ์ทางคลินิกของการบำบัดภาวะลิ่มเลือดหลุดอุดหลอดเลือดด้วยเฮปารินที่มีน้ำหนักโมเลกุลต่ำในผู้ป่วยมะเร็งชาวไทย, วรรณวัฒน์ สินเจริญ

การวิจัยนี้เป็นการวิจัยเชิงพรรณนาแบบย้อนหลัง เพื่อศึกษาความสัมพันธ์ของขนาดการใช้ยา LMWH กับผลลัพธ์ทางคลินิกด้านประสิทธิภาพซึ่งประเมินจากอาการแสดงทางคลินิกและผลภาพถ่ายทางรังสีวิทยาของหลอดเลือด และด้านความปลอดภัยซึ่งประเมินจากการเกิดเลือดออกในผู้ป่วยมะเร็งที่มีภาวะ VTE รวมถึงศึกษาปัจจัยอื่นที่อาจมีผลต่อผลลัพธ์ทางคลินิกข้างต้น เช่น เพศ อายุ ระยะของโรคมะเร็ง เป็นต้น โดยเก็บข้อมูลจากผู้ป่วยที่มารับการรักษาที่ศูนย์ให้ยาเคมีบำบัดแบบผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ตั้งแต่เดือนมกราคม 2557 ถึงกุมภาพันธ์ 2562 จำนวน 140 ราย ผลการศึกษาพบว่าขนาดการใช้ยา enoxaparin มีความสัมพันธ์กับผลลัพธ์ทางคลินิกทั้งด้านประสิทธิภาพและความปลอดภัย พบว่าขนาดยาสูงกว่ามาตรฐานทำให้ผู้ป่วยมีอาการทางคลินิกดีขึ้น (ร้อยละ 100) มากกว่าขนาดมาตรฐาน (ร้อยละ 90.4) และขนาดต่ำกว่ามาตรฐาน (ร้อยละ 54.2) (P<0.001) เช่นเดียวกับการประเมินอาการด้วยภาพถ่ายทางรังสีวิทยาซึ่งพบว่าขนาดยาสูงกว่ามาตรฐานทำให้ผู้ป่วยมีผลภาพถ่ายทางรังสีวิทยาดีขึ้น (ร้อยละ 80) มากกว่าขนาดมาตรฐาน (ร้อยละ 77.1) และขนาดต่ำกว่ามาตรฐาน(ร้อยละ 34.0) P<0.001) นอกจากนี้พบว่าผู้ป่วยโรคมะเร็งที่อยู่ในระยะ 2-3 มีอาการทางคลินิกดีขึ้น (ร้อยละ 100) มากกว่าระยะลุกลาม (ร้อยละ 69) (P=0.010) และผลภาพถ่ายทางรังสีวิทยาของหลอดเลือดก็ดีกว่าเช่นเดียวกัน (ร้อยละ 93.8 และ 50.0 ตามลำดับ ,P=0.001) ด้านความปลอดภัย พบว่าขนาดการใช้ยาสัมพันธ์กับการเกิดเลือดออก (P<0.001) ขนาดยาสูงกว่ามาตรฐานทำให้ผู้ป่วยเกิดเลือดออกมากที่สุดร้อยละ 57.1 รองลงมาคือขนาดมาตรฐานและต่ำกว่ามาตรฐานซึ่งเกิดเลือดออกใกล้เคียงกันคือร้อยละ 8.6 และ 9.5 ตามลำดับ และพบว่าเพศหญิงเกิดเลือดออกที่รุนแรง (ร้อยละ 10.8) มากกว่าเพศชาย (ร้อยละ 1.8) (P=0.037)
